Caught in, under or between a moving and a stationary object

Employee was using a pry bar to pry a jammed rock in the chute. The bar slipped and EE pinched EE's fingers between the bar and the chute. EE received a closed fracture on the 5th tuft of EE's right hand.

Caught in, under or between a moving and a stationary object

The employee was shutting the door on the haul truck and got middle finger on left hand between the door and the door frame. This caused a laceration to middle finger.

Over-exertion (Not Elsewhere Classified)

Employee was working in a ditch installing conduit for electrical wiring when EE got EE's right foot stuck in the mud. When EE lifted EE's foot, EE strained EE's right hip. This became an MSHA reportable injury on 03/23/2016 when EE went to see an orthopedic specialist and will now be receiving therapy.

Employee hit his head on a piece of angle when he went to climb up a fixed ladder. He suffered a laceration which required 7 staples.

Contact with hot objects or substances

Employee was using a torch to melt ice when the torch turned in his left hand and the flame went across the back of his right hand. He suffered a 2nd degree burn.

Struck by flying object

Employee was turning a water valve so he could adjust the spray bars on the 2B screen when the valve came apart and dirty water got into his eyes. The employee went to the emergency department where his eyes were flushed and a prescription was issued for a corneal abrasion.

Employee was sitting in rock truck under 2B bin. A rock fell off plant and struck his left index finger which was resting on the door. The rock broke his finger and he also needed stitches for a laceration.

A piece of equipment was lifting a 3"" pump. The pump slid forward and pinched the employees finger between the pump and handrail. The employee received a broken finger and stitches.

Employee was installing conduit and he contacted poison ivy. Went to hospital on 11/12/10 for an infection.

The man was welding when the steel he was welding fell on the top of his right foot causing a severe bruise. He will be on light duty until the pain and swelling subside.

THE INJURED MAN WAS EXITING HI LIFT WHEN HE MISSED THE LAST STEP AND PULLED A BACK MUSCLE. HE IS ON LIGHT DUTY FOR AN UNKNOWN PERIOD OF TIME.

THE MAN WAS BENDING TO PICK UP A PIECE OF SCRAP. HE PULLED A MUSCLE IN HIS BACK. HE WAS ON LIGHT DUTY FOR 4 DAYS AND RETURNED TO REGULAR WORK POSITION.
