Employee was taking down a piece of tubing. When it came down it struck the ladder EE was standing on causing EE to fall off the ladder and EE hit left arm on the tubing resulting in a strain to left shoulder.
Employee was walking out by the miner when EE's legs got tangled up in the miner cable causing EE to twist EE's left knee as EE fell. This resulted in a fracture in EE's knee cap.
While working on a brattice, employee turned to get some cap wedges when he tripped over a plaster bucket. As he was falling his body made a twisting motion causing severe pain in his lower back continuing down to both of his feet.
Employee was operating Fletcher walk thru roof bolter installing roof bolts in 4 right xcut. He had drilled hole in roof, inserted resin and started bolt into hole. He had bent the bolt due to low height. As he attempted to straighten the bolt to get in drill head the bolt shifted and he fell forward hyper extending his right elbow/shoulder.
Drill steel fell out of rack and hit employee on the left side pushing his right side into a piece of pipe. He was tying his boom to the steel to secure it from falling to be welded. This resulted in 1 fractured rib.
Employee was carrying a top roller when he tripped over another roller that was laying on the bottom. He dropped the roller catching himself causing pain in his left shoulder.
Foreman was standing on ladder hanging 8" water line when a scoop ran over the water line causing it to break, striking him on the right side and knocking him off the ladder resulting in 4 fractured ribs & a small puncture to his right lung.
Maxxim Shared Services employee was setting a check spad in the mine roof, the 4' step ladder being used shifted weight which resulted in the employee falling off of the 3rd step hitting the mine floor. He received a small cut below his right elbow resulting in 2 stitches. He was not under the operator's direct supervision.
Employee was stepping into the driver's side of the man trip bus. He struck his head on bus overhang above the driver's side door. He fell backwards landing on the gravel parking lot hitting his head and upper shoulders on the ground causing numbness to his legs, arms and a small laceration to his head.
While trying to exit the lo-trac, the employee stood up raising one leg up to step out when the opposite foot slipped causing him to fall on the handle towards the fork side of the lo-trac bruising his ribs and straining his shoulder. He continued to work up until he had shoulder surgery on 11/18/2014.
Employee stated he did not see cable and stepped over it. Car moved jerking cable causing his foot to be pulled to the side straining his right knee. Doctor took him off work on 8/15/14.
Employee was walking off section when he came across a water hole. Not realizing there was a drop off in the water hole of at least 12", he proceeded causing him to trip and twist resulting in a lower back strain. Doctor took him off work on 8/18/14.
Employee was throwing miner cable over on the rib when he slipped and fell. Employee stated when he tried to put pressure on the front of his foot, the back of his heel hurt. Employee continued to work until 7/1/2014.
Employee was unloading belt structure in break when he stepped backward and stumbled. He stated he fell flat down on his tail bone resulting in lower back pain.
Employee was working on top of scoop, changing hydraulic pump. He stepped down off the scoop onto uneven bottom causing him to twist his right knee. Employee had surgery as a result of this injury on 3/3/2014 and is now off work.
St. Paul Builder/Block vendor was unloading a pallet of block when he stepped backwards and stumbled over the out rigger and landed on his hip resulting in a fractured hip.
Employee was walking to the man trip carrying a dinner bucket, bucket of bits and a bag with hand tools when he felt a strain to his lower back.

Employee was pulling the cable off a shuttle car and fell down and hurt his hip. He continued working until he was taken off work on 9/24/12.

Employee slipped in the mud along the belt line and twisted his left knee. He continued to work until he had surgery on 6/22/12.
Employee was stepping over shuttle car cable when he was tripped by the cable causing him to land on his back when he hit the ground. Employee was scheduled off on the 19th and was unable to get to work on the 20th because of snow. He missed work on the 21st and was scheduled off on the 22nd and 23rd. He returned to work on the 24th.
Employee was helping roll the cutter head around on the miner to set bits when he slipped and a miner bit punctured a hole in the top of his left knee resulting in stitches.
Employee was in the process of taking off hydraulic cap on excavator when he lost his balance. He fell off of the track of the excavator. The excavator track is 42" from the ground. When he fell he landed on his left arm resulting in a fracture.
Employee had just finished moving an excavator out of the haul road. He was walking back to his dozer when he stepped in a hole. As a result he twisted his left ankle and fell.
Employee states while walking in the #3 entry he stepped in a hole and felt pain in his right knee.
Maxxim Rebuild employee was kneeling on ground and turned his body to pick up chock blocks and felt his leg snap and fell to ground. Fracture was on the left leg above his ankle.
A contractor employee was assisting in installing water line when the water line fell striking him under the chin causing him to fall backward into the Sandvik hauler canopy. This resulted in a laceration to the chin and back of the head requiring stitches.
Employee was walking up the #3 entry and slipped in the mud causing him to twist his left ankle causing a fracture.
The employee slipped on muddy ground while marking drill holes. Resulting in the employee falling and injuring his neck and both shoulders.
Employee was assisting in moving Shuttle Car cable slack up during a power move when his right foot got entangled in the slack as it was being pulled causing him to be pulled down twisting his right knee.
Employee stepped on uneven ground and twisted ankle. Resulting in sprained ankle.
Employee stated that he twisted his left knee while walking to check the # 5 Entry. He had surgery on 04/02/2010 but has continued to perform his regular duties as foreman.
Employee slipped off bumper of service truck, spraining knee.
Employee was pulling cable reel off of the roof bolter, when he slipped and fell, twisting his knee. Started losing time 9-9-09.
Performing 1000 hours service on 345 excavator. EE attempted to climb on to the machine and due to rainy & muddy conditions, his foot slipped off the step causing him to slip and twist his left shoulder while holding onto the handrail. Pain in shoulder got much worse on 3/2 and ee sought treatment.
Foot slipped while dismounting dozer.
Injured climbing up the ladder of the loader, when his right hand slipped off the ladder causing him to swing ground and fall from the loader hitting on his butt and then backwards causing his head to strike the bottom edge of the hyralic tank.
Employee was replacing blown hydraulic hose on trailer attached to Peterbilt tractor (coal truck); slipped in hydraulic oil and fell from back of tractor onto pavement (approx. 43"); bruised middle back and hip.

Employee stated that he was walking across section, slipped on loose material, went to doctor on 1-4-07. Continued to work until 3-20-07 when doctor removed him off work.
Employee was in the process of performing service on a Caterpillar 992G loader. He was walking around the side of the machine when he stepped on a small pile of dirt. The dirt shifted causing him to slip and loose his footing. In the process, he twisted his right knee.
