Employee was decending the ladder on a cat 994 loader when he slipped. Employee grabbed the hand rail pulling shoulder.

Truck driver was stepping across from the left side of the cab deck to the right side of the cab deck to perform a fluid check for his pre-shift inspection, when he missed a step with his left foot causing him to fall to the ground.

Employee got out of the Caterpillar D9N Dozer to check on something and slipped outside the cab causing left leg to go between engine compartment and tracks hurting left knee.

Employee was stepping over a pipe at the pump station to check tank levels. He lifted his left leg to step over the pipe, putting all his weight on the right leg. he twisted the right leg at the knee.

Removing trash from Ball Mill area, started walking backwards to allow truck to go by and stepped on dome grind media losing balance and falling, hitting tailbone on media and a wood plank.

Walking through #6 Ball Mill South, uneven grating, tripped and fell forward and was caught by another employee who broke the fall. Employee did not hit the ground or grating. Reported to work on Monday 8/6/07 and complained with pain in back and shoulders. Taken to Clinic and given prescription meds and released to return to work.

Employee was operating the 910 forklift. He stepped off and caught the toe of his left boot in the expansion joint and rolled his foot. He felt something pop. He checked his foot and notified co-worker. 3 fractured bones on top of foot.

Employee was working on roller frame, safety harness caught, causing employee to fall and caused knee to roll.

THE EMPLOYEE HAD GONE TO HIS TRUCK TO GET CHANGE FOR A POP MACHINE. AS HE WAS COMING DOWN THE STAIRS, HE SLIPPED OR LOST HIS BALANCE AND FELL (LACERATION TO HEAD).

MAN WORKING AT BOTTOM OF #2 MOTOR PIT, STANDING ON INSULATING PLATE. MAN WAS PULLING UP ON 1 1/2" DIAMETER CABLE. THE COMBINATION OF THE MAN'S WEIGHT AND THE FORCE OF PULLING CAUSED PLATE TO B REAK. MAN BROKE THROUGH PLATE, FELL STRAIGHT DOWN AND SCRAPED SHIN ON INSULATING PLATE.

EE WAS CUTTING TIMBER ON SAW WHEN HE TRIPPED/ SLIPPED AND FELL STRIKING HIS LEFT KNEE ON THE CONCRETE FLOOR.

AFTER CHANGING THE AIR FILTER ON THE LUBE TRUCK EMPLOYEE ATTEMPTED TO STEP DOWN USING A STEP LADDER. HE LOST HIS BALANCE AND GRABBED THE EDGE OF THE HOOD TO KEEP FROM FALLING. HE RCVD. A LACER ATION TO HIS RIGHT RING FINGER THAT REQUIRED 5 STICHES. HE WAS TREATED AND RELEASED BACK TO WORK THAT DAY.

EE TWISTED HIS RIGHT KNEE AND FELL TO THE GROUND AS HE CLIMBED OFF THE SIDE OF A STINGER TRUCK. HE FELLS HE MAY HAVE STEPPED ON A ROCK. EE WAS TAKEN TO THE ER IMMEDIATELY. THE ER DOCTOR RELEAS ED THE EE TO WORK WITH A RESTRICTION OF LIMITED LIFTING, WALKING AND NO RUNNING TO RETURN TO RETURN TO FULL DUTY ON 2-19-01. THE EE WORKED 2/14-16, WAS SCHEDULED OFF 2/17-18 & RETURNED 2/19/0

EMPLOYEE CLOSING GATE ON CATTLE GUARD. EE'S FOOT SLID BETWEEN GUARD RAILS DOWN TO KNEE FULL WEIGHT TWISTED LEFT KNEE.

AS EMPLOYEE TRIED TO MOVE A 12' PIECE OF PIPE HIS RIGHT FOOT SLIPPED IN THE MUD. HE FELT PAIN IN HIS KNEE BUT DID NOT FEEL THAT IT WAS SEVERE ENOUGH FOR TREATMENT. ON 8/9/00 HE REQUESTED TO SEE A DOCTOR AS THE PAIN HAD NOT SUBSIDED. THE DOCTOR EXAMINED HIM AND RELEASED HIS TO WORK WITH RESTRICTIONS. HE WAS EXAMINED AGAIN 8/16/00 & RELEASED.
