EE was helping to unload bundle of rebar from truck standing on bed with foot against rebar while 2 others were pushing rebar off side. Assuming as bundle went off truck, rebar caught pant leg & he lost balance falling off truck with rebar landing on ankle/leg. Acute sprain & contusions to ankle & chest & abrasion to elbow. * Closed TXT Medial Mallelous Fracture with manipulation.
Independent Hauler slipped and tripped on steps to scales and fell 18 inches twisting left knee.
EE was carrying buckets of oil walking down a narrow hallway that was dark and covered with cement dust. EE tripped over metal object he did not see & landed on his left ankle. EE was asked by Cemex Lyons supervisor why he was limping and EE told him he had fallen. Personal Touch supervisor took him to dr. X-rays were done results not broke.
When removing door on rotary ball mill, the door lodged in the opening and employee went to dislodge without relieving tension, the load released and employee was pulled into the opening sustaining trauma including bruising and 2 cracked ribs.
Miner was climbing down a fixed ladder and lost footing on the ladder. Miner's foot and leg went through the rung of the ladder causing him to twist his knee. Miner was believed to be in a rush causing him to lose footing on the ladder.

While walking from barge office to the barge bins, employee slipped in a muddy spot and his left arm extended up behind his head as he attempted to maintain his balance. (He did not fall). Employee experienced dull pain in his shoulder which was diagnosed in late August 2013 as a rotator cuff tear. Employee will undergo surgery for repair.

Employees foot/leg became tangled with wire for computer keyboard while sitting at his desk, when he stood up and turned to walk away from his desk he was put off balance, tripped, and landed with his right hand in the eco cylce container.

EE was reaching down to pick up a jack hammer, he lost his balance and his hand was smashed between the jackhammer and a door frame. EE broke a bone in his right hand below the knuckle on his pointer finger.

The first Dr visit was 1/28/2010 - surgery was done on left shoulder 3/25/2010 - resulting in a light duty status. There has been no lost time with this injury.
Slipped on cement slurry while walking to Finish Mill, fell on left hand spraining thumb. Medical treatment not requested until 10/14/09, splinted thumb/wrist.

When moving pump and placing Pipe on Valve flange, pipe slipped off & employee lunged forward using his hand to brake from a complete fall. Employee felt discomfort in lower back and right groin area. Employee continued working and thought resting he would be OK. Continued to hurt. EE went to see Doctor. Doctor reported a tear in muscle of upper rt. leg.
Employee was walking outside packhouse slipped in ice/slush/wet conditions, and landed on rt side pain in rt ankle.

An extension ladder was being used to access the work area. EE was preparing to mark the holes for installation of the rubber seal. EE was using a one inch imact gun in the removal of finish mill liners in the 1st compartment of the mill. Some small balls came back into the first compartment. 7/8" diameter. As he was lifting the imact gun the EE slipped on the balls, striking his left knee.
