Employee was pulling on the feeder cable when he felt pain in his right arm and fell at the same time. He fell striking his elbow and shoulder.

Was walking back to truck from taking piezometer readings of Dam#1 at Moss 3 facility. As crossing spillway, slipped on slick surface and slid 10' before culvert drain emptied into spillway carrying EE another approx. 150'. A large rock in spillway stopped slide and threw EE out of spillway. Rock hit him on tailbone and movement on right side below hip is stiff & sore.
Employee was lifting a crib block on uneven ground that had some thin ice in the surrounding area. The ice broke loose causing employee's foot to slip through resulting in a pull to the left calf muscle.
Employee slipped on ice in parking lot going to the supply truck.

INJURED STARTED TO STEP FROM THE HOOD OF A VOLVO TRUCK ONTO A LADDER. HIS FEET SLIPPED & HE MISSED THE STEP OF THE LADDER CAUSING HIM TO FALL TO THE GROUND & THE LADDER FELL ON TOP OF HIM WHILE HE WAS INSTALLING TIN TO A SHOP BUILDING.
EMPLOYEE WAS OPERATING THE CONTINUOUS MINING MACHINE. A PIECE OF ROCK SLID OFF RIB. EMPLOYEE MOVED OUT OF WAY OF ROCK, TWISTING KNEE. WENT TO DOCTOR ON 8-24-05. STARTED MISSING WORK ON 8-29-05.
Injured was dismounting a d-6 dozer when his foot slipped off of the pusharm and wedged between the push arm and the dozer frame. As he fell backward it twisted his left leg. He started loosing time 25 of 4-11-06.
Injured was replacing the bottom deck discharge lip on a raw coal vibrator. As he stepped down from the vibrator to the floor he felt a tingle in his right knee.

EMPLOYEE STEPPED FROM A FLATCAR WHILE UNLOADING TIMBERS AND STEPPED ON A ROCK, TWISTING HIS KNEE. STARTED LOSING TIME ON 1/24/05. HAD SURGERY 1/26/05.
INJURED WAS WORKING ON TOP OF THE EXPLOSION DOORS OVER THE CYCLONES OF THE THERMAL DRYER. HE STEPPED BACKWARDS OR HIS FEET SLIPPED CAUSING HIM TO FALL SIX TO SEVEN FEET STRIKING HIS RIGHT ANKLE AND LEG ON A METAL BEAM.
