Showing all 42Employee was flagging a shot at around 2:00pm, suddenly the ground gave way and the employee fell in a hole, the right leg outside the hole, the left leg inside the hole, the right knee was at about EE's chest, then EE felt a pop on EE's right knee, EE then called a co-worker to help EE get up.
Employee was walking between equipment and slipped on the ice.
EE exited EE's truck on the lower scale and started climbing the steps to the scale house. EE tripped on the top step and hit the bridge of EE's nose on a structural beam. EE exclaimed that EE was in a hurry and that is what led to EE tripping and falling. EE's nose was broken as well as a minor spinal cord injury from the impact of the fall.
While exiting truck on scale, lost balance and fell against safety rail. Twisted knee when falling against rail.
Slipped when exiting truck falling to the ground.
The employee was parking the grader in the shop to begin routine maintenance. When EE was climbing down the grader ladder (using three points of contact) EE lost EE's footing and grip with EE's left hand, EE caught self with EE's right arm/hand. EE originally thought EE just pulled a muscle but the range of motion and pain level continued to decline.
Employee was working inside kiln on brick rig platform. EE's foot slipped and fell off of the platform. Employee landed on EE's right foot, fracturing the ankle.
Employee was cleaning frozen chunks of coal and snow from the solid fuel grizzly when EE slipped and EE's foot/leg dropped into the grizzly. As the employee fell EE twisted EE's back.
Mechanic starts equipment in the morning. The mechanic was coming down after starting a haul truck and slipped on the ladder system. Sprained/strained right knee. The instance was noted at the time but EE did not seek medical care. Evaluation was sought in April and surgery was completed on 5/22/19 on EE's knee.

Climbing down from the 970 loader, operator missed the last rung and landed wrong (sprained knee). There was originally no "injury" as the medical evaluation found no injury. However, employee recently had surgery on knee and will miss a week of work recovering.
Note: employee did not seek medical for significant time following actual date of injury. Med status for days away 1/9/2018. Installing cylinder on loader, the cylinder slipped and forced weight to the shoulder causing their leg to slip out from under them causing strain to hip/shoulder.
Employee was working on a ladder removing a pipe. The pipe came free and swung into the ladder causing EE to fall and sprain ankle.
Descending a ladder the employee missed the second to last rung slipped and jarred his back. The employee reported the incident then. On 2/1/16 he reported to his supervisor that his back was giving him problems. He was taken to the emergency room on 2/2/16 for evaluation. He then followed up with his own physician which prescribed an anti-inflammatory prescription on 2/3/16.
employee was exiting the kiln and lost his footing on a ladder. he slipped from the ladder and landed about 4-5' below on the grating. employee was taken to a clinic as a precaution and found to have a minor fracture in his back.
Employee was walking across ground - fell on loose gravel lacerating left lower arm.
EE came off the ladder with his tool bucket, did not see opening. He fell approx 3'. His chest hit his tool bucket, which caused the bruising.
Employee slipped while loading a truck causing a sprain to his knee. It is thought that the employee tore his meniscus.
Under investigation. The individual involved in this incident is contracted by one of our customers to pickup product at our plant. We have been requesting follow-up information since the day of the incident, but have not received any information or documentation from our customer or from the individual involved in the incident. We do not know yet if this incident is reportable.

EE twisted knee. Addendum: Accident happened on 9-7-07 but was not reportable at that time. On 9-20-07 had to go to doctor, doctor drained knee, wants to do MRI.
Employee fell while wearing a dust mask with plastic exhalation valve and lacerated his right nostril requiring stitches. Most likely cause of the cut was from the edge of the plastic valve on the dust mask.
Accident is under investigation (EE slipped and hurt his back setting up a rail car).

DRIVER FELT LEG SNAP AS HE CAUGHT HIMSELF FROM FALLING, DRIVER REPORTED LEG WAS BROKEN AT SAME SPOT BEFORE.
EMPLOYEE WAS STEPPING DOWN FROM A PIECE OF EQUIPMENT ONTO A WALKWAY AND TWISTED HIS LEFT KNEE. NO IMMEDIATE INJURY, LATER THE KNEE SWELLED. EMPLOYEE REPORTED IT TO MANAGEMENT ON 10/17/01.
EMPLOYEE TRIPPED ON STEP COMING OUT OF COOLER PIT, TRIED TO GRAB HANDRAIL AND TWISTED LEFT SHOULDER. INJURED ROTATOR CUP. SHOULDER DID NOT GET ANY BETTER SO HE WENT TO SEE A DOCTOR ON 2-6-01. EMPLOYEE WAS GIVEN A PRESCRIPTION AND WILL BE TAKING PHYSICAL THERAPY.
SLIPPED ON ICY SLOPE AND FELL FRACTURING HIS HIP.
WHEN CLEANING RAILCAR, A GATE WAS JAMMED & WHILETRYING TO CLOSE WITH A PRY-BAR PUSHING WITH BODYWEIGHT, IT BROKE LOOSE--CAUSING EMPLOYEE TO FALL1-1/2' AND RE-INJURED BACK.
WHILE CLEANING #1 PREHEATER LOST BALANCE AND FELL THREE FEET AND LANDED ON WALKWAY BELOW ON BACKAND NECK. WAS STANDING ON PUSH RAM.
FEEDER PAN POKE HOLE CAP WAS NOT ON. GOT A LADDER AND CLIMBED UP TO PUT CAP BACK ON CAP WOULD NOT FIT SO WENT TO STEP OFF LADDER AND FELL TO GROUND ABOUT 3 FEET LITTLE FINGER ON LEFT HAND CAUG HT HAND RAIL ON THE WAY DOWN AND BROKE.
SLOPING GROUND COVERED WITH SNOW AND LOOSE ROCKS CAUSED EE TO TO SLIP AND FALL STRIKING HIS ELBOW ON THE GROUND. EE HAD SORENESS IN HIS ELBOW UNTIL MARCH 24, 2000 WHEN HE VISITED A DR. INCIDEN T WAS NOT REPORTABLE ON 2/2, NOW IS REPORTABLE.
