Walking from shop area to jobsite employee felt and heard a "pop" in right knee which caused employee to fall down. The knee was looked at that morning and a small fracture to the kneecap was diagnosed. Results of an MRI are pending as of 2/13/24.

As a utility operator was descending from the bricking rig platform, EE's foot slipped. EE tried to catch self with one hand on the platform access swing gate, and EE's thumb was pinched between the vertical member of the gate and its support post. EE received stitches and was diagnosed with a fracture to the tip of thumb.

While hand-wheeling a 380 lb. tractor drive wheel into position to be changed, a haul truck driver lost control of the wheel and then fell onto the wheel, resulting in 2 broken vertebrae in EE's back.

Employee lost footing on roadway and popped his right knee when he caught himself. This incident was not reported because of hearing dispute that was finalized last week. There was surgery to fix the tear in his meniscus. Stan decided to quit his job before returning to work.

While climbing up the ladder on 415 loader, at the 2nd rung, Employee missed grabbing the handrail with his left hand. As a result, he swung backward and to the right, hitting the left-rear loader tire with his back and went to the ground. Upon landing, he hit the ground with his left palm and right elbow.

Employee was stepping off a loader, one foot was on the ground as he placed the other foot on the ground he slipped on the snow, fell forward landing on his left wrist. Employee was taken to the hospital examined by ER doctor an x-ray was taken, diagnosis was a broken wrist.

Employee was working on top of the screw conveyor housing which is 18"above the catwalk. He stepped off of the screw, leading with his left foot. When his foot touched the catwalk his knee popped then twisted in towards his right knee. His upper body twisted to the left. EE felt instant pain in his left knee & it caused his to fall to the catwalk.

Ee tripped on a pipe as he stepped backward. As he fell he grabbed a handrail to break his fall. Ee placed his entire body weight on his right arm & shoulder area. At the time ee stated he was fine & continued to work. He had pain in his shoulder area that evening after work while at home. The next day, Dr diagnosed a tear of the deltoid muscle & rotator cuff contusion.

EE states that he twisted his right knee when he slipped on a pile of coal that was spilled on a stairway. EE had his knee examined by an orthopedic physician, and was initially diagnosed with a sprain and released o full work duties. Re-examined on 8-13-04 when he was diagnosed with a lateral tear of the meniscus.
