Showing all 23Employee was breaking a bolt lose on the crusher when it broke free causing EE to lose balance. EE fell backwards and attempted to catch self, causing EE to strain right wrist. Over the course of the day, it become sore. Diagnosed with a sprain but did not improve. Referred for an MRI and results reviewed on 03/19/24 showing a scaphoid fracture.
Tripped and fell over a hold plug, hurt right ankle
Employee states that right foot became stuck in the mud on Monday, January 3, 2022. When employee attempted to remove foot from the mud, employee twisted right knee. This has been a first aid only injury until 03-01-2022. On this date, an orthopedic surgeon placed employee on restricted duty, changing injury status.
Employee fell down on EE's right side and caused a sprain to EE's right wrist. The incident became recordable on 02-15-2022 due to physical therapy. Prior to this date, it has been a first aid only incident (hence the date of accident as 12/01/2021)
The employee had just finished moving a portable generator when they slipped and fell, striking an elbow on the deck of the dredge. This action resulted in the employee sustaining a closed fracture to the left ulna bone near the elbow.
Employee was working in feeder area. Employee lost footing, fell forward, and struck forearm, causing a fracture.
Employee was climbing onto a front end loader to begin work for the day. EE had EE's lunch box in one hand and the other hand was on the hand rail. The employee slipped/tripped and fell pinching EE's hand between EE's body and step. This caused a fracture to the employees right pinky. The employee did not report the incident until 7/30/2018.
Had just repaired the drill and was watching it drill to make sure repairs worked when a hose blew apart. Took off running to get away and blew out left knee.
Installing engine on Bulldozer. Climbed up on push arm and rt Ft slipped and fell to the ground catching himself with his right hand and arm and jamming his rt shoulder. Saw Dr., taking MRI and PT.
Employee was dragging a piece of 8" PVC tubing attempting to attach it to the water pump on a barge and stepped backwards into a gap between the barge and catwalk. He fell down and struck the backside of his left upper arm on the corner of the handrail which caused a laceration that required five stitches.
Employee was attempting to access the dredge via the skiff boat. He stepped over onto the skiff boat without realizing there was frost on the seat. Employee slipped on the frost and fell back and hit his head. This caused a laceration that required five staples.
Employees foot slipped from step and twisted back
Employee was helping to change screens. He stepped off onto a slope and sprained his knee.
Employee fell from walkway & fractured his left ring finger and received a laceration to his right leg.
Employee was going down the stairs at the industrial sand plant and tripped on the step.
EE's right ankle rolled to outside when he stepped down from control house @ Portable Crusher #1 and stepped on some sand that filled in an area around the ground rod--sand gave way under his weight. Result: sprained right ankle and injury to distal fibula.
EE was walking to the plant and soil caused with and caused him to trip and hurt knee was a first aid case until dr drained fluid off knee 2-14-05
INDIVIDUAL AND OTHER EE WERE ATTEMPTING TO JUMP START TRUCK. HE BEGAN TO WALK ALONG THE FRAME TO CHECK THE CONNECTIONS WHEN HE STOPPED ON THE BATTERY COVER THAT WAS LAYING ON THE FENDER. IT WA S TOO DARK TO SEE WHERE HE WAS STEPPING. HE FELLTO THE GROUND APPROX 4 FT AND HURT HIS RIBS LAYING ON THE GROUND.
EMPLOYEE WAS PULLING A WATER HOSE AND SLIPPED ON SOME ROCKS WHEN HE FELL HE INJURED HIS LEFT KNEE.
EMPLOYEE WAS CLIMBING LADDER ON THE PLANT, HAD MUD ON HIS FEET AND SLIPPED. THIS CAUSED HIM TO FALL AND HURT HIS LEFT ANKLE.
EE WAS STANDING ON RADIAL STACKER CONVEYOR. HE PULLED ON WINCH CABLE AND SLIPPED AND FELL.
EE WAS STANDING ON A LADDER WORKING ON A 988 CAT LOADER. A FOOT OF THE LADDER SANK IN THE SAND AND CAUSED THE LADDER TO LEAN. THE EE SLIPPED AND LOST HIS BALANCE, SLID DOWN THE LADDER AND CUT HIS CHIN.
