Showing all 40EE was rebuilding slurry pump at booster EE took rubber guard off of hub seal on pump and laid it on ground next to work area as EE continued to perform task EE tripped on rubber guard and fell on stump and injured lower right side of back and cracked a rib.
Miner was walking from a face drill to a man-lift, distance of about 150ft, when EE lost footing causing discomfort and some pain in the left knee. The miner reported the walkway/work area was illuminated (cap lamp, drill lights & man-lift lights) and was free of slip & trip hazardous.
On 3/15/17 the employee slipped and fell. EE was treated and released to full duty at that time. As days/weeks/months went by EE began having pain in right shoulder. A medical exam in June 2017 revealed a shoulder injury likely caused by the fall and requiring surgery. Surgery occurred July 5th.
Climbing down from the Anfo Loader, lost footing, fell backwards, hurt left wrist.

Crane operator (74 yrs old) fell 5+/-ft from a crane access step to the steel deck below. He was fueling and checking fluids on the Mantiwoc 4600 crane when he fell. He received fractured ribs, fractured collar bone, head & face trauma, temporarily unconscious and bruised lung. Was transported to local area hospital by ambulance.
A Contractor employee was working on top of a silo performing of an abandoned plant. The injured worker fell off the 64' silo to the ground, as a result the injured worker suffered a broken neck
The EE was standing on a 41" high conveyor frame using a torch to remove the walkway and handrails. The frame was positioned on the ground. While the EE was repositioning himself his foot slipped off the structure and went under on of the cross members. EE fell backwards, was upside down by 1 leg. EE sprained ankle and severe contusion to lower leg/sprained hip.
Worker received a sprained right ankle as he was stepping down onto a catwalk at the 5x16 screen. The catwalk was clean & free of debris.
While walking, the injured EE tripped on a small pile of sand, lost his balance and fell to his hands and knees. The injured EE sustained a strained right shoulder and returned to work with restrictions and prescription medication.

Worker fell apx 3 to 4 ft from catwalk of mechanical scaler while performing pre-shift exam on the mobile equipment. He stretched up to release a door to engine compartment when he lost balance and fell. He landed back-side first on mine floor against battery pack of his cap lamp.
EE was climbing down from the grizzly belt to the screen below. He then stepped from the screen to the handrai. As he was climbing down his foot slipped and his shin hit the I-Beam.
EE was walking and did not notice he was standing on ice, the ice was covered with sand. The ice broke and he went down to his shins and jarred his back.

EMPLOYEE WAS CLEARING MUD OFF OF TRUCK WINDSHIELD; STEPPED DOWN ONTO ROCK AND TWISTED HIS FOOT.
