Injured left index finger between wear iron.

EE sustained a partial amputation at the distal portion of the left middle finger-(middle of the nail bed) when the finger was caught between two pieces of metal grating. Injury involved loss of tissue and bone at the fingertip. EE received medical evaluation and will received surgical surgery to smooth fingertip

Struck by falling object

A mechanic was cleaning dirt from an oil drain plug using a screwdriver when a piece of debris dropped into their eye, resulting in material becoming embedded. The employee was immediately taken for medical evaluation. The metal fragment was removed, and antibiotics eye drops were administered and prescribed.

An employee experienced a sudden and severe cardiac event while at work. Coworkers immediately called for help and initiated emergency response procedures. Despite rapid assistance from onsite responders and EMS, the employee did not survive. The incident appears to have been a medical emergency unrelated to work activities.

A haul truck was backing into the crusher dump pocket and did not notice a loader already in the dump pocket. The truck's rear tire came into contact with the rear of the loader, jarring the operator in the cab of the loader.

An employee became ill after experiencing irritation from ammonia event. The employee was evaluated and was unable to return to work the following day.

While walking down the haul truck stairs, the operator tripped and fell, resulting in a laceration to the knee.

While setting up a drill rig at a new location, a Driller struck the tip of left thumb with a hammer they were using to install a 4""x4"" piece of wood to the wood deck to help keep the drill on alignment.

Struck by falling object

A Fixed Maintenance employee assisting with re-installation of a return roller for a conveyor belt. While lifting roller back into position, the supporting bracket weighing 7lbs - 10lbs slipped out of place and fell approximately 29 inches. Bracket struck the back of the employee's left hand resulting in a fracture of the 4th finger on the left hand.

Accident type, without injuries

Parked/unattended CAT D10 dozer was positioned on the coarse ore stockpile. The material beneath and around the dozer gave way, causing the machine to slide down into the coarse ore stockpile feeder area. At the time of the incident, no operator was in the cab, and no personnel were in the immediate vicinity of the sloughing event. No injuries reported.

EE was attempting to remove a drill bit from a drill basket when it became jammed. When the bit finally came loose, it struck the employee's right index finger, resulting in a chipped fracture and laceration that required stitches.

EE was attempting to remove a fuel filter from the 789 CAT haul truck when, using exertion, the filter broke loose. This sudden movement caused pain in the employee's right shoulder. An MRI was performed and results confirmed on 1/8/25 a torn rotator cuff. EE was issued restricted duty.

As Contractor security officer was walking down handicapped ramp, they stepped off the bottom of the metal ramp onto the gravel and rolled their ankle and fell back onto their tailbone.

EE was pulling under the lime silo at the final loadout. When the operator pulled under the lime silo, the front of the truck bed contacted the dispensing sock creating a cloud of lime in the air. The operator had their window down about 6 inches, and the lime blew into the cab, onto the operator's face, and into the left eye. antibiotic ointment was prescribed

EE was working near a conveyor belt when they tripped over their own feet which resulted in a fall towards the conveyor belt. During the fall, the EE's right arm made contact with the belt and went between the belt and the roller. This incident resulted in a fracture to the EE's right arm. The E-stop pull cord was activated stopping the conveyor.

While employee was walking from work area, they observed that the area needed to access had icy ground conditions. In an attempt to navigate through the ice, employee proceeded to shuffle slowly. Employee lost footing and fell backwards resulting in a fractured patella.

Injured EE without Fall Protection and the knowledge of the crew members, moved to the second deck and fell through an open hole used to lift items to the second deck. Fell to the ground with damage to EE's pelvis, Left Shoulder and ribs.

A haul truck driver, was standing on the emergency ladder platform located on the lower deck of a haul truck. EE was attempting to reach the air filter when they lost footing and fell approximately 4 to 5 feet to the ground. As a result of the fall, they suffered a fractured tibia.

Struck by flying object

An EE was assisting a welding with grinding operations on equipment. Shortly after the work was conducted, the employee experienced irritation in the right eye. EE was seen in a clinic and it was determined that no foreign object was present in the eye, but EE did have an abrasion. The employee was prescribed antibiotic ointment to address the irritation.

Contact with electrical current

Employee was working on electrical line that EE believed to be dead and grounded. EE came into contact with a live electrical line and received a shock. Exam from the hospital found no heart damage but hands received 1st and 2nd degree burns.

an EE was stepping off a loader and accidentally stepped on a rock, causing them to fall backward and land on their back, resulting in an injury to their right shoulder.

An employee was moving a fire hose cabinet from a skip pan. Skip pan was also loaded with uni strut frames at the time. The employee did not see a unit strut frame leaning on and behind the cabinet. While moving the cabinet, the unit strut frame struck employee's finger.

The employee was in the process of removing a stuffing box from a vertical pump. The employee lost the grip with the right hand and tried to grab it as it was falling. The stuffing box flange crushed two fingers on the right hand between the concrete below.

Employee was pulling wire. Employee overexerted self resulting in a hernia.

Over-exertion in lifting objects

Employee was working in the drill Conex, placing drill bits into the designated bin. After task was completed, employee noticed some discomfort in EE's abdominal area. After seeking medical evaluation, employee was diagnosed with an abdominal hernia. Employee stated that EE was not sure if it happened at work or home.

Fall from machine

While miner was entering light vehicle, miner's right foot slipped on running causing miner to fall down backwards. Miner landed on left leg causing injury to the left knee.

Employee was using a Grinder to cut flat stock backing strip, had both hands on the grinder when the grinder kicked back and EE's left hand slipped and cut EE's left index finger. Strip was on the ground when cutting.

Employee slipped and fell while pulling out fuel hose to fill up a generator landing on EE's right thumb/hand.

Leaving site due to poor weather. Complaint of dust in Left Eye during Inclement weather, High winds, Dust, Snow. Eye flushed. Went into doctor on EE's own and was prescribed a prescription.

Employee was setting the brake on machine, the holding clamp was in the upright position. The Cutter head moved into the open clamp causing the clamp to fall striking employee's right hand on the middle and ring finger breaking the tip of the middle one and lacerating the middle and ring finger that required stitches.

Employee was attempting to break a plug loose on a final differential on the 692 excavator, when the plug broke loose the employee felt a sharp pain in the right shoulder.

A miner operating a 472 haul truck (CAT 789 model) was exiting the cab, the operator's heel hooked on the door seal causing EE to fall backwards, hitting left elbow and forearm on radiator. EE was taken to the local hospital for further evaluation; where it was found that EE sustained a fracture to the left Ulna proximal to the elbow.

Employee was performing a routine clean up task and started experiencing pain in right elbow. After a few months the employee was still experiencing pain. At that time we decided to take the employee to an orthopedic center where employee was diagnosed with tendinitis on 1/19/2022.

While trying to reposition haul truck drive shaft, the shaft slipped, pinching the finger between shaft and yoke on the transmission.

Employees had rigged up four (4) Liners with separate rigging to hoist out of the hopper. One of the liners came taut with the crane earlier than the others which rotated the liner that hit the employee's ankle.

Struck against stationary object

EE was attempting to loosen brake cartridge with a wrench. When the cartridge broke loose, EE's hand contacted adjacent metal manifold causing a laceration. EE received 5 stitches.

Employee was attempting to push 24"" pulley onto forklift when it rolled backwards towards EE and EE was caught between that pulley and another one directly behind EE.

Mechanic was doing a PM on a 1500 haul truck and was on the top deck. The heel of the mechanic's boot caught the expanded metal when the mechanic was moving backwards. The mechanic fell back hitting head on the corner of the radiator shroud. The employee had a small gash on the back of head.

A mobile maintenance PM mechanic was walking on a concrete wash pad with a hose in hand when they slipped, landing on their right elbow and straining the right shoulder.

On 7/2/2019 employee twisted ankle while working on leach pad. Minor first aid was given. On 8/21/2019, employee sought medical treatment for a personal issue, upon x-ray found fracture in EE's foot.

Mobile maintenance lube tech was servicing equipment on a drill pattern when EE tripped over a large rock. When the employee tripped, EE landed precariously on EE's ankle, fracturing the left ankle.

During rescue training a team member was pulling on a rope hooked to a basket. When ee felt pain in shoulder. EE didn't want to see a doctor at that time. In August shoulder was still sore ee was seen by a physician and released full duty. In Oct ee still sore & was referred to Ortho for evaluation. An MRI showed rotator cuff requiring surgery Oct 15th 2018 becoming a recordable.

The employee was performing general labor near a conveyor structure. The employee's left hand was on the structure. The employee's left hand slipped and was pinched between the conveyor belt and roller.

At the west stock pile, 993K loader engine compartment caught fire which was eventually then fully engulfed the engine and cab of the loader. A mayday was called, mine rescue team was initiated. Additional resources were called out from lovelock fire department. 2 engines arrived on scene and the mines 20,000 gallon water truck extinguished the fire.

Employee was shoveling rock and dirt under the crusher conveyor guard when they heard a pop in their wrist. Employee continued to work the remainder of the shift as well as the next shift.

Subject stepped out of work trailer onto a loose rock causing the right ankle to roll. Ankle went right, body continued forward resulting in a sprain of the ankle.

While walking between two four-foot culvert pipes and carrying a nuclear test gauge in right hand subject, in anticipation of an uphill step, shifted weight to right leg and the nuke forward creating a rotation on the planted leg which caused EE's patella to dislocate and EE's leg to give out.

A shovel operator was working on the 6575 elevation in the main pit. The shovel was loading haul trucks from a shot dig face and cleaning up larger rocky material off to the right. As the operator was cleaning the floor of rocky material the bucket contacted a hidden unfired booster which detonated breaking the tooth adapter and sent small rock into the excavator.

Employee passed our in front of maintenance shop. It was later determined that the employee suffered from cardiac arrest.

While wearing appropriate PPE, employee was closing a valve that was left opening and was accidently discharging sodium cyanide solution. While closing the valve, gusty wind blew and may have aerosolized the solution contacting the employee's chin, mouth, and nasal passage.

A maintenance mechanic was working in the maintenance shop. He was sitting with his knees tucked under him. When he went to stand up he slid his knees and feet out sideways as he was trying to stand up causing pain in the left knee.

While making rounds on the primary conveyor system, the employee slipped on a wet spot on the B-belt concrete pad. The employee caught himself (did not fall) but hurt his left knee. The employee indicated having a previous injury.

An operator took his pocket knife out and started to cut a zip tie off of two airlines when he cut through the zip tie his knife blade struck his hand which was below cutting the webbing in between the index and thumb.

While de-energizing the breaker to Press Feed B pump in the process east MCC room, a flash over occurred. The flash was contained within the cabinet, but caused the employee to jump back, hitting the right side of his body against the adjacent wall of the MCC room.

An employee was changing out the center roller on a conveyor. As the employee was inserting the roller the belt dropped down onto the roller, pushing and seating the roller shaft into the frame bracket. The employee had his gloved hand near the roller shaft and the very tip of the employees left ring finger was pinched creating an avulsion.

The injured employee lifted a steel plate, which is designed for this use and is standard practice, off of the bed of a tire service truck and walked approx. 30 feet with this plate in his arms and against his midsection to position this plate on the ground to support and stabilize the hydraulic jack which was being set to lift a vehicle to install tires.

Employee was examined by a doctor on 9/27/13 and given a full release. Clinic follow up visit on 10/3/13 supported full release. Employee reported increasing pain, was taken to a doctor on 10/11/13, and placed on restricted duty.

An employee cut his middle left finger on the blower fan in the ceiling vent of a 992 D loader when the vent fell off, requiring stitches.

An employee was descending a ladder when he slipped down and struck his head on a pipe flange requiring stitches. Low level fall.

While replacing a ceramic insulator on a wet electrostatic precipitator (WESP), the insulator slipped and pinched the employee's thumb between the base of the insulator and it's mounting plate.

Employee was removing bolts on cutting edges of a loader with an impact wrench while attempting to hold the nut with his left hand resulting in a laceration to his left palm.

Loading drill pipe to advance hole, ee attached a drill sling to two different rods. He realized the problem and signaled the driller to stop. The driller immediately stopped and began to lower that rod. When the tension released, the rod fell. EE tried catching the falling drill rod. The force of the falling drill rod, fractured ee left four fingers on impact.

An Employee was going to put chock blocks down when they stepped over a trailer hitch catching the toe of their boot on the hitch. The employee fell to the ground contacting their left forehead on the ground, causing a cut.

Filling water truck with water. Notice pain in left hand and it began to swell up. Not sure if it was a bug bite or spider bite. Was taken to hospital to receive medical attention. Returned to work the next shift on regular duty.

Worker was trying to square off the plastic he was cutting. The knife slipped and cut through his glove on his left hand. He cut the palm of his hand resulting in 4 stitches. He returned to work with no lost time.

An EE noticed truck with driver slumped over steering wheel, called EE to see if he had a problem. EE said he was having chest pains. Notified Coeur & needed First Responders immediately. Coeur personnel administered first aid. EE not responsive to medical treatment, so he was transported to Pershing General Hospital in Lovelock, NV.

An employee was hammering a metal lock pin from a shank pin on the back of a dozer, when a small sliver of metal chipped off and entered the top of his left hand median to the thumb and index finger.

The employee was holding a bolt in place using a hammer, while another worker was using an impact wrench to loosen the nut on a bolt, when the employee slipped off of the bolt head letting the bolt drop down while the impact wrench was spinning the nut off the bolt. The bolt pushed down when he slipped and was spinning contacting the employees thumb cutting through his gloves.

An employee was adding drill steel to #260 drill. The employee was using a J wrench to secure the drill steel to break the connection. While installing the J wrench his left index finger came into contact with a burr on the J wrench that cut through his leather glove, requiring 9 stitches.

Crew folding a used track for 10-9-R dozer. The track had a kink in it, it came unkinked, the track dropped about 6"", contacting the bottom rail, which launched a projectile that struck individual in left knee area. He was standing approximately 15-20 feet away from the track removal. Taken to Hospital for medical treatment of cut on left leg-knee area and received stitches.

HE WAS PULLING OUT THE TRANSMISSION GUARD WITH HIS HANDS ON THE GROUND, LOST HIS FOOTING ON THE GROUND & SWUNG THE GUARD WITH BOTH HANDS, TOWARDS THE BACK OF THE TRACKS & PINCHED THE PALM OF HIS RIGHT HAND BETWEEN THE GUARD & THE TRACK.

While the dozer was pushing the windrow down the 2:1 slope, the operator backed up to catch the next windrow, a rock was buried out of sight. When the dozer climbed over it, dozer started to slide sideways & due to nature of the slope, the dozer rolled over. The operator suffered a laceration to his head & was taken to the hospital. Received staples to close the laceration.

On 6/7/07, an employee was shoveling dirt and muddy material near a conveyor, when he felt a sharp pain in his left abdomen.

On 5/22/07, an employee reported that he had gotten dirt blown into his eyes while working out on the heap leach pads. Both eyes were irritated and he was taken into the local clinic for a check up.

A loader operator in the pit was climbing up the stairs of a 992 G loader. As he was climbing up the three stairs he felt a pain in his right bicep.

A maintenance mechanic was installing cutting edges on a dozer. They were using an auto crane to carry the weight of the edge being installed. The employee was using a bar to center up the bolt holes to insert the bolt. As they were aligning the bolt holes with a small bar the employee was pushing it away from his body when he felt a pain in his mid thoracic back.

A Maintenance mechanic was cutting the rear frame of a pickup truck using an oxy/acetylene torch. As he cut the frame, he cut into the gas lines that lead to the gas tank. The tank pushed fire out of the inlet port inflicting first degree burns on his right hand. He as not wearing welding gloves only cotton/leather mixed (regular work glove).

On 9/21/06 an employee was working in a screen deck, driving plugs that hold the screen panels in, with a hammer. As he swung the hammer, it bounced off of an over head tube, hitting his left pinky finger.

On 9/21/06 a blast pattern was detonated in the main pit and seconds later the south high wall just above the blast sloughed down. Appoximately 500,000 tons.

On 8/31/06 an employee was welding a 26 inch long piece of pipe in an upright position, when it fell he tried to catch it and it pinched his finger between another piece of pipe. The right ring finger tip was broken in two places.

On 8/15/06, an employee working in the tire yard was pulling a 51"" tire flange from a leaning 45 degree position, to an upright position, when he felt a pain in his stomach. He initially thought he pulled a muscle and reported it the following day 8/16/06. Physician confirmed on 8/21/06 that the employee has a double hernia.

An employee was servicing #513 dozer when he grabbed onto a sharp piece of metal near the bottom of the dozer. The employee was not wearing his gloves and resulted in a cut to the right middle finger.

An employee was using a wrench and a cheater bar to break a bolt loose. As he applied pressure the wrench slipped off of the bolt resulting in a cut to his right pinky.

The operator of 436 haul truck was in the process of dumping on a waste dump berm when his truck went through the berm rolling twice approximately 70 feet in distance before landing on all four tires. The employee was complaining of pain in the right ribs spreading lateraly to the back. Also a contusion to the right eye and minor cuts and scrapes on the upper extremities.

A tire mechanic was fixing snow chains, when he hit his left thumb with a hammer. The initial doctors visit showed no fractures or dislocations. Upon a follow-up visit (2/14/06) a new x-ray showed there was a frature to the left thumb.

The employee was working on an uneven rocky material. The employee stated she had stepped on a sharp rock and hurt the bottom of her foot. X-ray shows a contusion to #1 and #2 metatarsals on the right foot.

An employee was doing maintenance on a 25 ton crane. While greasing the crane he reached for his grease gun and lost his balance falling from the fender of the crane to a flat surface on the ground, approxiamately 6 foot.The employee fell onto the left foot fracturing both the Tibia and Fibia in the left foot.

The employee was walking on a drill pattern and twisted his left knee. The employee did not want to seek medical treatment. On 6/27/05 the employee stated his knee was bothering him at which time he was taken to PGH and examined by a doctor who refered him for an orthopedic evaluation.

On 5/6/05 a portion of a top soil stockpile had developed cracking near the crest. Barricades and movement prisms were set in place to monitor the crack.The following week 5/11/05 this area was found to have failed.The area below the failure is classified as a wet lands area at no time are people working in this area.

On 4/20/05, an employee was tramming a drill in the pit, when he drove over a rock jarring his back. He did not want to seek medical treatment at that time. In June he stated he wanted to see a physcian. He was referred to a back specialist and on August 17th 2005 he had a minor back procedure performed.

An employee working in the refinery was pushing a leaf filter on the press, back into place when it fell off the slide bar landing on the employee's right wrist. This fractured the Ulna in the right wrist.

Two employees were removing a large rock from a conveyor frame on routine clean up of the belt. As they both lifted the large rock in rolled on to one of the emplyees left pinky. This lacerated the pinky and broke the very tip of the left pinky. The employee was wearing leather gloves.

The employee was walking to a break room and tripped on a rock. As employee was falling he put his hand out to catch him self hit his left wrist on a pallet. Also note a he was wearing a metal wrist watch on that wrist ( possible contributor).

Employee was on the back of a truck. He stepped on the tailgate and it gave way causing him to fall.

Employee stated ""I was walking across the pit pattern and I stepped on a rock. The rock rolled and I twisted my right ankle.""

Employee was cleaning up rock by A-belt when a rock fell off the A belt convyer and struck the employee on the right side of his back.

Report was filed 3/3/04. Ee was seen by a physician & filed a claim. The claim was not accepted at this time. The claim was accepted in late August after several tests. The Loss control supervisor contacted our local MSHA office to find out how to file this correctly. They were unsure & gave me Denver's number & we were given directions today 9/8/04.

He was walking back to the maint. shop and slipped on ice build up on the sidewalk. **Note to file--The injury occurred on 1/20/04 and was not seen until 3/3/04 at which time he was released to work. He had persistant pain and was referred to an orthopedic clinic at which time they determined the extent of the injury and scheduled surgery.**
