As employee was getting off ladder of hopper car, set foot down on the edge of concrete walkway twisting left ankle
The employee's pants leg caught a piece if grating causing them to fall. The employee denied medical attention until 2/5/2018 at which time they were placed on restricted duty.
Moving some bundles around maintenance shop, miner stumbled over own feet. In catching balance, left thigh was strained.
Employees was climbing steps to access top floor of milling area, while making a turn on second level platform EE's right knee buckled. EE stated the right knee was bothering EE on 5/21 and was seen by a doctor on 5/22. Doctor put EE on light duty and a follow up appointment was scheduled.
Employee was getting off the dozer and lost their balance and fell backwards. When employee tried to catch self with their hands they fractured a bone in their left wrist.
Shredder hopper was sitting on floor. Employee was preparing to hook chains to the hopper and loader. When the employee put EE's weight on the hopper, it moved causing the employee to hit loader bucket with EE's back. The rim of the hopper struck the employee in the stomach and rib area. The employee's back and ribs were bruised.
Employee was walking to clean out equipment and made a sudden turn. Employee heard a pop and employee's left knee causing employee severe pain.
Driver was trying to hurry to get back to truck, slipped on wet clay and struck knee on the ground.
Employee was exiting dozer had a lunch bag in hand. He started to step down from dozer and his foot slipped on the tracks. He fell to the ground.
Employee states that while moving a pallet of stacked poly bags to the stretch wrapper the bags fell off the pallet. Employee stopped the forklift and proceeded to reload the pallet with the poly bags. Employee states he had already lifted 10 bags and was on the 11th bag when he lost his footing. While regaining his footing employee felt a pain in lower back.
Employee stepped backwards and stepped into a 12 inch discharge pipe opening, causing a 3 1/2 in. laceration to upper left leg.
Employee was mounting front end loader when he lost grip with his right hand and fell to the ground
Employee was cleaning up and washing area around silos with a water hose. After cleaning up, he was rolling up the hose when he slipped and fell and broke his ankle.
Was walking out of rotary dryer room when the operator hollered at employee to come back and restart the dryer. When he turned to go back into the rotary dryer building, he twisted his left knee. Surgery done on 3-7-12.
EMPLOYEE GOT OUT OF VEHICLE AND SLIPPED ON ICE, TWISTING HIS LEFT ANKLE, CAUSING A HAIRLINE FRACTURE.
Employee was going down 5 steps behind the bagger, tripped and fell to the floor, broke his left leg.
MINER ON ROOF WEARING SAFETY HARNESS BUT NOT TIED OFF TO EXISTING CABLE WHERE EXISTING RETRACTABLE WAS AVAILABLE. MINER WAS STEPPING BACKWARD WHILE PULLING HOSE IN PREPARATION FOR COATING ROOF WHEN HE STEPPED ONTO & THROUGH SKYLIGHT SHEETING. MINER FELL APPROX 17 FT ONTO CONCRETE FLOOR BELOW & SUFFERED BROKEN FEMUR.
Employee #1 heard a strange noise and called out to co-worker. When he did not get an answer he walked around some of the mill structures some 40 feet away and found employee #2 lying face up on floor breathing heavy and unconscious. Employee #2 had sustained a broken bone in his left wrist during the unexpected slip, trip or fall.
An employee was descending a ladder when his foot missed the bottom rung causing him to fall. His landed on his elbow. His elbow was fractured.
Employee was climbing off a backhoe, missed a step, fell to the ground.
A PALLET CONTAINING ELECTRIC MOTORS WAS STORED ON A SHELF ABOVE FLOOR LEVEL. THE PALLET HAD BROKEN SPILLING THE MOTORS ON THE SHELF. AFTER THE MOTORS WERE REMOVED EE CLIMBED ONTO THE SHELF TO REMOVE THE PALLET. AS HE STEPPED OFF A PIECE OF PALLET BROKE OFF UNDER HIS FEET. HE GRABBED ONTO THE SHELF TO BREAK HIS FALL AND FELT PAIN IN HIS RIGHT SHOULDER.
WHILE WALKING TO WHERE WORK WAS TO START EE SLIPPED AND TWISTED LEFT ANKLE.
EE WAS WALKING THROUGH THE PLANT, HE STEPPED AND SLIPPED ON A WET SPOT ON THE FLOOR. WATER/WET SPOT WAS OBSCURED BY DUST. THE INCIDENT OCCURRED IN THE 53 MILL AREA.
EE PREPARING TO UNLOAD LOADER OFF OF LOWBOY TRAILER, CLIMBED UP ONTO LOADER TO REMOVE TEMPORARY RAINGUARD FROM EXHAUST STACK. AS HE WAS CLIMBING DOWN HE TOSSED THE COVER. WHEN HE TURNED AROUND TO STEP DOWN, HE LOST HIS FOOTING & FELL APPROXIMATELY 5' - 6' TO THE GROUND. HE IS OUT OF WORK UNTIL AT LEAST 02/06/03 WHEN HE GOES TO ORTHOPEDIST.
INSTALLING FALL PROTECTION SYSTEM WHEN EE SLIPPED AND FELL INTO A SKYLIGHT. FELL APPROXIMATELY 16 TO 18 FEET.
HANGING WALL GIRTS HE STEPPED IN MUD, LOST FOOTING AND FELL, HITTING ARM ON COLUMN FLANGE.
EE SLIPPED ON WET CLAY & STRAINED HIS LEFT KNEE
EMPLOYEE TRIPPED OVER A FLOOR MAT AND FELL, STRIKING HIS SHOULDER ON AN AUGER.
EE WAS PICKING UP SMALL CROSS TIES AND PLACING THEM ON A PALLET EE FOOT SLIPPED AND WHEN EE CAUGHT HIS BALANCE HE STRAINED HIS BACK. EE DID NOT FALL. IT HAD BEEN RAINING.
EMPLOYEE FELL OUT OF CHAIR & STRUCK HAND ON TOOL BOX.
