EE tripped on low step and fell forward. As EE fell, threw arms in front of self to catch self. Rt. hand struck a pipe on the west side of the walkway. The EE tore the triceps muscle in rt. arm.

The injured EE lost footing and fell backward (fall on same level) while pulling a return roller into place. As EE fell attempted to brace self with rt arm and felt a "popping" sensation as EE struck the floor. The injury is a strain to rt shoulder.

EE was pulling a new section of conveyor in place when he lost his footing and tripped, falling backward.

Employee reached down to pick up a chain. When he stood up he was a little dizzy and fell backwards. He stuck his left arm out to break the fall and dislocated his elbow.

EE was walking behind hydraulic drive cabinet on kiln 1 drive pier when he began to slip and fall on an oily surface. EE caught himself but the motion strained his lower back.

The EE made a repair to a D9T dozer. After climbing down from the dozer he stepped backwards and tripped in a frozen rut. He landed on both hands spraining both wrists.

At 9am driller observed EE's knees buckling and going to the ground. EE insisted being fine just tired from lack of sleep. He finally agreed to be driven to hospital. He was admitted for heat exhaustion and severe dehydration. Later diagnosed with knee contusion and hairline ankle fracture from fall. He was released 8/16. His return to work is pending medical release.

Employee was walking down a flight of steps on the PreHeater Tower at Lafarge Cement Harleyville, SC and missed a step. He fell down approximately 5 steps and suffered compound fracture to his leg.

Operator injured his arm when he lost his grip and fell while mounting dozer.

An employee was removing the holding screw for the shredder teeth. He lost his balance and cut his arm on one of the teeth.

EE was removing oxygen cylinder from high pressure cylinder storage rack when he slipped and fell on ice. Cylinder fell on his knee.

Employee was descending ladder with tool in hand. Employee fell approx. 10 feet to the ground. Sprain to right knee was result of accident.

IRON WORKER WAS DRAGGING AN I-BEAM OUT OF THE KILN AND SLIPPED. THE I-BEAM CAUSED A LACERATION TO LEFT INNER THIGH.

Employee tripped while exiting a pick-up truck. Employee fell on his left wrist and shoulder. Note: Originally this was not reportable, however, employee saw a physician in February (2007) and then began restricted duty on March 5, 2007.

EE fell tripped on crack in concrete slab injuring his left ankle. EE has pre-exiting condition toward severe damage to both ankles. Still under internal investigation.

Employee slipped at limestone reclaimer. Accident became reportable in December.

There was loose material on the floor and it had snowed that morning. The employee slipped on the material and fell on his back and sprained his back.

Note: Employee failed to report incident on a timely basis. The company is investigating whether the incident occurred on the mine site. Initial employee report to supervisor was on 3/6/2006. Employee claims to have twisted left foot while stepping off mobile equipment.

Employee rolled and sprained right ankle when he stepped on a seam between concrete slabs in the Maintenance Shop

Employee slipped in grinding aid area and rolled right ankle.

The employee tripped over a pile of material he was shoveling from the floor of the pelletizer. He attempted to stop his fall by bracing himself on the pelletizer dust collector fan housing. He cut the back of his right hand. The wound required three stitches. No time was lost. He returned to normal duties.

The employee slipped on ice and fell on the deck of the barge Alexandria. The weather at the time was sleet and freezing rain. There is the potential for a slipped disc.

EMPLOYEE WAS CLIMBING DOWN OUT OF STORAGE TRAILER AND HE STEPPED ON A BLOCK OF WOOD AND LOST HIS FOOTING AND FELL ON HIS RIGHT SHOULDER. EE SHOULD HAVE USED A LADDER AND TIED IT OFF TO THE STORAGE TRAILER TO PREVENT MOVEMENT. THE ON-SITE SAFETY MGR. EXPLAINED TO EMPLOYEE THAT HE SHOULD USED A LADDER.

WHILE WALKING ON GROUND LEVEL CONCRETE FLOOR IN PACKHOUSE, EMPLOYEE INTENDED TO STEP-UP TO NEXT LEVEL. EE MISJUDGED HIS STEP HEIGHT & STUMBLED FORWARD, STRIKING HIS LT. WRIST AGAINST A SHARP EDGE OF METAL, THUS CAUSING A MINOR CUT TO HIS LT. WRIST.

Employee was accessing front end loader and reported a "pop" in his knee. Employee was sent to doctor and returned to work the same day withouth restriction. Employee's knee did not seem to heal and in December an MRI was conducted identifying a minor tear requiring surgery to repair.

WATER HAD DRAINED ONTO THE FLOOR FROM THE AC IN SILO 20. THE EMPLOYEE SLIPPED IN THE DRAIN WATER AND FELL.
