Employee was sitting in a chair working on a wire saw control box and the chair shifted and EE fell out of the chair to the ground from a seated position. Review of the incident inconclusive of chair malfunction or employee weight shift could have caused the fall. Chair was disposed of regardless.

Showing all 4Thawing mud and ice conditions at top of wall. Miner stepped off ladder to edge of wall. The frozen mud gave way and he slipped, falling 8-1/2 ft backward. Landed on right side/knee.
Employee was working on a Volvo L330D Loader from a temporary platform constructed for the purpose of gaining access to the area in need of repair. The employee attempted to re-position the equipment by moving it, while standing on the platform and steering. Platform collapsed and employee fell.
Miner was decending on ladder tied off ladder. Ladder slipped, miner lost balance and fell approx. 8 ft. Hit right shoulder and head on floor. Iced/heat applied to shoulder and neck immediately. Headache and stiffness in shoulder/neck area.
