Employee was getting into his loader and missed the last step and fell. He hit his chin on the upper step and cut his left hand on the door latch
Reported to Irvings on 02-14-14 no medical action taken, On 4-24-14 MRI requested showed Rotator Cuff Sprain, No lost time Working, Pain Level higher Talked with Medical on 5-21-14 Requested time off. Surg on shoulder off work on 6-3-14 will be off app. 3 Months. Slipped on ice exit a loader
Employee was working in the scale house and walked out to the safe area and tripped on some rocks and fell. EE refused medical attention at the time of the incident. About three hours later EE asked to go into RediMed and have her hand check out. RediMed stated EE had fractured her Left wrist and it was going to be splinted. At this time she was able to return to work.
Working on ground by water pipe. Employee was stung by several bees. First report employee pulled a calf muscle and twisted his left ankle.
EE was using a step ladder to had oil to his loader. His foot slipped from the step and he fell from the ladder. He was app 3 foot from the ground when he slipped. He complained of pain and refused medical treatment. On Tues. 4-23-13 he requested medical for his knee and ankle. RedImed treated and put him on lite duty until 4-30-13.
EE reported he slipped, when he was working on a 4 foot step ladder. This was reported on 5-8-2012 but refused medical treatment. I was contacted on 5-9-2012 by EE and he requested medical treatment at RedIMed. EE complained of pain in his Right Elbow and forearm.
Operator reported he twisted his ankle but not sure if it was at work or home on 4-25-2012. On 5-2-2012 employee was put on lite duty By Doctor and was told to wear a boot cast to relieve the pain. Employee was moved from the work site to the office. No MISSED Work.
Employee had stepped across the frame of a conveyor when he slipped and twisted his left knee, causing great pain and swelling.
Employee was helping replace a side panel on the crane, when he slipped off the top of the battery box and fell onto the track of the crane. He then rolled from the crane track and fell to the ground app. 4 feet.
The employee fell from app 8'. The employee was setting on a I-Beam and leaned out to push on a second I-Beam. The I-Beam slid causing the employee to fall from where he was setting. No safety equipment was being used at the time of the accident. ( Second time this was reported- no report no. on file )
