Employee sprained their left big toe while exiting a screen deck. After three weeks, the injury did not resolve. Follow up with doctor on 03/24/26 resulted in same diagnosis, but new requirement of work boot to stabilize the toe.

Employee was driving through the yard in a service truck. They struck a concrete block, resulting in a laceration below the right eyebrow.

On 12/04/2024 at 01:29 pm, an aggregates maintenance mechanic was climbing into a trackhoe and attempted to slide the seat back when IW felt a pop and burning sensation in left bicep. The IW RTW full duty immediately. The IW underwent an MRI which revealed a torn left biceps tendon. Results were made available on 12/11/2024.
Employee was removing the end of bolts on a flexco fastener. The grinder came back towards the employee lacerating the side of EE's right knee.

Employee was exiting the equipment. As employee went to twist around to face out, one foot got caught on the other foot. Employee heard a pop and quickly felt pain in right foot.
Crew was removing a bearing from the shaft at the shaker. They were trying to get the race free. The employee struck the bearing resulting in shattered metal coming back at the employee's hand. Employee was wearing gloves but the metal cut through to the top of the hand.

On 12/16/2022 the employee was slinging chains around a concrete block when EE felt a twinge in the neck. EE did not believe it to be serious and did not report it to the supervisor. On 12/19/2022 EE was removing a coupling when EE felt the twinge again. On 12/20/2022, the employee woke up experiencing severe neck pain and stiffness. Employee was diagnosed with a neck strain.

Employee was servicing the 6"" Multiquip pump. EE removed the filling cap and pressure from the reservoir caused hot water to escape the cap. EE received burns on the left arm, side, groin and thigh area. EE was transported to Hillcrest Medical Center in Tulsa for treatment.

Employee received a foreign body in the left eye when a piece of slag got behind welding hood. The employee did not report the injury until 2/11/20 because they did not realize anything had entered the eye resulting in the foreign body having to be removed by a ophthalmic burr.

Employee was asked to move water pumps. EE drove EE's service truck to the first pump. As EE reached down to pull the ring on the cam lock, the ring pulled opened and sliced the employees left middle finger. Requiring stitches to close the laceration.
Employee wrongfully entered a permit required confined space by self to clean material that had accumulated inside the chute. A large chunk of fines broke free and struck the employee in the lower left leg causing fractures to both lower bones in the left leg.
P5 best stopped loaded with rock causing the drive belts to burn off. Employee was shoveling/raking rock out from underneath the belt and felt a sharp pain in EE's left shoulder.

Employee was climbing onto a front end loader to begin work for the day. EE had EE's lunch box in one hand and the other hand was on the hand rail. The employee slipped/tripped and fell pinching EE's hand between EE's body and step. This caused a fracture to the employees right pinky. The employee did not report the incident until 7/30/2018.

The employee was cleaning out an unused silo that had filled with material. Employee used the handle of the shovel to see if debris blocking the door was movable while standing to the side. The door itself, which is steel & approximately 24"" x 24"" & of a clam-shell design, broke loose from the concrete ceiling. It fell to the ground 7' below, striking employee in the lower leg.

Miner was removing aggregate material from a stopped conveyor belt. He was working out of a manlift basket when he felt a burning sensation in his right shoulder. He did not want to be examined at the time. He was taken to a clinic where he was diagnosed with a rotor cuff strain in his right shoulder. He received an injection and was prescribe meds for pain.

Employee was a supervisor who was working with the plant electricians while replacing a transformer that had failed. The employee was attempting to remove insulation from the 750 CFM cable when his knife slipped and resulted in a laceration to the back of his left thumb.

Driver was speeding which lead to roll over accident of articulating dump truck.
Employee was utilizing a cable come a long to move a shaft on the man sand tail pulley. The cable on the come a long broke as the employee was removing the race to change the bearing. This caused the employee's left hand to be caught between the shaft and conveyor frame. Employee received a crushing injury to his middle, ring and little finger on his left hand.

Employee was tied off inside the primary crusher attempting to unplug the crusher. He was moving a 25 to 30 lb rock when his left elbow go t caught in his lanyard causing the weight to the rock to shift to his left hand. When the rock shifted, he lost control of it and smashed his left hand in between two rocks.
The employee was pulling electrical wire and felt a pain in his right shoulder.
Employee was stepping off a forklift when he stepped onto a small rock and twisted and sprained his left knee.

EE was climing into 773A Haul truck when he attempted to open the door and lost his hand hold. He fell backwards off the catwalk approximately 8 feet to the ground and landed on his left wrist.
Employee was turning around on level ground to walk to the restroom and felt a pain in his right knee.
EE was using an air chisel to hammer out a metal pin on the front of a haul truck when a piece of metal fell off and stuck in the tip of his nose causing a laceration.
Employee was lifting motor frame with a ratchet cable puller when the hook slipped off the motor frame and the motor frame struck the employee on the left knee.

EE PULLED HARD ON A GREASE HOSE THAT HAD BECOME LODGED AT THE 8 X 20 SCREENING DECK AND HE FELT A SHARP PAIN IN LOWER STOMACH AND GROIN AREA.

HE SAID WHEN HE WENT IN FOR HIS FINAL CHECK THE DOCTOR RELATED TO HIM THAT HE HAD ACUTE SILICOSIS. WE HAD HIM SIGN A MEDICAL RELEASE & SENT FOR HIS RECORDS.
