Over-exertion (Not Elsewhere Classified)

Employee was descending the stairs on a haul unit when EE lost footing on a slick surface. EE grabbed the handrail with left arm and managed to swing self to the ground landing on feet. Pain in back developed over the next few days. Medical visit on 10/27/16.

Absorption of radiations, caustics, toxic and noxious substances

Employee was transferring Brake Parts Cleaner from the 50 Gallon Drum into a 5 Gallon Bucket. As EE was cranking the hand crank attached to the 50 gallon drum the nozzle spun out of place and sprayed a stream of brake cleaner onto the top of the bucket. The brake cleaner splashed up into behind EE's glasses, onto EE's face and into eyes and mouth.

Fall down stairs

Employee was going up the stairs on P-11 Conveyor Belt when EE slipped at bottom 2nd or 3rd step. EE held the rail with the right hand as EE fell forward and braced self with left hand to keep face from hitting the stairs. The fall pulled on the right shoulder really hard.

Struck by falling object

Employee was removing a drive shaft from Loader. As EE was lowering the shaft down, it fell to the ground. One end of the shaft landed on top of EE's right hand.

Over-exertion in lifting objects

Employee lifted a 5 lbs bucket with tools inside of it (wrenches, hammers etc) from the ground. EE immediately felt pain in EE's lower back. EE requested medical treatment on 1/6/16. Diagnosis: Back sprain.

Over-exertion in lifting objects

Employee was shoveling spilled material off of the cement pad at the P-13 Conveyor Belt. In the process of shoveling, his left knee began to hurt. He sought medical attention on 12/18/15. Diagnosis: leg sprain

8-22-15 Employee not scheduled to work-seen for medical evaluation due to not feeling well. 8-24-15 Did not report to work due to medical evaluation. 8-25-15 reported to work per medical release-remained in scaleshouse-complained of dizziness-fell to scalehouse floor-transported to medical facility in ambulance. 8-26-15 remains hospitalized diagnosis unknown.

Employee's foot got tangled in the cap wire while loading shots. He tripped, stumbled forward and braced himself from falling using the loading pole. His upper body pushed against the pole and wrenched his arm in an awkward position pulling an arm muscle.

Struck by falling object

Employee was cleaning up spilled rocks under conveyor. A piece of limestone weighing approx 14 ½ lbs fell approx 35 feet from above, struck him on his hard hat knocking it off employee remained conscious and requested a medical evaluation. Was driven by supervisor to a medical facility. Diagnosis is a mild concussion.

Employee was getting down from the cab of the truck, stepped onto first step when his foot slipped. he lost his balance and fell striking his left knee against the ground.
