While dragging a 480 VAC pump cathead and cable to a ride, EE slipped in the mud initially claiming to have "popped" EE's left knee but later claimed to have strained EE's lower back resulting in significant pain.

Showing all 101-17-19 Employee stated ee fell over miner cable or miner pull strap on #2 unit on 1-9-2019 stated didn't report to supervisor thought it would be fine. 17th ee reported and we took ee for medical attention, released no restrictions! 1-21-19 I sent ee back to Dr. ee states ee was having trouble 1-22-19 is the first lost day of the incident.
Employee states while moving miner cable on #2 unit stepped in hole of mine floor twisting and straining left calf muscle. Finished shift and returned to work next day, did not become lost time until 5-1-2018.
Employee states walking over to left return brattice line just outby # 2 unit to get a flashing light for the unit. Stepped on a rock with left foot and while stepping over with right foot the rock shifted causing them to fall putting all their weight on left toes causing fracture to small toe at base of the foot.
Injured was walking at the battery charging station and tripped. Right shoulder made contact with the battery ram car bed. EE felt a pain in the right shoulder that subsided through out the shift. EE continued to have mild pain and went the Doctor on 2/13/2017. EE was placed on restricted duty at that time.
Injured was on opposite operator side of the roof bolter. EE was bending roof bolts for the next row. The roof bolter trammed forward and trammed across a rock causing the rock to move. Injured was standing by the rock and the movement caused Left knee and ankle to twist. Suffered a strain/ sprain.
Injured was walking beside the roof bolter as they advanced to the next row to bolt. EE twisted knee and fell down.
Injured tripped while boarding a 6 man diesel man trip. EE is unaware of what EE tripped on and feels EE tripped over EE's on feet. EE fell to the ground and injured EE's left shoulder.
EE was rock dusting belt line. Was walking backwards and tripped on a hump in the mine floor. Lost his footing and twisted his left knee. REPORT IS BEING SUBMITTED ON 03/18/2016 DUE TO FACT HE HAS BEEN AT WORK UNRESTRICTED ON HIS NORMAL JOB UNTIL 3/14/2016. HE WAS EXAMINED BY A DOCTOR ON 3/15/2016 MEDICAL CARE WAS ADVISED FOR EE REQUIRING LOST WORK DAYS.
Injured was climbing onto a Komatsu HM350 gob truck to enter the operator's deck area. His left foot slipped and he fell. He suffered a contusion to his left knee.
