While spraying out a heavy medum sump a 1 inch conduit being used to direct water jumped up from pressure striking employee in the left side of the temple area. Employee received a fracture to the cheek area.

Injured was off the clock and traveling to work on the haul/travel road on mine property. Approximately 1 mile from St Rt 593, injured lost control of the personal vehicle and hit rock wall along travel road. Injured suffered a concussion and contusions.

Injured was climbing onto a Komatsu HM350 gob truck to enter the operator's deck area. His left foot slipped and he fell. He suffered a contusion to his left knee.
Injured was dumping a bag of dry cat into the cat feeder. He felt a pain in his lower back.
Injured slid off the hood of a Komatsu dozer after changing a headlight. He stated he may have pinched his right testicle. He was unsure of how he received the injury and stated he must have received it during this event. Accident report was filled out on 6/13/2015. After medical inquiry it was deemed to be a reportable event.
The injured was taking apart a water line fitting with 2 pipe wrenches on 2/06/2015. He felt a pain in his shoulder. He reported accident on 2/10/2015. He had not missed work and continued to do his normal job but required surgery on his right shoulder on 5/29/2015. He has not been released to come back to work since surgery on 5/29/2015.
