EMPLOYEE WAS WALKING TO HIS WORKING AREA AFTER LUNCH. SUDDENLY HE SLIPPED AND FELL ON THE FLOOR CAUSING CONTUSION ON HIS LEFT KNEE AND ELBOW.
EE WAS ACCOMODATING A FILLED CEMENT BAG ON A PALLET ON THE MOVABLE PLATFORM ON PALLETIZER #2. WHEN HE PUSHED THE CEMENT BAG, HE LOST HIS BALANCE AND FELL BACKWARDS ON THE FLOOR.
EE WAS ALIGNING CONVEYOR BELT E. TO ACCOMPLISH THE TASK, HE NEEDED TO SIT ON A GRATING TO POSITION HIMSELF ON TOP OF CHUTE. WHEN HE WAS CLIMBING TO SIT DOWN, HE HIT THE GRAING, STUMBLED AND PA RTIALLY FELL INSIDE THE CHUTE CAUSING TRAUMAS ONHIS RIGHT ARM, BOTH KNEES AND LEFT FOOT.
EE WAS CLIMBING DOWN A LOADER'S LADDER AFTER HE FINISHED THE REPAIR. SUDDENLY HE HIT THE LADDER WITH HIS LEFT KNEE CAUSING PAIN AND A CONTUSION.
WHILE INSTALLING A SLING TO VISE A BEAM HE TRIPPED AND HIT THE RIGHT SIDE OF HIS FACE WITH ANOTHER BEAM, NEAR THE RIGHT EYE.

EE WAS WALKING AT THEH ADMINISTRATION BUILDING. SUDDENLY SHE SLIPPED AND FELT BECAUSE THE FLOOR WAS BEING WAXED. SHE HAD KOWLEDGE THAT THE JANITOR WAS CLEANING THE FLOOR.
EE WAS CHANGING A DUST COLLECTOR MOTOR AT CEMENT SILOS. WHEN HE BEGAN TO WALK DOWN THE STAIRS HE TWISTED HIS LEFT ANKLE.
INJURED WAS PLACING STEEL BEAMS IN KILN'S CLINKER COOLER. WHILE HE WAS PERFORMING HIS TASK HE SLIPPED INSIDE THE COOLER AND STRUCK A BEAM CAUSING HIM CONTUSIONS, BACK PAIN AND CERVICAL STRAIN.
