Showing all 9Employee was walking down the stairs on the plant when EE lost EE's balance and fell. The fall resulted in a broken rib and dislocated shoulder.
Employee was completing a pre-operation check of a skid loader when EE stepped off of the skid loader track into a hole and twisted EE's ankle.
Employee was backing up a trailer during a time of snowfall, and, because of ground conditions caused by the weather, the trailer became stuck. When the employee exited his truck to assist in freeing it, he slipped and fell as he was walking, and landed on his upper back / ribcage area. The fall resulted in a strained back muscle.
Employee was stepping down on the bank next to the dredge when his knee gave out. The employee and foreman did not report the incident until 1/26/09. The employee went to the doctor on 1/26/09 where it was diagnosed as a sprain. The employee returned to work on the same day and did not miss work.
