Miner was walking from SS to SCD, slipped and fell striking EE's head on the ground. The miner didn't lose consciousness and EE got up and went to SCD where EE reported the fall. Miner initially refused medical treatment then decided to go to the ER for evaluation. Physician removed miner from duty for 1 shift as part of a mild concussion treatment.

A technician was performing EE's lube routes Sunday, 17-Dec-23 at approximately 2 p.m.. EE was carrying a bucket with EE's lube gun. While walking on the transition point, EE slipped and fell to EE's knees. EE attempted to break the fall with the arm carrying the bucket. EE felt a "pop" in EE's right shoulder.

On Wednesday October 4th, 2023. A swing shift employee of 2 years and 3 months in the company suffered a back sprain while descending from the G-loader at the beginning of their break at 22:30. As the employee stepped down the ladder, their left foot slipped and went through the last and 2nd to last rungs making them loose grip of their left hand from the handrail.

Employee was walking down stairs. Heel of boot got stuck on grate and employee tripped and fell down 6-7 stairs.

A Scalehouse employee reported on EE's shift at 6:45am. The employee decided to take a break outside the scalehouse area, and leaned up against a metal chain connected to a trailer building and scalehouse structure. As EE leaned on the chain, the wood trailer structure gave way, causing the employee to fall on EE's back with the chain wrapped around EE's lower left leg and ankle.

Truck driver having truck positioned onto above ground scale to load product into tank trailer. As driver exited truck cab and stepping onto scale railing (flat surface) EE held a 10 foot aluminum pole to position spout, lost footing and fell onto railing. Result was fractured rib. Doctor has reduced EE's duties to modified duty at desk after one day off.

Operator was instructed to clean out G-15 dust collector with the vacuum truck. EE was returning from evaluating the job and ee began descending down a steep set of stairs. The operator's left foot came down on a step 3-4 inches below what ee expected. The jarring impact fractured the employees heel.

Employee slipped and fell on oil residue. As EE was falling EE's right arm hit a metal guard resulting in a laceration. EE was taken to the local hospital where EE was treated and released without limitations.

Miner parked his forklift and dismounted onto an uneven surface and rolled his ankle spraining it. He stepped off the forklift facing away from it instead of facing it using three points of contact as he was trained to do.

The Miner went up the stairway to Mill #6 to find the electrician working up there. After finding the electrician, the Miner and the electrician proceeded down the stairway. At a landing the Miner turned right to continue going down. While turning his right boot caught on the grated landing and the Miner twisted his knee.

While connecting a product off-load hose to a rail car filled with lime, the employee slipped on the snow that was in the area. When he stood up, he felt a pain in the lower back.

Employee was walking and turned his ankle

While walking on snow covered ice from the Quarry Lunchroom the employee slipped and fell backwards injuring his left shoulder and right hip. No medical treatment initially. On 01/16/2014 he was evaluated at local medical facility and released for full duty with a referral to physical therapy (P.T.) Incident became recordable on 01/24/2014 when he began P.T.

A worker had climbed on top of CBE14B in order to inspect the back side of the middle feed chute. As he was descending, his foot caught on the safety pull cord. This caused him to lose his balance and he jumped to the floor from a height of 33". He wound up on his feet on the tunnel floor but the jar resulted in a muscle spasm in his upper back.

The employee was walking from one building to the next, when he slipped on the ice and injured his left wrist.

Employee was walking up an inclined walkway when he felt a pop in his right calf.

Employee slipped on slush covered surface and fell striking his knee on the ground.

Employee was climbing down ladder of tanker truck trailer and at bottom/ground stepped on uneven ground and twisted ankle.

Employee was greasing secondary crusher and dryer(SCD) L-150 D Loader and slipped on snow/ice and landed on right shoulder. Employee decided not to go to see a doctor until 2/17/2007. Reportable due to restricted work duty days starting 2/17/2007.

While charging (carrying bags of briquettes) furnace #10, the employee went to place a bag on the chute, he planted his left foot & then felt a sharp pain in his left knee. Injury under investigation.

Employee was walking to work area on driveway (asphalt paved) and slipped on block ice.

Employee was climbing down the ladder of QT-3 haul truck. His foot missed the top step of the ladder. His left foot then was caught under the step second from the top. He fell backwards and was hanging upside down from the ladder. A nearby employee was there to lift him up almost immediately. He injured his lower left leg and was placed on restricted work duty for one day.

Standing at the quarry edge next to the crusher watching blast in quarry with two contractors and the quarry supervisor, blast went bad. We ran, I fell trying to avoid flying rocks and landed in a truck tire rut. Injured back, legs, right arm, shoulder, knee, right eye and left abdominal pain.

EE WAS GETTING OUT OF LOADER AND SLIPPED ON THE BOTTOM RUNG OF THE LADDER. EE LEFT FOOT SLIPPED AND BANGED LOWER LEFT LEG ON THE RUNG

EE WAS TRYING TO PULL A CONDENSER OUT OF RETORT #3 IN CALCIUM FURNACE #8. HE GRABBED ONTO THE PIN OF THE CONDENSER WITH HIS HANDS AND WHEN HE PULLED THE CONDENSER IT CAME OUT EASIER THAN EXPECTED AND HE FELL BACKWARDS, LANDING ON HIS LEFT WRIST. HE WAS BROUGHT TO THE HOSPITAL AND DIAGNOSED WITH A FRACTURED LEFT WRIST.

After filling the seal water vessel under tank 112 employee stepped backwards down an incline and strained his right knee. He was seen at a local clinic and was placed on restricted duty.

Employee was getting off of forklift when he felt pain in foot.

Employee was cleaning floor with a water hose and slipped and fell. His left knee came in contact with a cement floor causing a contusion. He was seen at a local medical clinic and placed on restricted duty.

Employee was climbing a fiberglass step ladder to install a new water line and the rear support leg of the step ladder split. Employee fell onto existing piping and injured his ribs. He was lowered to the ground by the employee he was working with. He was seen at a local clinic and is on restricted work.

Employee stepped on uneven floor surface, causing ankle sprain.

EE WAS REPLACING GUARD ON TAIL PULLEY OF B BELT IN SCREEN STONE BUILDING. WHEN HE WAS ON STEP LADDER HE SLIPPED OFF AND THE GUARD HIT HIS RIGHT ANKLE.

EE STARTED CLIMBING DOWN THE LADDER AND LOST HIS FOOTING. HE TRIED TO GRASP THE ROPE THAT IS TIED TO THE TIE OFF AND FELL APPROXIMATELY 7-8' TO THE GROUND. HIS RIGHT FOOT IS SWOLLEN.

EE WAS WALKING FROM QUARRY CRUSHER TO BREAKROOM AND SLIPPED ON SNOW COVERED ICE AND FELL ON RIGHT SHOULDER.

EE STEPPED FROM TOP RUNG OF LADDER (5'4"HIGH) TO20"X 24"PLATFORM TO GET SAMPLE FROM A FINES FEEDER. EE TRIPPED ON EMERGENCY STOP PULL CORD THAT IS LOCATED 4"ABOVE PLATFORM. EE FELL, CATCHING HIS LT. HAND ON A HANDRAIL IN THE IMMEDIATE AREAEE TREATED BY MD FOR SPRAIN/STRAIN OF LT. THUMB.EE RELEASED TO MODIFIED DUTY. NO PUSHING,PULLINGLIFTING WITH HIS LT. HAND.

WHILE LOADING LAST COMPARTMENT OF A BULK TRUCK, EE LOST BALANCE WHEN HE STEPPED FROM A PLATFORM ONTO THE TRUCK. HE TWISTED HIS RIGHT ANKLE, FELL ONTO THE TRUCK AND BRUISED HIS RIGHT ELBOW. RAI LINGS ARE IN PLACE AT THIS LOADOUT STATION. EE IS CURRENTLY ON RESTRICTED DUTY FOR A SPRAINED ANKLE.

WHILE EE WAS CARRYING 6 FOOT STEP LADDER TRIPPED ON PLASTIC ON FLOOR AND LANDED ON RIGHT KNEE. OCCURED IN FURANCE BLD. EAST PUMP ROOM.

EMPLOYEE WALKED THROUGH A PRODUCT SPILL THAT LED TO THE ENTRY TO A CONVEYER BELT WHICH CONCEALED A LARGE STONE. HE STEPPED ON THE STONE AND TWISTED HIS RIGHT ANKLE AS HIS FOOT ROLLED OFF THE S TONE. EE HAD HIS RIGHT KNEE ON THE GROUND CAUSING A CONTUSION OF THE KNEE. EMPLOYEE WAS TREATED BY A MD AND RELEASED TO RESTRICTED DUTY.

EMPLOYEE FELL WHILE BOARDING FORKLIFT. EE HAD RIGHT FOOT IN STEP AND LEFT FOOT ON PLATFORM AND LOST FOOTING OF RIGHT FOOT FALLING BACKWARDS ONTOLEFT BUTTOCKS. EE COMPLAINED OF PAIN IN LEFT SHO ULDER BLADE AND LEFT BUTTOCK. EE WAS TREATED BY MD AND RELEASED TO RESTRICTED DUTY. LIMITED USE OF LEFT ARM.

EMPLOYEE WAS STEPPING OFF FORK TRUCK AND STEPPEDON UNEVEN METAL FLOORING AND TWISTED ANKLE.

EE WAS CARRYING A 5 GAL BUCKET UP A FLIGHT OF STAIRS WHEN HE SLIPPED AND A STEP. EE DID NOT FALL BUT CAUGHT HIMSELF. EE FELT PAIN IN HIS LEFT BUTTOCKS. EE WAS SENT TO MD WHERE HE WAS TREATED, GIVEN PAIN MEDICATION AND PLATED ON RESTRICTED DUTY NO PUSHING, PULLING, LIFTING NO PROLONGED STANDING, NO OPERATION OF EQUIPMENT.

EE FINISHED LOADING TANK, STEPPED ONTO PLATFORM ON TOP OF TANK LADDER. TURNED AROUND, USED LEFT ARM TO REACH FRO HANDLE ON RIGHT BUT HANDLE WASN'T THERE. LOST BALANCE AND GRABBED HANDLE ONLEFT , TWISTED LEFT SHOULDER IN THE PROCESS.

EE WAS WORKING ALONE AND IS NOW HOSPITALIZED AND HAS NOT BEEN SPOKEN TO ABOUT THIS INCIDENT. IT IS SPECULATED THAT HE WAS WORK OFF OF A 10 FT EXTENSION LADDER AND FELL APPROX SEVEN FEET AND HI T THE RIGHT BACKSIDE A HIS HEAD. HE WAS TAKEN BYAMBULACNE TO BERKSHIRE MEDICAL CENTER AND WAS SEDATED FOR OBSERVATION. AT SCAN TAKEN WAS NORMAL.

EE WAS WORKING ON TURN HEAD IN MILL BUILDING WHEN LARGE PLUME OF DUST ROSE UPWARD THRU BUILDING CAUSING POOR VISIBILITY PLUME WAS CAUSED BY HIGH WINDS OUTSIDE BUILDING EE THOUGHT PLUME WAS CAU SED BY AN EARTHQUAKE AND BECAME VERY EXCITED. EETRIED TO VACATE AREA IN A HURRY WHEN HE SLIPPED AND FELL.

EE WAS FIXING A LIGHT AND STEPPED OFF 2 INCH HIGH LANDING AND TWISTED RIGHT ANKLE.

EE WAS WALKING TO AREA INSIDE TUNNEL TO PERFORM CLEAN-UP OF THE ROCK PILES WHEN HE SLIPPED ON A ROCK, FELL, HITTING HIS RIGHT SHOULDER.

EE PICKING UP TOOLS ON BACK TAILGATE OF PICKUP AND WHILE STANDING ON TAILGATE SLIPPED ON ICE/SNOW.

EE TRIPPED AND FELL INTO SMALL TABLE IN RAYMOND MILL BAGGING AREA. STRUCK SHOULDER ON TABLE. 1ST DAY OF LOST/RESTRICTED DUTY WAS 10-11-00.

EE WORKING ON AN AUTOMATIC STRETACH WRAP MACHINE STEPPED OFF OF AN 18" LANDING ONTO A RAMP GOING INTO THE WAREHOUSE. WHEN HIS FOOT HIT THE RAMP HE ROLLED HIS LEFT ANKLE AND FELL ONTO THE RAMP. HE SPRAINED HIS LEFT ANKLE, X RAYS WERE NEGATIVE-HE WAS PALCED ON RESTRICTED DUTY.

EE WALKING OVER TO TRUCK AND LEFT FOOT SLIPPED ON MUD AND ANKLE BUCKLED UNDER.
