Slipped and Twisted Right Knee.
Employee was putting out a pump to remove surface water. He slipped on an ice chunk and fell. Employee sprained left shoulder and bruised left elbow.
He was running to shut off feeder on breaker; while on his way he stepped wrong and hurt his left knee. He continued working that day. Oct 31, he came to work and realized that he needed to have a doctor look at his knee. Doctor told him to take it easy over the weekend and let the swelling go down while using crutches, and follow up with him on Monday, Nov 3rd.
Employee was on cat walk pulling cable back. Cable was stuck on something and when it released from what it was caught on, employee fell backwards, losing balance. He fell through the opening between the railings, which was about an 8'-10' fall. Resulted in an injury to the head and left shoulder.
Stepping down onto a fuel tank, lost balance and had to jump approx. 6 feet, landing improper and injured right knee when landed.
