Emp lost grip on ladder between 2nd/3rd rung, fell approx 2 1/2 to 3 ft onto floor. Right knee hit floor. No medical treatment needed, no prescription, X-ray negative.
Employee slipped on a patch of ice while walking by the old raw mill. This was originally treated as a first aid but turned reportable when he went for a follow-up on 3/23/15 and was given an Rx for anti inflammation and scheduled physical therapy. No restricted duty was given.
EE was helping to unload bundle of rebar from truck standing on bed with foot against rebar while 2 others were pushing rebar off side. Assuming as bundle went off truck, rebar caught pant leg & he lost balance falling off truck with rebar landing on ankle/leg. Acute sprain & contusions to ankle & chest & abrasion to elbow. * Closed TXT Medial Mallelous Fracture with manipulation.
Independent Hauler slipped and tripped on steps to scales and fell 18 inches twisting left knee.
EE was carrying buckets of oil walking down a narrow hallway that was dark and covered with cement dust. EE tripped over metal object he did not see & landed on his left ankle. EE was asked by Cemex Lyons supervisor why he was limping and EE told him he had fallen. Personal Touch supervisor took him to dr. X-rays were done results not broke.
When removing door on rotary ball mill, the door lodged in the opening and employee went to dislodge without relieving tension, the load released and employee was pulled into the opening sustaining trauma including bruising and 2 cracked ribs.
Miner was climbing down a fixed ladder and lost footing on the ladder. Miner's foot and leg went through the rung of the ladder causing him to twist his knee. Miner was believed to be in a rush causing him to lose footing on the ladder.
Employee walking down stairs his foot slipped off. Put his hand out to catch himself. Employee fractured his metacarpal on right hand. Meds - given splint and placed 0n lifting restrictions
While walking from barge office to the barge bins, employee slipped in a muddy spot and his left arm extended up behind his head as he attempted to maintain his balance. (He did not fall). Employee experienced dull pain in his shoulder which was diagnosed in late August 2013 as a rotator cuff tear. Employee will undergo surgery for repair.
Employee was climbing into 777D truck, when he slipped, grabbed onto the handrails and strained his left shoulder.
Employee was exiting the forklift when his right foot slipped off. He continued hanging on with his left hand and felt a "pop" in his left shoulder. Prescribed Naproxen and placed on restrictions of no use of his left arm.

Note: 1st doctor visit 6/2/2011. On 5/24/2011 employee was climbing into crane cab slipped on ladder rung. Was able to hold on with both hands but caused a sprain/strain to right shoulder. Continued working notified supevisor of injury at end of shift. Weather conditions were wet and muddy.
Employees foot/leg became tangled with wire for computer keyboard while sitting at his desk, when he stood up and turned to walk away from his desk he was put off balance, tripped, and landed with his right hand in the eco cylce container.
Employee was dismounting an L-50 front end loader. After reaching the ground, he apparently stepped into a depression in the ground and caused a sprain to his left ankle.
Employee was climbing down ladder from our Cat Water Truck at the end of the shift. As he touched his right foot to the ground (where there was a high spot of a rut due to previous trucks parking) the high spot gave way causing him to lose his balance twisting his right ankle.
Injured was walking in the cooler room when he tripped on a bolt sticking out of floor. Injured threw his arms up to regain his balance and felt a pain in his left shoulder. Injured went to doctor on 6/10, but he was NOT put on restricted duty until 7/1/2010.
Injured was stepping off of a platform onto a ladder.Injured missed the step on the ladder and fell backwards (Approx.20"). Injured sustained bruises to lower back area.
EE was reaching down to pick up a jack hammer, he lost his balance and his hand was smashed between the jackhammer and a door frame. EE broke a bone in his right hand below the knuckle on his pointer finger.
Employee was helping to set up concrete forms. He went to step over a stationary form already in place and misstepped and tripped over a form. A co-worker was trying to put in place but was not yet stationary and EE lost his balance & fell on left knee on frozen ground. No lost time - no restrictions - will do physical therapy.
While working unloading coal cars - cleaning off grizzly above the feeders. left foot slipped into the opening on the grizzly causing employee to fall on top of the grizzly landing on left shoulder and left elbow. The first Dr visit was 1/28/2010 - surgery was done on left shoulder 3/25/2010 - resulting in a light duty status. There has been no lost time with this injury.
Slipped on cement slurry while walking to Finish Mill, fell on left hand spraining thumb. Medical treatment not requested until 10/14/09, splinted thumb/wrist.
Employee was stepping down from Cat 40 ton truck, slipped on the last step and struck his knee against the ladder.

Employee was exiting a D-8 Dozer, missed the foot step and fell from the dozer
When moving pump and placing Pipe on Valve flange, pipe slipped off & employee lunged forward using his hand to brake from a complete fall. Employee felt discomfort in lower back and right groin area. Employee continued working and thought resting he would be OK. Continued to hurt. EE went to see Doctor. Doctor reported a tear in muscle of upper rt. leg.
Employee was walking outside packhouse slipped in ice/slush/wet conditions, and landed on rt side pain in rt ankle.
Snow and freezing temperatures, employee was walking to the Electric office, along side roadway, slipped and fell. EE landed on his back and hit his head on the ground. EE did not lose consciousness, had discomfort in the upper middle back and a headache.
Employee was stepping from fuel truck, slipped under icy conditions and struck forehead against fuel tank, resulting in 5 stitches to close the wound.
Employee was carrying a bucket of ANFO to the holes. After loading a hole he was on the way back down on a slope and lost his footing. During the fall he caught himself and strained his left middle finger. Went back to DR 9/26 and became a medical treatment injury.
Stepped in puddle of water on catwalk. When stepped on stairs feet went out from under and employee landed on left elbow, bruising/jamming left shoulder. Right hand hit hand rail jamming right hand ring finger at center knuckle.
Employee was tied off stepping from man-lift onto cement cooler. Stepped into opening around cooler shaft and twisted his left knee. Returned to regular job, no restrictions. 7 months later still having problems. Had knee scoped and medial meniscus repaired.
Employee was walking into feed-end building where GFK2 feed pump is located. The clean-up people had suck truck and hose on ground. When employee stepped over hose, he caught his left foot on hose and came down hard on ground striking left knee.
Employee was stepping off of a D8 dozer and twisted his right knee when stepping to the ground.
Twisted left knee while walking from parking lot to work location (Vehicle Shop in Quarry). Surface was very muddy from rain from the prior evening. Note: Employee did not receive medical care until 05/20/2008, at which time he was returned to regular duty - with Physical Therapy appointments and a knee brace.
An extension ladder was being used to access the work area. EE was preparing to mark the holes for installation of the rubber seal. He stated that as he climbed the ladder, he stopped momentarily on the first rung before proceeding. He then climbed to the third rung. The ladder then slipped out from underneath him, causing him to fall and strike his jaw on the Crusher frame.
Employee waslking down preheater steps & missed a step causing pain in knee. Tried physical therapy. Became a lost time when employee had surgery April 1, 2008. First day off April 1, 2008.
Employee was moving parts with other employees. When he was done, he turned away and his left tow was caught under a stack of equipment. He twisted his left knee when he turned away.
Employee was walking on the west side of kiln coal mill and slipped on water and mud in this area. Recent rains had caused this area to acculumate water. Employee experienced sprain of left knee area.
Injured was descending steps on east side of preheater and slipped in some standing water(recent heavy rains) employee fell and twisted left knee. Injured did not seek treatment until 2/6/08
EE was using a one inch imact gun in the removal of finish mill liners in the 1st compartment of the mill. Some small balls came back into the first compartment. 7/8" diameter. As he was lifting the imact gun the EE slipped on the balls, striking his left knee.
Employee was carrying a rivet buster into kiln when he slipped and caught himself with his hands. Rivet buster fell onto his left ring finger and crushed it.
While carrying a bag of trash, he passed by an air horn, lost his balance and fell on his buttock causing unknown injury.
Employee fell in the minidome and cut the bridge of his nose.
Employee stated he was climbing up a step ladder when he fell off the ladder resulting in a right knee sprain. Underwent arthroscopy right knee surgery on 1/16/2008.
