Employee was working in a ditch installing conduit for electrical wiring when EE got EE's right foot stuck in the mud. When EE lifted EE's foot, EE strained EE's right hip. This became an MSHA reportable injury on 03/23/2016 when EE went to see an orthopedic specialist and will now be receiving therapy.

Showing all 10Employee exited the CAT D6 Dozer because the fire extinguisher was rattling against the window. When he went to re-enter the cab, he took a step and his foot missed the track. He fell to the ground injuring his back and left hip.
Employee had descended a set of steps and as he was turning to walk another direction he felt a pop in the back of his right leg.
The injured man lost his footing and caught himself with his left hand to prevent a fall, but in the process, his left shoulder was dislocated. He missed the next work day and then he is expected to be on restricted duty for two weeks.
EE was replacing slury line when he slipped in the mud and twissted his left knee causing a strain. He came back to work with restrictions.
WHEN EE WAS STARTING HIS SHIFT HE TRIPPED AND FELL. THERE WERE NO SYMPTOMS AND HE CONTINUED TO WORK UNTIL NOV 6 WHEN HE STARTED TO TREAT WITH HIS DOCTOR FOR HEART PROBLEMS. HE WAS THEN OFF WOR K ON NOV 13 IT WAS DECIDED THAT HIS HEART WAS OOK AND THAT HIS PROBLEMS WERE DUE TO THE FALL.
THE MAN FELL IN A HOLE ON THE RIVER BANK CAUSING BRUISES, LACERATIONS AND A TWISTED LEFT KNEE AND SHIN ON AUG 23,01 HE NOTICED SOME INFECTION AND WAS TREATED FOR THE INFECTION THERE WAS NO LOS T TIME, OR WERE THERE ANY RESTRICTION TO WORK.
THE INJURED MAN FELL WHEN HIS LADDER SLIPPED. HE FELL ON THE LADDER AND BROKE 4 RIBS.
THE MAN FELL ON AN ICE PATCH ON THE GROUND, HURTING HIS LOWER BACK.
THE INJURED MAN WAS EXITING HI LIFT WHEN HE MISSED THE LAST STEP AND PULLED A BACK MUSCLE. HE IS ON LIGHT DUTY FOR AN UNKNOWN PERIOD OF TIME.
