Contractor driver from TRANS CEMENT LLC completed the activity of closing the truck hatch and proceeded to enter the truck cab while keeping the door open. While inside the cab the driver fell backwards to the ground.

Tech was squatting down to install a drain plug on a wheel loader. As they stood up, pivoting their feet and their left became caught behind right foot. Tech then rolled their right ankle and fell down. Tech sustained a fractured tibia.

IT worker fell walking down steps while carrying a printer
The employee was walking to get a material sample at the raw mill when the employee tripped on a structure beam and fell
When lifting a bucket of material sample, of around 35 lbs., to put it into the back of a truck, EE felt a pain in right knee and cannot keep balance and fell.
While taking lab samples with a shovel, the employee felt a pain in EE's right knee that prevent EE to supported EE's weight. NOTE: EE has NOT lost any days but is working on restricted activity.

AFTER SERVICING EQUIPMENT, EMPLOYEE WAS ON HIS WAY DOWN WHEN HE MISSED THE LAST STEP AND FELL.
WHILE GETTING READY TO COME DOWN FROM A DUMP TRUCK BED, EMPLOYEE STEPPED ON SOME CONCRETE BLOCKS BEING USED AS STEPS. THE BLOCKS BROKE AND HE FELL BACKWARDS STRIKING HIS LOWER BACK.
