On 10/17/22 around 5:40am, employee was walking to the shipping office to clock in for work when slipped and fell on right side injuring shoulder. The area that the EE was walking through was wet from the rain during that time. The slip & fall resulted in the employee dislocating the shoulder and having to be put on restricted duty.

Showing all 39Employee was performing an inspection of a conveyor belt. As they were climbing down a ladder they stepped in mud build-up twisting their foot and leg. The twisting motion caused them to temporarily lose footing and employee felt a pain in their back.
Employee was walking near the Ball Mills when EE stumbled and fell backward on to the ground. EE's left elbow scraped a cement pad causing a laceration that required sutures to close.
Employee was trying to climb up a small ladder while carrying a 44 lbs motor in EE's hands and lost EE's balance. The motor slipped and pinched EE's hand between the ladder and the motor causing a contusion

EE was moving from the platform between the cab and the glad hands when EE's foot slipped off step. EE caught self by grabbing the handle on the side of the tractor with EE's left hand. EE felt pain in EE's left shoulder.
Employee was descending the stairs on a haul unit when EE lost footing on a slick surface. EE grabbed the handrail with left arm and managed to swing self to the ground landing on feet. Pain in back developed over the next few days. Medical visit on 10/27/16.
employee was changing lights on #3 RK Kiln Deck. As EE was helping to dismantle the scaffolding, EE's left foot slipped into a groove twisting EE's left ankle. Pain and discomfort increased and subsided over the next few weeks. Ultimately, the pain and soreness increased prompting a medical visit on 11/3/16.
Employee was going up the stairs on P-11 Conveyor Belt when EE slipped at bottom 2nd or 3rd step. EE held the rail with the right hand as EE fell forward and braced self with left hand to keep face from hitting the stairs. The fall pulled on the right shoulder really hard.
Employee's foot got tangled in the cap wire while loading shots. He tripped, stumbled forward and braced himself from falling using the loading pole. His upper body pushed against the pole and wrenched his arm in an awkward position pulling an arm muscle.
Employee was standing on the top of face in the Quarry looking at how the shot had gone. The ground beneath him gave way suddenly causing his left leg to buckle at the knee. Requested medical treatment on 11/17/14. Diagnosis sprain knee.
Employee was walking up the access gate to enter the C-Section of Crushing and Screening. He slipped on the ground at the access gate and caught himself by extending his left arm. The palm of left hand struck the ground and bent his wrist backward.
Employee was climbing out of the operators cab of the Water Truck when he lost his footing on the bottom step. When his foot slipped off the step, he lost his balance, let go of the cab grab rails and fell to the ground. The back of his head and back struck the ground.

Employee slipped on a rock and lost footing. As he fell to the ground, he extended his right hand down to break his fall. His right shoulder and right elbow struck the ground.

On 7/3/12 the employee was walking down a set of steps. He skipped the second-to-the-last step and his back popped. He felt pain in his lower back a little while later. The pain got worse over the next three days and he requested medical attention on 7/10/12. Diagnosis lumbar sprain.
Employee stepped up onto the platform at the rear of a tractor trailer in order to disconnect a hose. A strong gust of wind blew past him and he lost his balance. He jumped from the platform to the ground landing on both feet but immediately fell to the ground unable to get up. He was diagnosed with a fractured tibia.
While leaving a work area the employee tripped while going down a flight of stairs and bumped and cut his head. Employee was treated at ER and release the same day.

Employee was blowing off the deck around the coal belt and classifiers. As he performed this task, the employee backed up towards a flight of stairs. The employee fell down approximately 12 stairs to the landing below, suffering undisplaced fractures to the left and right wrists, as well as a nose fracture.
Employee claims that while repairing the cyclone air lock on top of hydrate bin #5, he placed a section of 2" X 8" wood across a guard rail support and a metal frame about 14 inches from the floor. The employee than stood on top of the 2X 8 to assist is repairing the air lock. The wood broke and he fell to the floor, striking his right arm above the wrist.
EE CLAIMS THAT HE FELT A SHARP PAIN IN HIS RIGHT KNEE WHILE HE WAS DISMOUNTING FROM A FORKLIFT. HE CLAIMS THAT HE FELT HIS KNEE TWIST AT THE TIME HE FELT THE PAIN. HE HAD BEEN UNLOADING A TRAI LER OF EMPTY PALLETS BEFORE THE INJURY.
EE CLAIMS THAT HE FRACTURE HIS LEFT ANKLE WHILE HELPING TO ASSEMBLE A CAR-HOE IN THE SOLID FUELS STORAGE AREA. EE CLAIMS THAT HE JUMPED TO THE GROUND (ABOUT 30 IN.) FROM THE CAR-HOE AND HIS LE FT FOOT LANDED ON A COMEALONG THAT WAS LAYING ON THE GROUND AND TWISTED HIS ANKLE. HE WAS RELEASED TO RETURN TO WORK ON LIGHT DUTY AFTER TWO DAYS OF LOST TIME.
EE CLAIMS THAT HE FELL OFF OF THE CAB PLATFORM OF A CATERPILLAR, MODEL IT38, FRONT-END LOADER. EE CLAIMS THAT HE DROVE THE LOADER THRU THE DRIVER THRU CAR WASH AND THEN WENT TO THE LUBE SHOP T O DROP OFF THE LOADER TO GET IT SERVICED. HE FURTHER STATES THAT AS HE WAS ABOUT TO DISMOUNT FROM THE LOADER, HE ATTEMPTED TO BRUSH OFF SOME LIME PEBBLES FROM THE TOP OF THE CAB WHEN HE SLIPP
EE CLAIMS THAT HE SLIPPED AND FELL WHILE SHOVELING DEBRIS FROM THE INSIDE OF ROTARY KILN #2 SCRUBBER, CAUSING HIM TO TWIST HIS BACK. HE WAS TREATED AT PRO-MED CLINIC AND RETURNED TO WORK ON LI GHT DUTY.
EE CLAIMS THAT AS HE WAS STARTING TO CLIMB THE STAIRS TO THE ROTARY KILN PREHEATER, HIS RIGHT FOOT SLIPPED AS HE PLACED HIS RIGHT FOOT ON THE FIRST RUNG OF THE STAIRS. HE THINKS HE MIGHT HAVE STEPPED ON A ROCK. THERE WAS NO EVIDENCE OF LOOSE ROCK OR SLIPPERY MATERIAL IN THE AREA OF THE ALLEGED ACCIDENT. HE WAS RELEASED TO RETURN TO LIGHT DUTY BY HIS DR.
