The equipment operators were helping with the continuous miner move in the last open cross-cut. The water and power cables that trail the miner were part of the move. The injured employee tripped and struck EE's rt. knee on the uneven ground. Soreness and swelling developed but EE managed to complete EE's shift. An exam occurred. No treatment or RX were required. 7 days off work.

Mine operator arrived at site riding with a co-worker, operator got out of personal vehicle in employee parking and walked to the site Haul truck parking location wearing slides (sandals) not approved PPE, before clocking in, as the employee went over the berm their left leg got stuck in the mud, the operator tripped backwards spraining their Left Knee.

Operator was walking in a muddy area when boot got stuck in mud, EE attempted to pull boot out of mud, the force of the pulling motion caused them to trip backwards and caused a small stress fracture on top of foot, leading to 1 day lost time for swelling and requiring a brace.

EE stumbled on flat ground and landed on the buttocks and felt pain in the left knee. EE reports EE did twist the knee during the fall. Went to the doctor on 12/06/2019, Doctor prescribed a mobilizing knee brace for a hyper-extended knee. No work restrictions.

Loader operator was dismounting the front end loader and fell to the ground when ee lost footing stepping on the last rung of the step falling to the ground and landing on left elbow and lower back.

Employee was operating a dozer in the pit area and stopped to get out. As the employee stepped to the ground from the D-10 dozer steps, he twisted his body slightly and felt an slight pain in his back. The employee reported it to his supervisor. The following morning, the employee reported that his back was sore and he needed a day to rest.

Miner stepped off the man trip at the top of the mine slope, onto a mud clod, causing him to slip/lose his balance and drop to a "3 point stance". On 2-21-07, the miner reported soreness in his left wrist. X-rays were negative on 3-7-07. The doctor did not prescribe or restrict. On 5-1-07 a wrist brace and prescription anti-inflammatory were prescribed.

AS HE WAS CLIMBING DOWN FROM THE CAB OF HIS TRUCK HE MISSED THE BOTTOM STEP AND STRUCK HIS LEFT KNEE ON STEP, LOST BALANCE, STUMBLED AND FELL LANDING ON HIS BACK.
