Fall to the walkway or working surface

Employee was backing up a trailer during a time of snowfall, and, because of ground conditions caused by the weather, the trailer became stuck. When the employee exited his truck to assist in freeing it, he slipped and fell as he was walking, and landed on his upper back / ribcage area. The fall resulted in a strained back muscle.

Over-exertion in lifting objects

Employee suffered a back strain while removing an 8' fiberglass ladder from a wall mounted storage rack. Employee was taken to a clinic for evaluation and was given a prescription for a muscle relaxer. It was also advised that he be restricted to Light / Medium lifting duties.

Struck by flying object

A foreign object made its way into the ee's right eye when grinding on a metal drive sprocket. Safety goggles were worn by the ee but continued to fog up. The foreign object may have entered the eye when removing the goggles to clear the lens. The ee's eye was irritated & required medical attention of removing a rust spot & prescribing an antibiotic to clear up the irritation.

EE was holding a steel tubular spring with a apair of vise grips while another EE attempted to cut the steel with a hacksaw. The hacksaw slipped and cut the EE's hand resulting in four stitches. Both EE's were drug tested per co. policy. The injured EE tested positive for illegal drugs and was suspended until a 2nd and conclusive drug test is completed in approx. a week.

EE was moving a metal component approx. 30 pounds when he lost his grip and dropped the object. The metal component bounced off of the concrete slab and hit his left shin, which caused a laceration.

Inhalation of radiations, caustics, toxic and noxious substances

Employee had respiratory allergic reaction to fumes in the shop caused by heating of a urethane liner attached to a steel chute being worked on. The allergic reaction caused asthma like symptoms and required medication to counter the reaction.

Struck against stationary object

Employee was lifting a metal wear shoe out of a steel 55 gallon drum when he cut his hand on a sharp piece of metal on the inside of the steel drum. The laceration of the hand required eleven stitches.

Shop foreman slipped on ice outside of shop and fell. Left ankle was twisted and strained. Scraped right knee. Did not require doctor visit date of incident. He requested doctor visit on 2-15-07. Doctor diagnosed minor swelling & prescribed Tylenol as needed and follow up doctor visit on 2-26-07.

Lifting an x-brace, lost footing in sand causing finger to get caught between x brace and a piece of beam. smashed middle finger on left hand causing two lacerations and broken finger tip.

WHILE WELDING HE PULLED TRIGGER ON MIG WELDER BEFORE HELMET WAS COMPLETELY DOWN. HOT SPARK FLEW INTO LEFT EYE BURNING CORNEA & CONJUNCTINAL SAC.

HOISTING A GEARBOX, THE RIGGING BROKE AND GEARBOX FELL TO FLOOR AND ON EE'S LEFT FOOT. BROKE A BONE IN LEFT FOOT.
