While waiting on water removal process to be completed, EE picked up an air hose and began walking backwards to remove slack from the hose and stepped into the manhole causing employee to partially fall into opening striking lower back

While the employee was climbing down a fixed ladder EE slipped on the 3rd rung from the bottom and landed awkwardly on EE's left ankle.
Employee was working on housekeeping around the plant. While working with the vac-truck operator employee was giving hand signals to the driver during the backing up process. While doing this their feet slipped and they began to fall. during this process their right leg became entangled underneath them and they broke their leg.
Employee was climbing into the cab of the bulldozer EE operates when EE's foot slipped off the step and EE fell injuring EE's ankle in the process.
EE was walking from EE's office to the front break room for the morning meeting. EE heard some voices from the direction of the break room. EE turned EE's head to see who was speaking. At this time, EE's foot went into a depression in the walkway, causing EE's knee to twist awkwardly. EE felt something pop in EE's knee and immediately began to feel pain
EE was assigned to build a platform and hopper. EE was wearing a harness but did not tie off and fell through the unfinished platform. The fall was approximately 13 feet. Injuries include a cut about the eye, wrist fracture and teeth knocked loose, and a cut on lip.
While walking they slipped on ice and bumped a knee.
EE was descending inside staircase to exit building. EE missed a step, slipped and fell down approx 5 remaining steps landing at the bottom of the staircase resulting in a fracture of the left ankle

EMPLOYEE WAS CARRYING DOWN A RIVET GUN AND A CHIPPING GUN FROM SECOND FLOOR, THE AIR HOSE SLIPPED OUT OF EE'S HANDS AND EE TRIPPED OVER THE HOSE CAUSING EE TO STUMBLE. NOW EE'S KNEE IS SWOLLEN AND TIGHT LIKE EEE STRAINED IT.
The employee was guiding a 400-500# sprocket to the landing area to be picked up by a crane. While rolling the sprocket along the catwalk it began to tip over and the contractor that was helping roll the sprocket said it was tipping and EE couldn't hold it. The employee was trying to get out of the way and the sprocket caught EE's shirt pulling EE to the floor under the sprocket.
While stepping off the switcher to disengage the rail car the employee slipped on some ice/snow buildup on the rail and to the side of the rail.
Employee states: while walking along a concrete walkway leading to W3801 tunnel EE slipped on a patch of ice. EE didn't fall to the ground. EE caught self with EE's left hand holding onto the handrail hurting EE's left elbow. Doctor diagnosed EE with an elbow strain. This became reportable on 6/16/22 when professional medical treatment was needed.
Individual was climbing a fixed ladder and lost EE's grip or footing and fell approximately 6 feet to the ground below fracturing the humerus of EE's right arm and dislocating EE's right shoulder. Employee is on restricted duty.
EE was walking from the lab back to the control room. EE said: turned the corner from the lab, slipped and fell on right side. EE felt some pain and soreness in upper right arm/shoulder area. The floor was dry and nothing was on the floor.
Employee fell through sheet metal roof and was suspended above an operational kiln. Injuries were due to the extreme heat, not from the fall. Burns were primarily to the buttocks and backs of legs.

Individual was climbing ladder and lost EE's grip. EE jumped back landing on EE's feet. In doing so EE sprained EE's left knee and right ankle.
An employee fell while descending a ladder. Employee misjudged the length of the last step to the ground and lost balance. Employee fell backwards into a pump motor and fractured their tailbone.
Employee slipped on wet pavement while hurrying through the rain. Employee stated knee popped as employee caught self from falling. Worked through the pain for a couple of months then knee popped again while carrying conduit. Required meniscus surgery on 12/3/2020.
Employee was stepping from an even platform onto a railcar and felt knee "give away"
Employee's foot slipped from the bottom rung of a ladder and EE fell against the rail of the ladder bruising EE's ribs, shoulder and neck. Employee continued to complain of pain. On 4-28-20 a doctor placed EE on a 28 day lifting restriction of 25 pounds. EE will also have therapy sessions for the next 4 weeks.
EE was exiting building,stepped down from concrete floor to ground and rolled ankle. treated and released on 09/09/19 for ankle sprain no restrictions. F/U apt. shows EE to have a bone avulsion to left foot, received restrictions on 09/18/19.

Employee had gone to 3801 belt tunnel to winterize heaters. Employee was servicing south heater from an eight foot ladder and somehow fell from ladder. The injured employee was discovered unconscious by electrical maintenance worker that was looking for injured employee.
The employee alleged that while walking down steps employee's left foot slipped behind step into the opening between the step causing pain in left knee.
Employee lost balance while descending stairway.
Employee was walking backwards dragging a spray pump. Employee tripped over a drain line attached to water tank and fell backwards dropping spray pump on lower right leg resulting in a fracture of the fibula.
