Employee climbed step ladder to fuel Roll Plant in quarry. When descending, both feet on ground, one foot was on top of a rock. EE pivoted & foot on rock caused to lose balance, did not fall. EE was able to walk on the foot. Told supv. that they "took a digger". After seeking treatment, determined to be a R ankle sprain. We accommodated light duty and there was no lost time.

The injury occurred when a trained miner fell from a height of 4 to 5 feet onto the rock floor of the quarry. The condition contributing to the fall was the miner placing himself in an elevated position with no tie off. There was one injured party.

WHILE REMOVING A WORN PIECE OF SCREEN CLOTH FROM SCREEN BOX, THE OPERATOR TURNED TO RELEASE SCREEN PANEL TO ALLOW IT TO FALL TO THE GROUND. WIRE BECAME ENTANGLED IN OPERATORS GLOVE DRAGGING HI M TO THE GROUND.

OPERATOR WAS IN THE PROCESS OF SHUTTING DOWN PIECE OF EQUIPMENT WHEN HE SLIPPED ON A SNOW COVERED ICED OVER SUPPORT IRON FOR CLUTCH ASSEMBLY STRIKING HIS LEFT SHOULDER.
