While moving from the cab to the window area, EE missed a step. Despite having good contact with the hands, EE was unable to hold on and fell off the loader. This transition area can be tricky and varies by manufacture on how to best access the area. EE had contact in three points but fell when switching.

Employee was getting concrete blocks for a project and a rock got into the loader bucket. When he tried to push the rock out, it slipped back and smashed his finger.
