Drug impaired trespasser entered mine and was throwing rocks at haul trucks from pit slope. Authorities were called, & when PD tried to coax them up out of pit, they chose to head down & tumbled approx 100' down slope wall to pit floor. They then got up and ran before collapsing. FD transported them offsite. Awaiting PD report.

Miner claims to have slipped stepping down an A-frame ladder & injured shoulder at the primary, but wasn't witnessed by those working in the area. Didn't mention it to foreman until 1/5/2021, foreman did not take action as no medical issue was stated. We were notified by the staffing agency of the injury on 1/19/2021.
Miner was descending down a ladder when a foot slipped while they had a grasp of the ladder with left hand. The resulting sudden pull dislocated the left shoulder.
While walking down a set of stairs the miner tripped over EE's own foot and fell stumbling down landing on EE's right knee on the stairs. Injuring the right knee causing a contusion to right knee.
Miner attempted to access the side of the jaw crusher when EE slipped and fell injuring EE's left ankle. Fall protection was in use at the time and the injury resulted in a broken ankle and torn ligament.
Miner was descending down the stairs of 992 front end loader when they slipped on steps slid down 2 steps but remained upright never falling. The steps were wet from the rain and the miner strained their left lower back.

Miner was removing belting from a conveyors return side. When miner went to exit conveyor frame they slipped stepping down and hit right ear, forearm and thigh on conveyor frame. Contusion to head with Avulsion of right ear, contusion with abrasion to right forearm and right thigh.

Miner was inspecting the train before loading. Miner was dismounting off of a piece of equipment when an article of clothing got caught causing the miner to fall. Miner received a cut to elbow and a bruised hip.

Jaw crusher plugged filling the surrounding area with material, EE & 2 workers removed the material they used a crane & sling operation because some of the damaged deck could be a trip hazard they removed 2 sections ee was standing with his back to 1 hole he then turned & lost his balance fell approx. 12' onto his side, ee had a bad cut over his right ear
Employee failed to follow Company procedures for safe access when entering belt press area. Employee removed a removable catwalk (only used when replacing the gear box is necessary. The gear box is lowered through the opening using a crane) and proceeded to climb down framework to catwalk 5 ft below. While climbing down, employee slipped and fell approx 3 feet.
EE was inside the cone crusher inspecting the crusher for a problem and opened the hatch door to let some fresh air into the inspection area, after several minutes he forgot the hatch was open and leaned back and fell through the door and fell approximately 10 feet.
EE was cleaning up the material that had spilled around the sand screws discharge collecting box with a shovel and in performing this task he slipped and lost his footing and fell thus cutting his right forearm with a 2.5 cm cut. He wasn't sure what actually cut his arm, he was released to come back to work from the clinic but he decided he needed a day of rest.
The employee was working with another EE off a man-lift without a safety harness. The man-lift needed to be moved & he elected to stay on the belt without being strapped in, and at some point got impatient & he took it upon himself to walk down the belt leading to the belt press and lost his balance and falling approximately 16' breaking a bone above his right ankle.
