EE tripped on low step and fell forward. As EE fell, threw arms in front of self to catch self. Rt. hand struck a pipe on the west side of the walkway. The EE tore the triceps muscle in rt. arm.
The injured EE lost footing and fell backward (fall on same level) while pulling a return roller into place. As EE fell attempted to brace self with rt arm and felt a "popping" sensation as EE struck the floor. The injury is a strain to rt shoulder.
EE was pulling a new section of conveyor in place when he lost his footing and tripped, falling backward.

The EE made a repair to a D9T dozer. After climbing down from the dozer he stepped backwards and tripped in a frozen rut. He landed on both hands spraining both wrists.

At 9am driller observed EE's knees buckling and going to the ground. EE insisted being fine just tired from lack of sleep. He finally agreed to be driven to hospital. He was admitted for heat exhaustion and severe dehydration. Later diagnosed with knee contusion and hairline ankle fracture from fall. He was released 8/16. His return to work is pending medical release.

Employee was accessing front end loader and reported a "pop" in his knee. Employee was sent to doctor and returned to work the same day withouth restriction. Employee's knee did not seem to heal and in December an MRI was conducted identifying a minor tear requiring surgery to repair.
