Showing 50 of 71Employee was walking out by the miner when EE's legs got tangled up in the miner cable causing EE to twist EE's left knee as EE fell. This resulted in a fracture in EE's knee cap.
While working on a brattice, employee turned to get some cap wedges when he tripped over a plaster bucket. As he was falling his body made a twisting motion causing severe pain in his lower back continuing down to both of his feet.
Employee was operating Fletcher walk thru roof bolter installing roof bolts in 4 right xcut. He had drilled hole in roof, inserted resin and started bolt into hole. He had bent the bolt due to low height. As he attempted to straighten the bolt to get in drill head the bolt shifted and he fell forward hyper extending his right elbow/shoulder.
Drill steel fell out of rack and hit employee on the left side pushing his right side into a piece of pipe. He was tying his boom to the steel to secure it from falling to be welded. This resulted in 1 fractured rib.
Employee was carrying a top roller when he tripped over another roller that was laying on the bottom. He dropped the roller catching himself causing pain in his left shoulder.
Foreman was standing on ladder hanging 8" water line when a scoop ran over the water line causing it to break, striking him on the right side and knocking him off the ladder resulting in 4 fractured ribs & a small puncture to his right lung.
Maxxim Shared Services employee was setting a check spad in the mine roof, the 4' step ladder being used shifted weight which resulted in the employee falling off of the 3rd step hitting the mine floor. He received a small cut below his right elbow resulting in 2 stitches. He was not under the operator's direct supervision.
Employee was stepping into the driver's side of the man trip bus. He struck his head on bus overhang above the driver's side door. He fell backwards landing on the gravel parking lot hitting his head and upper shoulders on the ground causing numbness to his legs, arms and a small laceration to his head.
While trying to exit the lo-trac, the employee stood up raising one leg up to step out when the opposite foot slipped causing him to fall on the handle towards the fork side of the lo-trac bruising his ribs and straining his shoulder. He continued to work up until he had shoulder surgery on 11/18/2014.

Employee was walking off section when he came across a water hole. Not realizing there was a drop off in the water hole of at least 12", he proceeded causing him to trip and twist resulting in a lower back strain. Doctor took him off work on 8/18/14.
