Miner was walking from SS to SCD, slipped and fell striking EE's head on the ground. The miner didn't lose consciousness and EE got up and went to SCD where EE reported the fall. Miner initially refused medical treatment then decided to go to the ER for evaluation. Physician removed miner from duty for 1 shift as part of a mild concussion treatment.

Showing 50 of 53A technician was performing EE's lube routes Sunday, 17-Dec-23 at approximately 2 p.m.. EE was carrying a bucket with EE's lube gun. While walking on the transition point, EE slipped and fell to EE's knees. EE attempted to break the fall with the arm carrying the bucket. EE felt a "pop" in EE's right shoulder.
On Wednesday October 4th, 2023. A swing shift employee of 2 years and 3 months in the company suffered a back sprain while descending from the G-loader at the beginning of their break at 22:30. As the employee stepped down the ladder, their left foot slipped and went through the last and 2nd to last rungs making them loose grip of their left hand from the handrail.

Employee was walking down stairs. Heel of boot got stuck on grate and employee tripped and fell down 6-7 stairs.
A Scalehouse employee reported on EE's shift at 6:45am. The employee decided to take a break outside the scalehouse area, and leaned up against a metal chain connected to a trailer building and scalehouse structure. As EE leaned on the chain, the wood trailer structure gave way, causing the employee to fall on EE's back with the chain wrapped around EE's lower left leg and ankle.
Truck driver having truck positioned onto above ground scale to load product into tank trailer. As driver exited truck cab and stepping onto scale railing (flat surface) EE held a 10 foot aluminum pole to position spout, lost footing and fell onto railing. Result was fractured rib. Doctor has reduced EE's duties to modified duty at desk after one day off.
Operator was instructed to clean out G-15 dust collector with the vacuum truck. EE was returning from evaluating the job and ee began descending down a steep set of stairs. The operator's left foot came down on a step 3-4 inches below what ee expected. The jarring impact fractured the employees heel.
Employee slipped and fell on oil residue. As EE was falling EE's right arm hit a metal guard resulting in a laceration. EE was taken to the local hospital where EE was treated and released without limitations.
Miner parked his forklift and dismounted onto an uneven surface and rolled his ankle spraining it. He stepped off the forklift facing away from it instead of facing it using three points of contact as he was trained to do.
The Miner went up the stairway to Mill #6 to find the electrician working up there. After finding the electrician, the Miner and the electrician proceeded down the stairway. At a landing the Miner turned right to continue going down. While turning his right boot caught on the grated landing and the Miner twisted his knee.
While connecting a product off-load hose to a rail car filled with lime, the employee slipped on the snow that was in the area. When he stood up, he felt a pain in the lower back.

While walking on snow covered ice from the Quarry Lunchroom the employee slipped and fell backwards injuring his left shoulder and right hip. No medical treatment initially. On 01/16/2014 he was evaluated at local medical facility and released for full duty with a referral to physical therapy (P.T.) Incident became recordable on 01/24/2014 when he began P.T.
A worker had climbed on top of CBE14B in order to inspect the back side of the middle feed chute. As he was descending, his foot caught on the safety pull cord. This caused him to lose his balance and he jumped to the floor from a height of 33". He wound up on his feet on the tunnel floor but the jar resulted in a muscle spasm in his upper back.

Standing at the quarry edge next to the crusher watching blast in quarry with two contractors and the quarry supervisor, blast went bad. We ran, I fell trying to avoid flying rocks and landed in a truck tire rut. Injured back, legs, right arm, shoulder, knee, right eye and left abdominal pain.

HE WAS BROUGHT TO THE HOSPITAL AND DIAGNOSED WITH A FRACTURED LEFT WRIST.
After filling the seal water vessel under tank 112 employee stepped backwards down an incline and strained his right knee. He was seen at a local clinic and was placed on restricted duty.

EE FINISHED LOADING TANK, STEPPED ONTO PLATFORM ON TOP OF TANK LADDER. TURNED AROUND, USED LEFT ARM TO REACH FRO HANDLE ON RIGHT BUT HANDLE WASN'T THERE. LOST BALANCE AND GRABBED HANDLE ONLEFT , TWISTED LEFT SHOULDER IN THE PROCESS.
EE WAS WORKING ALONE AND IS NOW HOSPITALIZED AND HAS NOT BEEN SPOKEN TO ABOUT THIS INCIDENT. IT IS SPECULATED THAT HE WAS WORK OFF OF A 10 FT EXTENSION LADDER AND FELL APPROX SEVEN FEET AND HI T THE RIGHT BACKSIDE A HIS HEAD. HE WAS TAKEN BYAMBULACNE TO BERKSHIRE MEDICAL CENTER AND WAS SEDATED FOR OBSERVATION. AT SCAN TAKEN WAS NORMAL.
EE WAS WORKING ON TURN HEAD IN MILL BUILDING WHEN LARGE PLUME OF DUST ROSE UPWARD THRU BUILDING CAUSING POOR VISIBILITY PLUME WAS CAUSED BY HIGH WINDS OUTSIDE BUILDING EE THOUGHT PLUME WAS CAU SED BY AN EARTHQUAKE AND BECAME VERY EXCITED. EETRIED TO VACATE AREA IN A HURRY WHEN HE SLIPPED AND FELL.
