Employee was exiting a D11R. EE stated that EE's right foot was on the last step on the push arm as EE was lowering EE's left foot down to the ground. EE lost EE's balance on some frozen material and twisted EE's knee as EE tried to regain EE's balance.

On 03/17/2018 the employee went to the doctor and found that the employee in-fact fractured arm. Employee reported this to supervisor on 03/18/2018. The employee has not missed any time due to this accident as of this date.

Management has been made aware through medical documentation that the employee is missing time due to the shoulder accident Jan 21, 2014. On that date the employee alleges that he slipped on ice, fell, and struck his left shoulder against the push arm of a dozer that was being repaired.
EE was carrying the top part of an overcast when he slipped on loose rock and fell causing a fracture to the left hand. EE was getting ready to weld a stop of for an overhead crane. EE fell 20 feet to the steel floor below landing on his left wrist and knee. EE was transported to CMAC General and was treated for a broken wrist and knee. EE was released on 12/19/13 and is now home.

EE evening shift EE reported to work at 15:35p.m. at the Gate 2 Guard shack. While attempting to enter guard shack a strong gust of wind blew the door into EE knocking him to the ground. EEG received a laceration to his left elbow requiring stitches.
Employee was operating a roofbolter and slipped on a lump of coal tired to catch himself grabbing the roof bolter tray causing him to strain his shoulder. Employee sought medical attention and was released with no days lost or restricted. On 11/27/12 employee sought additional medical treatment.
