The employee was closing a sliding door at the loading dock. The removable guard rail was not in place, leaving an open dock approximately 4' above ground level. As the employee pulled on the door to close it, it began to move causing him to loose his balance and fall to ground level. He landed on his feet twisting his right knee.

Routine maintenance and leaned on handrail, handrail gave way due to broken welds. Fell 6'6" to the concrete floor. Due to fall he fractured spine, broke sternum, lacerated his head.

THE EMPLOYEE WAS STACKING BAGS NEAR THE #9 BAGGER WHEN HE SLIPPED ON THE WET FLOOR. HE HIT HIS RIGHT KNEE, RIGHT ELBOW, AND HIS HEAD ABOVE THE LEFT EYE WHEN HE FELL DOWN.
