Showing all 50Employee was walking on a drill bench and stepped into a drill hole felt a pop in right knee but continued to work. On 9/10/2025 the employee was taken off work by their doctor for torn Meniscus.
After climbing up on to the track of a Dozer, the employee was attempting to latch the drop-down handrail when the handle broke and they lost balance and fell. The employee was taken to clinic, employee has fractured Spinous Process, given medication and rest for a week.
Employee was exiting the Number 2 truck dump slipped on the step and fell. EE went to Logan medical center and x-rays show a fractured rib.
Employee was walking around equipment steam cleaning, stepped into a low place and twisted EE's ankle. Had x-ray and told to take off work for a sprain ankle.

Employee was climbing upon a Rock Truck, when EE's foot hit the first platform on the lower level EE's foot slipped and EE lost grip falling on to the ground approximately 3-4 feet on EE's left side. EE was transported to the Hospital and later released that evening.
Employee stopped EE's service truck in an area where a drill had been repaired. When EE walked around to shut a cabinet door EE stepped into a drill hole. EE received a sprain left ankle and a small fracture in EE's foot.
Employee was working on a drill, EE slipped and fell in the walk way hurting EE's left ankle. Employee went to the hospital X-rayed and given time off to rest.

Employee was exiting cab of dozer. When they stepped on the deck it had oil on the deck and they slipped and fell hitting an elbow on the deck and hit their tailbone on the dozer track.

Employee states his foot slipped coming back up over a belt line to get back to a catwalk. As he slipped he caught all 210 lbs of his weight with his left arm stretching tendons on left shoulder. Did not report injury to office until 7-11-05.
THE MECHANIC WAS ATTEMPTING TO CLIMB DOWN OFF THE BACK OF A MECHANICS TRUCK WHEN HIS FEET SLIPPED CAUSING HIM TO FALL TO THE GROUND ON HIS RIGHT ANKLE (STRESS FRACTURE)
A SAFETY CHAIN IS LOCATED AT THE END OF THE WAREHOUSE LOADING DOCK. THE CHAIN WAS IN PLACE AND THE EMPLOYEE WAS LEANING ON IT WHEN THE LATCH OR HOOK BROKE CAUSING HIM TO FALL BACKWARD STRIKING THE BACK OF HIS HEAD ON A PVC PIPE.
FALL TO GROUND WHILE WALKING
EE WAS CLEANING UP TRASH OUT OF THE CREEK BEHIND THE SECURITY STATION. EE STEPPED ON A ROCK AND SLIPPED AND FELL. EE HIT FACE ON ROCK. EE BROKE HIS NOSE AND LACERATED HIS FOREHEAD ABOVE HIS LEFT EYEBROW.
TWISTED RIGHT ANKLE WHILE WORKING ON A 785 TRUCKSPINDAL.
EE WAS WALKING AROUND BACK OF SERVICE TRUCK AND TRIPPED OVER A ROCK. WHEN HE FELL FORWARD, HE PUT HIS ARMS OUT TO CATCH HIMSELF, FELL ON RIGHT HAND/WRIST.
HE WAS DUMPING THE COAL TRUCK, AFTER HE DUMPED HE GOT OUT TO SEE IF TRUCK WAS EMPTY. HE FELL ON ICE AND INJURED HIS KNEE.
EMPLOYEES EXCAVATOR HAD BROKEN DOWN. WHILE ASSISTING A MECHANIC HE CLIMBED UP ON A DOZER BLADE TO RETRIEVE A TOOL. WHEN DESCENDING FROM THE BLADE HE TWISTED HIS KNEES. CONTINUED TO WORK THEN W ENT TO DR ON 12-4-01. MRI COMPLETED, TORN CARTILAGE. SCHEDULED FOR KNEE SURGERY ON 12-3-01.
