While dragging a 480 VAC pump cathead and cable to a ride, EE slipped in the mud initially claiming to have ""popped"" EE's left knee but later claimed to have strained EE's lower back resulting in significant pain.

Employee stated that while coming out of the mine they pushed airlock door open and large amount of dust blew up under glasses filling their eyes up. Employee was treated and released w/medicated eye drops.
Employee states they were double booming a six foot glue bolt and while bending roof bolt straight to install it rolled out and caused them to lose balance, hyper extending their left arm causing pain from bicep to shoulder in elbow area.
Employee states while working under #4 shuttle car on #2 unit a large amount of material fell on EE (coal, Dirt) fell landing in EE's eyes and in EE's left ear. Took employee to ER at Baptist health in Madisonville and received antibiotic eye drops. Employee back to work next shift.
1-17-19 Employee stated ee fell over miner cable or miner pull strap on #2 unit on 1-9-2019 stated didn't report to supervisor thought it would be fine. 17th ee reported and we took ee for medical attention, released no restrictions! 1-21-19 I sent ee back to Dr. ee states ee was having trouble 1-22-19 is the first lost day of the incident.

Employee tightening a ratchet strap securing a slinger duster and hook came loose striking EE in the mouth breaking a tooth in middle top of EE's mouth and loosening three more.
Rock fall in-by the tail piece on #2 unit in the #7 entry in the 2nd east sub mains.
Employee attempting to remove shuttle car tire with another employee, the tire came off before they thought and caught left hand between tire and frame. After examination found chip fracture in top of hand.
Employee states while traveling back to the Cont. Miner to get a load of coal on #1 unit and between entries 6 and 7 drove over a large rock causing them to bounce into the canopy hurting their upper back. Employee worked the next shift and went to doctor and is now off until next appointment.
IE beating dust out of pinner steels with a 1' wrench when wrench bounced back and hit IE in forehead resulting in laceration needing 3 sutures to close.
Employee welding on #1 unit golf card, holding metal and didn't get assistance or use welding hood that was provided.
Employee states while moving miner cable on #2 unit stepped in hole of mine floor twisting and straining left calf muscle. Finished shift and returned to work next day, did not become lost time until 5-1-2018.
Employee states while loading six foot roof bolts on bolter EE twisted strained lower left back, pain to lower left back. Employee taken to medical care and treated for strain.
Employee cutting jack pipe with grinder and had rust from pipe fly up in left eye. Employee was taken to Hospital and eye was washed out and given prescription eye drops.
Employee installing roof bolt, while booming up ee hit rotation while hand was still on pin plate and glove caught on pin plate rotating middle and pinky finger, causing fracture to end of pinky finder and chipping bone on middle finger.
Employee was working off side of operator preparing to load roof bolter, as the partner backed up they walked too close allowing employee's right foot to be run over by front tire of bolter. Employee was taken to Doctor the day of accident and returned 8-23-2018 without restrictions and during follow Doctor visit 2-27-2018 was given restrictions and became lost time 2-27-2018.

Employee was installing six foot fully grouted bolt in #1 entry on #1 Unit 16+23 in the 6th south panel. While installing bolt six footer, Employee was double booming bolt to attempt to install, smashed hand between mine roof and bent roof bolt employee was putting up. Injury to three fingers required stitches.
Employee states walking to assist hanging #1 car cable felt back pop and was assisted out of mine complaining with lower back pain and transported to medical facility.
Employee states walking over to left return brattice line just outby # 2 unit to get a flashing light for the unit. Stepped on a rock with left foot and while stepping over with right foot the rock shifted causing them to fall putting all their weight on left toes causing fracture to small toe at base of the foot.
Employee states while hooking up a pull rope up to miner cable to pull forward to working face the knot was caught as employee jerked felt a pain in back
Employee turned in a accident report on 7-20-2017 stating struck their head on mine top or key hole plate causing pain to neck. No lost days or restricted duty until 9-8-2017 employee went back to Doctor complaining with pain to upper back and neck is currently off work pending test.
Employee states while removing belt framing from 1B belt drive felt pain in lower left side of back.
Employee working on #2 unit assisting miner operator on left side of section, lifting rub rail on miner heard and felt a pop in left bicep.
Employee states while getting out of the battery hauler on #1 unit, Employee hit the breaker from inside the cab and while getting out, the cab section of battery hauler rolled on EE's right heel.
Employee states that while picking up belt framing on 1B belt, stepped and turned and felt discomfort in neck. At time of report it was file only, but after Employee went to Dr. found that needed surgery. Employee didn't miss work or restricted until 4-21-2017 for surgery on Neck.
Injured was hooking up a reclaim trailer to a diesel mantrip. The trailer shifted and the trailer tongue hit injured on right shin and caused a laceration that required 11 stitches.
Injured was walking at the battery charging station and tripped. Right shoulder made contact with the battery ram car bed. EE felt a pain in the right shoulder that subsided through out the shift. EE continued to have mild pain and went the Doctor on 2/13/2017. EE was placed on restricted duty at that time.

Injured swung boom of roof bolter out to put up outside pin. The steel struck a piece of top coal. The piece of coal fell and struck the injured on left foot. Employee suffered a fracture.
Injured was moving over a 3 foot section of miner cable and felt a pain in lower back.
Injured was in a resting position on EE's knees filling out paperwork in the 10R x-cut in the 2nd last open. A loaded shuttle car passed by EE and ran over a piece of coal or rock causing the material to burst and send a piece toward injured. EE was struck on the left ear and the impact caused EE's ear plug to lodge in EE's ear canal.
Injured was on opposite operator side of the roof bolter. EE was bending roof bolts for the next row. The roof bolter trammed forward and trammed across a rock causing the rock to move. Injured was standing by the rock and the movement caused Left knee and ankle to twist. Suffered a strain/ sprain.
Injured was assisting in hanging a miner cable. EE felt pain and a pop in right shoulder.
Injured was turning a corner heading to the section feeder. The dumping end of the ram car hit the coal rib and knocked the coal loose from the rib. The coal hit EE in EE's right eye.
On 7-20-16 EE was walking across # 1 unit and walked through a wing curtain in # 4 entry hitting a brow breaking hard hat. Reported accident, but employee lost no work days until 8-13-2018. Employee had medical treatment related to injury and now off work.
Injured was bending a roof bolt using hand and stepping on it with left leg. When injured applied pressure felt pain and a pop in left knee.
Injured was bending a 6' roof bolt. The bolt slipped and EE twisted arm and felt pain in right shoulder.
Injured left the miner tramming shuttle car to the feeder. Injured trammed too close to the rib and an area of rib made contact with the canopy and grab handle which cut right index finger.
Injured was walking beside the roof bolter as they advanced to the next row to bolt. EE twisted knee and fell down.
Injured was straightening out a 6' roof bolt. Felt a pop in left arm and arm, forearm, and elbow was sore. Injured continued to work. Injured had a Doctor appointment on 08/09/2016. Injured is now off work due to restrictions. First day off work was 08/09/2016
Injured tripped while boarding a 6 man diesel man trip. EE is unaware of what EE tripped on and feels EE tripped over EE's on feet. EE fell to the ground and injured EE's left shoulder.
Section foreman was helping repair a damaged shuttle car cable. The belt knife blade slipped out of the cable and cut through EE's glove and resulted in a laceration to EE's left hand that was holding the cable. EE received 5 stitches.
Injured was operating a ram car. EE went to make a turn and the center section of the car bottomed out against each other. When the center section connected it jarred EE and EE felt increased pain in lower back.
Injured got glove caught between pinner steels. EE applied rotation and EE's left hand, wrist and arm were injured when the steel rotated.
Original report date 2-25-2016, EE continued to have issues and sought medical attention on 5-22-2017. Pennyrile continues to investigate and contends that the issues are related to a prior injury at another location.
Employee was roof bolting and checked the pin to see if it was tight. After, EE placed hand on top of the canopy and swung the boom and caught EE's hand between the canopy and mine roof.
Injured was bending a pin that had been placed in the top to align it back into the roof bolter rotation pot. The pin slipped causing EE to strain EE's left shoulder.
Injured was swinging a pipe to remove a roof jack support for the header during reclamation and hurt right shoulder. EE had been treated for the illness since 2006 and was supposed to have surgery on it in January 2016 and injured elected not to have the surgery to treat condition. EE was rock dusting belt line. Was walking backwards and tripped on a hump in the mine floor. Lost his footing and twisted his left knee. REPORT IS BEING SUBMITTED ON 03/18/2016 DUE TO FACT HE HAS BEEN AT WORK UNRESTRICTED ON HIS NORMAL JOB UNTIL 3/14/2016. HE WAS EXAMINED BY A DOCTOR ON 3/15/2016 MEDICAL CARE WAS ADVISED FOR EE REQUIRING LOST WORK DAYS.
Injured cut his right thumb requiring 5 stitches. He reached for his belt knife in his pouch and the blade was still exposed in the knife resulting in a laceration.
Injured had his body in a strained position. He was pushing in a breaker switch inside the cover on a battery ram car and strained a chest muscle.
Injured was repairing a damaged miner cable. He was cutting the outer jacket when then belt knife slipped and cut his inner left knee. He received a laceration that required 7 staples.
Injured was installing a rotation unit on a roof bolter on the #2 unit. The injured picked up the rotation unit and received a laceration on his right pinky finger from a sharp edge on the rotation unit.
Lime was dropped into the rock dust drop hole instead of rock dust. The injured inhaled the lime dust and the dust irritated his lungs.
The injured was putting up a cable bolt. The cable bolt hung on a rock. The injured pulled on the cable bolt and caused the pie pan to move resulting in a laceration to his left arm near his inner bicep. The laceration required 6 stitches
Injured was attempting to tighten a pressurized was line fitting at the water tree. He pulled at the hose and the hose came off and hit him in his forehead. He received 6 stitches
Injured was lifting miner cable. He was holding it against the top when another miner tried to pull slack in the cable pulling the injured' s extended right arm backwards. The injured felt a pop in his shoulder. Injured has a shoulder strain.
Injured was building a brattice and he felt a pain in his right knee. Knee was swollen and sore the next day.
Injured cut right hand thumb with a belt knife while cutting a piece of suction hose for a Fletcher roof bolter. He received 5 stitches.
Injured cut finger while installing a flex plate on a diesel scoop engine. He was tightening up flex plate bolts when the plate broke and cut his hand. He received 5 stitches on his finger on his right hand.
The injured was lifting/pulling the pod rock duster hose. He was bent over and twisted. The injured coughed and felt a sharp pain in his right rib cage area.
There was a rock fall inby the loading point on the #1 unit MMU-002. The fall occurred in 9R and the intersection in 10 at cross cut 35 spad # 21+60. There was no interruption to ventilation, no one was inby the fall when it occurred and no one injured.
Roof bolter was installing the inside pin in the #10 entry on the #1 unit (MMU 002). A piece of draw rock measuring 2.75' x 2.75' x 2"" thick fell from between the rib pin and the rib making contact with the roof bolter's left foot bottom. He received a contusion and sprain as a result.
Employee was assisting in changing the tire on a roof bolter in the #2 entry on the #2 unit MMU 003. The spare tire was standing beside him and fell over making contact with his hand. A fracture may be present pending an MRI the week of 03/02/2015. Injured sought medical attention on 02/27/2015. He has a splint/brace on his hand.

EE was dragging cable for a power move on 3rd shift and his back made contact with a damaged pin plate that was hanging down. He received a laceration on his back that required 16 stitches.
THE INJURED WAS TRAMMING THE MINER OUT OF THE #4 ENTRY FACE XCUT IN THE FIRST PANEL SOUTH ON THE #1 UNIT AFTER FINISHING A CUT AND WAS STRUCK BY A PIECE OF ROCK THAT FELL OUT FROM BETWEEN THE RIB AND OUTSIDE PIN ON THE INTAKE SIDE OF THE ENTRY. THE ROCK STRUCK HIS LOWER BACK.
The injured was carrying 2 sections of belt structure railing on the back of a 4 man diesel Wallace. The injured turned the corner to enter the #2 entry and the rail caught the rib causing the rail to swing and make contact with the injured's face. The injured was looking to his right side at time of impact.
cutting strap on bundle of gloves, was cutting away from body with razor knife when knife slipped and cut his thigh. (stitches)
The injured was roof bolting on opposite operator side. He was putting up his outside roof bolt. He was straightening his pin when a pie pan slide down the pin making contact with his left forearm causing a 1/2"" laceration.
While operating a double boom Fletcher roof bolter in the #9 entry bolting a turned left on the #1 unit, while installing the outside rib pin on the opposite operator side, the injured was struck by a piece of draw rock that fell out from between 2 pins. The rock fell out and struck the right ankle. He suffered a hairline fracture that required a boot cast be worn.
EE was securing metal flashing using self drilling screws. EE was putting a screw, felt something went in eye. EE reported incident his eye was flushed out. Claimed to feel ok went home. Next day, claimed still felt something in eye and was taken to clinic. At clinic, eye was flushed out and prescribed antibiotics. Was rechecked on Monday 03, and medically released

Employee was installing a brace for a steel beam when the angle-iron brace flopped over and mashed his right index finger requiring stitches. He was wearing leather gloves at the time. Employee will be on light duty for several days until released by the physician.
Fellow employee closed lid to powder box, and caught injured worker's finger between lid and frame. This resulted in sutures to the injured finger.
