Employee was moving sandbags by SB-6 when he slipped and fell causing his left leg to bend underneath his body. NOTE: On Sept. 16, 2009 employee felt discomfort in his knee, went to physician and was removed from duty.

Employee was walking along on the Mezzanine Level above the offices when he tripped on floor grating and fell. He attempted to break his fall with his arm, and subsequently broke his right arm.
