Showing all 31Miner conducting a work area inspection. Miner looked up at approaching pickup headlights and slipped on a patch of ice. X-ray taken 12/12/2024 no fractures and released to full duty. MRI referral was made on January 1/24/2025. 1/27/2025 rotator cuff tear was diagnosed. On 2/3/2025 referred to Orthopedic specialist for surgery.
Employee was closing the lids on top of a railcar when they tripped and fell forward. The employee was wearing their fall arrest and using a retractable lanyard. When they fell their arm was caught by the lanyard, resulting in a shoulder separation. Employee had surgery on 11/14/2023. Worked until then.
Employee was stepping off of forklift, felt slight twinge in right knee. Worked for several hours while knee was sore. After taking a break for lunch employee was unable to bear weight on leg. Waiting for doctor diagnosis.
Employee fell down a flight of 10 metal stairs in the Hydrating building. Employee failed to firmly plant foot on first step and failed to maintain 3 points of contact. Employee suffered minor bruising, lacerations and friction burns to their knees, shins, left cheek, left elbow, left shoulder, neck and right hand.
When stepping out of vehicle in the morning prior to shift, employee slipped in the parking lot. It had snowed the previous night and was snowing still that morning. Strain to right knee.
Employee slipped on ice while exiting vehicle in parking lot. The employee landed on their elbow. Initially, employee did not report an injury. On 3/22/2019, employee reported to work with note stating they could not return to work for 2 weeks. On 4/15/2019 the injury was determined to be work related.
Employee was raking ash, the ash rake slipped off the large piece of ash and the employee lost his footing. Employee fell to the floor and landed on his left wrist. Left wrist fractured.

Employee was checking the level of the bucket elevator shaft when he slipped on ice accumulation on the grating. As he was falling, his arm caught on the handrail causing a break in the upper right arm.

EMPLOYEE WAS HELPING TO INSTALL NEW SCREENS ON THE S131 SCREENER. HE WAS USING TWO PRYBARS TO LEFT THE SCREENER DECK HOUSING. HIS FOOT SLIPPED CAUSING HIM TO LOSE CONTROL OF THE WEIGHT. HE ALLEDGEDLY FELT A SHARP PAIN IN HIS LOWER BACK.
EE WAS WALKING PAST THE CASE SKIDSTEER. HIS FOOT CAUGHT THE EDGE OF THE LOADER BUCKET. THIS CAUSED EE TO LOSE HIS BALANCE AND FALL FACE DOWNONTO THE GROUND. EE DID NOT SEEK MEDICAL ATTENTION UNTIL MARCH 1, 2004.
EMPLOYEE WAS CLIMBING DOWN THE LADDER ON A RAILCAR. HIS FOOT SLIPPED OFF THE LAST RUNG CAUSING HIM TO FALL AGAINST AN ADJOINING RAILCAR WITH HIS LOWER TORSO. HE THEN FELL TO THE GROUND LANDING ON HIS LEFT SIDE. HE DID NOT SEEK MEDICAL ATTENTION UNTIL FEB. 2, 2004.
WHILE DESCENDING THE STAIRS IN THE HYDRAT PLANT. EE S FOOT SLIPPED ON THE BOTTOM EE THEN FELL FORWARD ONTO THE HYDRATE FLOOR. HE DID NOT FEEL ANY PAIN AT THAT TIME. ON 12-9-03 HE DID FELL A DU LL PAIN IN THE MIDDLE OF HIS BACK. THE PAIN IS STILL THERE AS OF TODAY 12-11-03.
EMPLOYEE SLIPPED PARTIALLY INTO A CATWALK GRATE OPENING. HE HAS A LACERATION ON HIS LEFT LOWER LEG THAT REQUIRED SUTURES.
EMPLOYEE WAS PULLING AN EMPTY BULK BAG OUT OF A BUNDLE OF BAGS. HE FELL BACKWARDS ONTO THE CONCRETE FLOOR, EE HIT HIS LEFT ELBOW/ ARM ON THE FLOOR.
EE WAS DESCENDING FROM A FIXED LADDER. HIS LEFT HAND SLIPPED: EE FELL APPROX. 4 FEET TO A CONCRETE FLOOR. EE THOUGHT HE HEARD A "CRACK" AND FELT PAIN IN RIGHT ANKLE. TWO FELLOW EMPLOYEES STABI LIZED ANKLE: ICE PACKS PUT ON AND EE TAKEN TO EMERGENCY ROOM.
EE WAS STEPPING DOWN FROM THE PAYLOADER. HE FELT A PULL IN HIS LEFT LEG.
IT IS ALLEGED WHEN EE WAS GETTING OFF THE TRACKMOBILE, HE SLIPPED AND FELL AGAINST THE TRACKMOBILE. HIS RIGHT SHOULDER AND RIGHT ELBOW ARE STIFF AND SORE. EE HAS GONE TO DR ON 8-15-02, HE IS O N RESTRICTIONS WITH A POSSIBLE RORTATOR CUFF INJURY.
WHILE PEROFRMING MAINTENANCE WORK, THE EE STEPPED ON A CATWALK AND THE GRATING GAVE WAY. EE FELL THRU WALKWAY AND LANDED ON A PIPE BELOW. EE DID SEEK MEDICAL ATTENTION.
EE WAS INSPECTING THE TRUCK HE WAS DOING MAINENANCE ON WHEN HE TRIPPED OVER A PALLET LYING ON THE GROUND. HE FELL ONTO A CONCRETE SLAB. HE TRIEDTO CATCH HIS FALL BUT HIT HIS RIGHT FINGER ON A PIECE OF IRON ON THE CONCRETE SLAB. HIS FINGER BECAME SWOLLEN. HE DID NOT SEEK MEDICAL ATTENTION; FINGER IS FRACTURED.
EE WAS ENTERING THE DUST COLLECTORS. HE SLIPPED ON THE LADDER AND FELL FORWARD INTO THE DUST COLLECTOR. HE LANDED ON HIS RIGHT HAND. EE DID SEEK MEDICAL ATTENTION.
EMPLOYEE STUMBLED WHILE WALKING AND FELL FACE DOWN INTO A METAL POLE. EMPLOYEE DID SEEK MEDICAL ATTENTION.
EE MISSED A STEP WHILE CLIMBING DOWN FROM THE PREHEATER. HE NOTICED SWELLING AND PAIN APPROX ONE HOUR LATER. EE WAS TAKEN TO HOSPITAL.
WHEN EE WAS DONE LOADING A PEBBLE TRUCK, HE WAS WALKING TO THE MIRACLE PLANT AND SLIPPED ON THE ICE, RESULTING IN PULLING A GROIN MUSCLE.
EMPLOYEE FELL OFF HIS TRUCK WHILE TARPING HIS LOAD. HE FELL FACE FIRST ON THE PAVEMENT. HE WAS TAKEN BY THE RESCUE SQUAD TO THE HOSPITAL. CORPORATE OFFICE NOTIFIED UMTHUN TRUCKING IMMEDIATELY AND WE CORRESPONDED HOSPITAL INFORMATION TO THEM
