EE had just exited the excavator and walking down the slope to the Trommel when material gave way causing EE's knee to buckle. Conservative treatment provided up until 6/26 when EE received MRI results showing a meniscus tear. Resulting in an upcoming surgery.

Employee was climbing down the loader steps, using 3 points of contact, when their foot missed the last step. The sudden shift in weight to their arms pulled on their shoulder in a way that left them in pain. Employee was unable to lift their elbow past the height of their shoulder. Employee was released from care on 4/8/22 and MRI results came in on 9/20/22 showing a tear.

While moving from the cab to the window area, EE missed a step. Despite having good contact with the hands, EE was unable to hold on and fell off the loader. This transition area can be tricky and varies by manufacture on how to best access the area. EE had contact in three points but fell when switching.

Entering loader, reached for the handrail on the door while stepping to top stair. Missed door handrail & fell backwards. Caught self with hand. This action caused a heavy load on EE's right shoulder. MRI shows a Bicep tear. Delay in reporting as Insurance has been debating if the tears are Chronic or Acute.

After employee finished working on under cone belt and splitter employee was stepping off the belt, employee's right leg slipped off the new conveyor belt section employee just installed. Employee lost balance and left arm/elbow hit the troughing idler. Employee's coworker helped employee step off the belt.

Employee initially had a first aid injury in Dec. 2015. On Feb 15, 2016 his injury became recordable due to surgery on his shoulder for a slip and fall on ice from his Dec 2015 injury.

Employee climbing out of the haul truck, right foot on the ground, when his left foot got caught on the teeth of the bottom rung, causing him to lose his balance. his grip with his left hand slipped loose, and he swung to the right falling on his left hand wrist on the ground. tried to catch hiself with the left hand.

Employee stopped loader next to berm so that he could get out and give a set of keys to a supervisor. Descending the ladder, no 3 point hold, he jumped to the berm landing on the slope side with toes pointing upwards, overextending the calf muscle. a couple hr later declined medical help. a week later 11/8/06 pain still continued, he then decided he should be looked

Employee was walking around his loader to get grease to prepare to grease and lube the loader. He slipped on the snow-covered ground to his rear end. Upon getting back to his feet his back was hurting but he didn't want medical help that day. Next day he went to the industrial physician; follow up on 1/27/06 resulted in restriction to his work duties.

Operator was climbing out of the loader, did not set his foot completely on the bottom rung (approx 18" from ground), causing his foot to slip off the rung. Employee held on tight with the rig ht hand (no thumb on hand) causing all his body weight to pull on the right shoulder. He began to experience pain under the shoulder blade and his upper shoulder immediately.
