Employee was climbing down a ladder, near the end of EE's descent EE's foot slipped off the ladders rung causing EE to twist EE's ankle.
Employee was inspecting the #4 kiln pinion gear. He stepped from one level of concrete to a lower level, planted his foot wrong twisting his left ankle.
Employee stepped off the #4 kiln coal mill 5 inch high concrete platform, to retrieve a welding lead. He planted his foot on the edge of the 5 inch high concrete platform and twisted his ankle.
The employee exited his vehicle and was walking to his work area. It had been snowing for several hours. He stopped to make contact with the front-end loader operator working in that area. He took a couple more steps lost his balance and fell.
# 7 Rotary kiln was taking down with a problem to the drag chain. The injured employee was washing material away from the drag chains drain chute. The maintenance man dropped his flashlight, he ask the injured employee to toss it back to him. the light hit the wall fell onto the conveyor belt. The injured employee reached for the light, tripped on a plate that divides the tunnel.
Employee was exiting the cab of the water truck. He hold the steering wheel with his left hand and the grab iron with his right, with both feet on the top step. He placed his left foot on the lower step, as he lifted his right foot his left foot slipped of the step, allowing his right foot to be caught between the cab and first step. With ee falling backwards to the ground.
The employee was replacing the maintenance service door on the #1 coal mill, as he was tightening one of the treaded bolts, it broke causing the employee to suddenly lose his balance. He regained his balance a short time later he felt a sharp pain to his lower back (Lumbar area).
EE was walking down a slight grade when he lost his balance, when he tried to regain his balance he strained himself.
EE left the MCC room with a shovel in hand to clear the snow and ice from the #1 screening tower area so they could start up that part of the plant. Preceding to that area the EE lost his balance and fell, injuring his right shoulder.
EE was walking to inspect the clinker tunnel conveyor. The area which he was walking was wet from a leaking water pump. The wet area had grown some algae and no one had noticed the slippery spot. The EE walked through this area and slipped and feel to the concrete pad.
Two electricians were changing a motor on a dust auger. As they entered onto the concrete foundation underneath the kiln's bag house, one slipped and fell hands first into wet fly ash, a byproduct from coal that also has lime mixed with it. He stopped to wipe his hands off then they continued to finish their task of changing the motor.
EE entered the mechanic shops open bay to get some electrical parts. He was approaching a pallet with hydrauylic hose placed on it. About a foot of the hose was hanging off of one corner of the pallet. EE saw a wheelbarrow in the area of his walk space. He was focused on the wheelbarrow and did not see the hose. He tripped and fell into the wheelbarrow causing the injury.
One of our employees was having trouble putting v-belts on the auxiliary unit. EE stepped down from a two foot concrete platform that the auxiliary unit is mounted to. When EE stepped down he stepped into a trough also made of concrete. He stepped on the edge of the trough & twisted his left ankle.
EE was dismounting the caterpillar 980-B frontend loader. EE claims he had mud on his boots that caused his foot to slip off of the top step of the loader he fell to the ground he hit his knee when he fell.
The EE climbed onto the coal mill journal base plate to see if the coal mill was receiveing coal. On his descent, his feet slipped off the coal mill journal base plate and he fell 32" to the bar grated platform.
Employee was in a squatting position washing down a concrete pad. As he was backing up with the hose, he slipped on some stalite material and the hose came out of his hands. The high pressure hose whipped back toward him. The high pressure water hit him on his left cheek.
EE WAS HELPING SHOVEL ROCK OUT OF THE #2 KILN DRAG CHAIN COMPARTMENT. AS HE WAS EXITING THE DOOR, HE ALLEGEDLY SAID HIS KNEE WAS ON THE DOOR JAM. WHEN HE PULLED HIS LEFT LEG OUT OF THE DOOR HIS RIGHT KNEE SLIPPED OFF THE DOOR JAM AND HIS KNEE POPPED.
Co-worker was on a ladder & injured walked up a ladder with a piece of sheet metal to hand to him. When injured stepped off ladder he stepped down on a 24" long X 18" wide piece of loose concrete & lost his balance & twisted or sprained his right ankle. This was at 3:30pm. About 30 minutes later he removed shoe & ankle was blue.
EMPLOYEE CAME FROM THE KILN CONTROL ROOM TO GET SOMETHING TO EAT. WHEN HE WENT TO THE BREAKROOM, HE SAID THAT HIS HANDS STARTED SHAKING, THEN HE FAINTED & HIT HIS RT. SIDE OF HIS BACK. HE BRUISED HIS BACK.
EE WAS CLIMBING DOWN THE LADDER OF THE 928 CATERPILLAR LOADER. HIS FOOT SLIPPED OFF OF THE STEP, CAUSING HIM TO FALL ONTO THE SHOP'S CONCRETE FLOOR. FROM HIS FALL EE RECEIVED A FRACTURED FOREARM AND A FRACTURED HIP, BOTH ON HIS LEFT SIDE.
THE EMPLOYEE WAS CHANGING V-BELTS ON A CRUSHER. HE STEPPED OFF OF THE PLATFORM & FELL BACKWARDS, HE WAS PREVENTED FROM FALLING TO THE GROUND BY HIS SAFETY HARNESS. HE STRUCK HIS RT. LEG ON THE PLATFORM DURING THE FALL CAUSING THIGH CONTUSION & INJURED HIS KNEE.
EE WAS WASHING DOWN (TUNNEL). HE KNELT DOWN TO AVOID A PIECE OF METAL. HE HIT HIS KNEE ON A PIECE OF METAL PROTRUDING FROM THE WALL, THEN LOST HIS BALANCE, FALLING BACKWARDS ONTO AN I BEAM, HURTING HIS LOWER BACK.
EE WAS GREASING THE SCREEN BEARINGS ON THE 2 DECK SCREEN. HE USED THE HANDRAIL FOR A LADDER. AS HE WAS STEPPING DOWN ONTO THE SCREEN WALKWAY HE TWISTED HIS RIGHT ANKLE.
EMPLOYEE WAS WALKING ACROSS A SLAB TO CHECK ON A PIECE OF EQUIPMENT. AS HE STOPPED, HIS RIGHT FOOT SLIPPED OFF OF THE SLAB AT WHICH TIME HE SPRAINED HIS ANKLE.
EMPLOYEE WAS TURNED AROUND DISMOUNTING THE 980-B CATERPILLAR LOADER. HIS RIGHT FOOR SLIPPED OFF THE SECOND STEP, FORCING ALL HIS WEIGHT TO HIS LEFT KNEE THAT WAS BENT. WHEN HE SLIPPED HE WAS ABLE TO HOLD ONTO THE HANDRAILS TO KEEP FROM FALLING TO THE GROUND.
EE WAS CLIMBING DOW3N THE STEPS OF THE 928 CAT FRONT END LOADER. HIS FOOT SLIPPED OFF THE STEP. HE COULDN'T HOLD HIMSELF WITH THE HANDRAILS. HE FELL ON HIS LEFT KNEE TO THE GROUND.
THE EMPLOYEE WAS WALKING AT NIGHT AND STEPPED IN A DEPRESSION WHICH CAUSED HIM TO FALL TO ONE KNEE. HIS KNEE CAME DOWN ON TO CONCRETE WHICH CAUSED A LACERATION. EMPLOYEE RECEIVED 12 STITCHES.
EMPLOYEE WAS WALKING INTO THE ELECTRICAL HALLWAYTO TURN OFF A BREAKER WHEN HIS FOOT SLIPPED ON SOME ALGAE ON THE CEMENT FLOOR CAUSING HIM TO FALL.
EMPLOYEE WAS CLEANING UIP BOARDS, PLYWOOD AND OTHER MATERIALS FROM HIS LEFT LEG WENT THROUGH THE HOLE AND WENT DOWN ON HIS RIGHT KNEE AND LEG. EMPLOYEE KEPT THINKING IT WAS GOING TO BE OK BUT FINALLY HAD SURGERY ON 9-26-00, NO LOST TIME ON RESTRICTED.
