On 8 Jan 2020 the employee was working in the Dry Plant area and individual tripped over EE's own feet and when EE fell forward EE placed EE's left hand out and palm of hand impacted a small rock on the ground. The rock made a cut into the hand. Several stitches were given for the laceration.

Employee was checking belts and slipped on ice resulting in hitting head on the ground. Employee has previous medical non work related history with head injuries, and is off work for continued evaluations.

Employee was walking on top of a rail car when fall protection device became snagged on the supporting structure. This caused employee to be stopped mid stride while stepping from one car to the next. This caused employee to lose balance and fall between the cars. Total fall before arrest by retractable lanyard was approximately 4 feet.

The employee was descending a metal stair way and misjudged the last step thinking ee was at the ground. The employee stated that ee was looking around the plant when this happened and not fully focused on the stairs. It was dark out and ee may have cast a shadow over the bottom stair as ee descended.

Employee was loading on top of rail cars. Employee was tied off to fall protection the entire time. Employee was using a retractable lanyard and let it trail behind while walking on the car and moved too quickly causing the retractable lanyard to lock. Employee lost balance and fell onto the catwalk on the rail car and sustained injury.

EE was painting walls in a QC lab. EE stepped off of the step ladder that was being used and unto the counter top in the Lab. EE lost balance and fell from the counter top to the floor. EE hit head on the floor causing a laceration that required 7 sutures to close the laceration. EE was able to return to work that day with restrictions.

Miner was walking down provided stairs and using handrails in railcar preparation area and upon stepping off of last step to the ground rolled right ankle. Miner fell to the ground and continued work for several more minutes and then reported injury.

Individual was stepping off railcar onto load out platform and right foot caught edge of platform and fell forward scraping right arm and left hand. Majority of impact occurred onto right hand resulting in strained right shoulder.

While being tied off onto rail fall protection system, as he stepped across between 2 railcars the supplemental restraint locked and pulled him off balance. He fell slightly backwards between the cars, approximately 3 feet. He alone stepped back onto the railcar and reported incident.

EE was climbing down a ladder and after he reached the ground he turned and slipped on some snow that he just brushed off a railcar. This incident happened in our 500A silo. EE fell landing on his tailbone. He was taken to the local hosp. and was released the same day. At this point he is on light duty for two weeks as per doctors orders

While standing on the running board of the maintenance truck, employee was observing a head pulley on a feed hopper when his right foot slipped off the running board. The right foot hit the ground first causing it to roll over. Running board surface in conjunction with sand and standing on tip of toe caused slip.
