Showing all 8Employee was on top of a pneumatic trailer, working with lid number 2, when he fell off the trailer and landed on the ground. He sustained 3 broken ribs (left rear) and a broken left thumb.
Employee slipped and fell on muddy surface carrying hydraulic pump.

Employee was operarting the skid steer cleaning out the loading floor. Quicklime on the floor was damp, steam vapors surrounded the skid steer. Operator then tried to get out of the loader and slipped. Using his left hand to break the fall, the employee's hand stuck in the quicklime.
WHILE WALKING DOWN THE HILL FROM CLOCKING IN, EMPLOYEE SAID HE WAS WALKING AND STEPPED IN A DITCH IN THE ROAD AND TWISTED HIS RIGHT ANKLE.
EMPLOYEE SETTING 4" STEEL PIPE ON AN EMBANKMENT, SLIPPED IN THE MUD, CAUSING THE PIPE TO SWING BACK AND HIT HIM IN THE FACE.
