Our driver was attempting to unhook hoses from the tractor to the trailer so they could disconnect the tractor from the trailer. Their foot got tangled with the step on the back deck of truck. Although they were holding on to the safety bar, they slipped and fell. They bruised their left side.

Showing all 50Employee was replacing a take up on a conveyor belt when, while doing so EE slipped resulting in a loss of balance and EE's arm impacted a guard adjacent to the belt. Went to doctor and was released to full duty that day. Followed up with doctor and fluid containing blood was drained from hematoma. Received doctors' notes on 12/4/23.
Employee was getting on the bus to ride underground and tripped on the first step. When EE caught self, EE hit EE's finger on the step.
Employee was steam cleaning haul truck (H-14) at the steam jenny. Climbing down off of truck and foot slipped. Was holding wand in one hand. Fell from 2nd rung from the top landing on the ground hitting the left side of EE's body. ER visit confirmed nothing broken. Put on work restriction due to soreness and limited motion.
Employee working in 24E @ 37S, tripped over rock and hit shin on another rock. Employee worked next two days without issue. On Monday 5/14 employee asked for doc. apt to have evaluated. Injury evaluated 5/15, diagnosed with severe contusion employee put on limited duty 5/16 to 5/18.
The driver was pulling a load from the mill location in the afternoon of 10/19/17. EE had an incident with the truck and trailer causing the load to shift. While trying to remove the straps from the load, EE fell off the trailer. EE broke wrist and toe. EE received stitches near eye, as well.
Employee on bed of pick up truck picking up empty paint cans. When employee attempted to exit bed of truck, EE elected to step off on the tire side, as opposed to back. The back had a step provided by bumper. As EE exited the side, right foot on tire, left foot caught the wooden bed rail, as EE unhooked the foot, EE pulled it back, landed on left foot awkwardly, twisting knee.
Employee over stepped the first step and fell down four steps.
It was raining. The employee slipped on wet lime. This caused him to fall to the ground. He was checked out at the hospital. He has an inflammation in his back.

EE WAS WORKING WITH ANOTHER ON THE HOOD TO #5 KILN WHEN HE TRIPPED ON SOMETHING AND FELL FROM THE DECK OF THE STEPS, ABOUT 3 FEET, ONTO HIS RIGHT SHOULDER.
THE EMPLOYEE WAS RUNNING A RUBBER AIR HOSE FROM THE PUMP HOUSE TO THE #24 TIRE OF THE #5 KILN, USING THE CONDUIT FOR SUPPORT. WHEN HE ATTEMPTED TO TOSS THE HOSE TO THE PLATFORM AT THE TIRE HIS FOOT SLIPPED ODD THE CONDUIT AND HIS FOOT LANDED ON ROCKS ALONG SIDE THE CONDUIT.
EE WAS EXITING THE CAT D300D HE MISSED THE SWINGING STEP AND FELL TO THE GROUND.

THE EMPLOYEE FELL 11' ONTO AN UNCLINE OF H ARD MUD AND ROLLED OR SLID ANOTHER 9' TO EXIT. THE BIN AND THEN FELL ANOTHER 3' TO THE GROUND LEVEL AND LANDED ON TOP OF SCREENINGS.
THE EMPLOYEE TWISTED HIS KNEE WHEN HE WAS GETTING OUT OF THE BOLTER.
THE EE WAS DOING HIS PRESHIFT INSPECTION WHEN HE SLIPPED ONT HE LADDER CAUSING HIS BATTERY PACK TO JAM INTO HIS RIGHT RIBS AND BACK.
THE EE WAS ON A MINE RESCUE TRAINING SITE, AT ONAN UNDERGROUND COAL MINE. THE BATTERY ON HIS HEAD LAMP WENT DEAD AND HE TRIPPED AND FELL.
THE EE WAS CLIMBING OUT OF THE SKID STEER WHEN THE HEEL OF HIS BOOT CAUGHT THE TOP OF THE BUCKET CAUSING HIM TO FALL. HE LANDED ON HIS RIGHT KNEE, BOTH HANDS AND THE RIGHT SIDE OF HIS FOREHEAD . EE DID NOT SEEK MEDICAL ATTENTION UNTIL 5/1/00.
