Placed ladder on uneven surface, climbed ladder with a 3" pipe on shoulder, ladder shifted causing employee to fall striking ladder and ground.
Employee states that employee was dragging ventilation tubing and tripped landing on knee. Employee says that the knee landed on a rock or chunk of coal.

Grating had been removed from a floor drain. The IE could not see the drain that was full of water and had stepped into it causing a sprain/strain to their left knee.
Employee was walking on the second floor of the train load out to trace out a strip of heat tape for the load out waterline. The employee tripped over a used motor and pump that was out of service sitting on the floor. The employee landed on elbows causing pain to left elbow and shoulder.
EE was walking through Surface Weld Shop when they slipped in a puddle, catching themselves with right arm. EE did not experience any pain as a result of the fall until next morning when they began to notice pain in shoulder. EE was referred to an Orthopedic and continued to work until 08-30-19 when they had surgery to repair a torn rotator cuff.
IE was setting up to do an alignment on a motor to pump coupling on 5mn xc 25. as ee was walking around the pump skid ee stepped off the pump skid rail, rolling foot outward. IE sustained a fracture to the 5th metatarsal bone and missed next scheduled shift on 11/14/18 making this reportable at that time.
Injured worker was involved with a slip, trip and fall incident in which the IW fell striking EE's head on the mine floor. IW was taken to the emergency room and immediately released to full duty. IW remained working, but continued to experience symptoms related to the injury. IW's first loss time date was on 10/25/2018.
A plant operator was helping lift a piece of HDPE pipe to reconnect the bent sieve circuit. Another employee was lifting the pipe but was having difficulty getting it into place. The IE employee stepped onto the top of the bent sieve bin and at this time, the pipe shifted, knocking the IE off balance causing IE to fall.
EE was walking across floor and stepped on some flocculent that had been spilled and not noticed. Employee fractured a wrist.
EE told safety on May 24th of back, leg and knee pain. On June 2, EE sought medical treatment. Diagnosis unclear with possibility of two separate issues or all back related. EE stated stepped in a hole January 28, felt discomfort but continued work. In May EE's pain was below the knee & up the thigh. MRI & surgical notes show degeneration/torn meniscus. Meniscectomy on 7/11.
EE diagnosed with lateral meniscus tear & other knee pathology in 2014 that will eventually require knee replacement. EE returned as a fire boss in 2015 and in Jan. 2017, slipped doing ee fire boss duties. EE felt pain in knee, MRI revealed the same meniscus tear. IE subsequently had surgery 3/3/17 for meniscectomy. Iron worker was cutting apart an old discharge chute with a cutting torch. When the chute section was cut apart it shifted causing the employee to lose EE's balance and strike EE's left wrist against a sharp piece of metal. EE received a laceration to the top of EE's left wrist and ulna tendon which required surgery to repair.
Pain in knee after bolting in 01/2016. IW declined treatment. 01/2017 the IW had surgery for lateral meniscus repair and debridement, patellar tendon debridement and repair of chronic patellar tendonitis. Diagnosis was a longitudinal tear of the lateral meniscus and chronic Patellar tendonitis, and early parameniscal cyst formation consistent with chronic, degenerative conditions.
Employee was walking to perform pre-shift on his mantrip. He slipped on the metal grating causing him to fall. He received a laceration to his knee as a result of the fall, which required sutures to close.
Employee was walking up the stairs to the control room and tripped on the 3rd stair, landing on his left knee. The impact caused the skin to rip just below the knee cap. The wound required sutures to close.
while traveling through a man door carrying a label maker, the employee slipped and fell, catching his leg between the door and the frame. The employee returned to full duty. an MRI was conducted and the employee required surgery and missed his first shift on 11/13/13.
EE was leaving from work when she slipped on ice and broke her ankle. It was a very hot day the day before but then snowed as the sun went down, re-freezing everything that had been melting. Heidi lost her balance and fell straight down to the parking lot.
Employee climbed up ladder to tighten tube strap. He lost his footing on the ladder and slipped. Falling onto the ground.
Employee climbed a ladder to the third rung of a 3-legged ladder. The ladder slipped out from under him and he fell approximately three or four feet to the mine floor, landing on his right side. Employee was taken to the ER and was diagnosed with several fractured ribs, liver contusion and a punctured lung.
EE complained about knee pain shortly stepping out of mantrip 698. He said his right knee popped. Observation of area showed no unstable ground. EE had commented he has had problems with his knee doing this before, contributed it to popping due to how he was sitting in the rear seat of mantrip 698.
Employee states he was assisting in installing a concussion door. As he was holding the door in place, it was hit with a blast of air, knocking him backwards and causing him to fall. He received a laceration on his head requiring six stitches and a dislocated finger on his right hand. Employee received medical care and was released for work.
EE was carrying a piece of timber down a hill, he stepped in a washed out area and twisted his left knee.
Employee states he had climbed up a ladder to check a possible overfill imulsion tank. He states the light was in his eyes and as he leaned in one direction to get out of the light the ladder moved in the opposite direction. As he started to fall he pushed away from the ladder and landed on his right side. He was transported to the ER and diagnosed with 5 fractured ribs.
The employee was cutting on the longwall when he stepped off a shield causing him to stumble striking his left side on a base lift. He was taken to the ER where he was then diagnosed with a fractured rib. He showed up for his next scheduled shifts that weekend. We are filing in protest after the mine was issued a citation 8468426 on 08/26/2010.
Employee states he stepped out of a mantrip in a hole and fell face first to the ground. He landed in a push-up position but his right shoulder took the most impact. Employee continued to work but ultimately went off for shoulder surgery on 08/11/2010.
Employee states he was cutting mesh down on a ladder to form a seal. Rib came down, employee jumped off of the ladder onto kennedy panels onto left ankle. The injuried employee was taken by ambulance to the ER where it was determined that he had a serious sprain to his left ankle.
Employee slipped on wet/icy roadway and put their right hand down to catch themselves, jamming their wrist. Employee was diagnosed with right elbow lateral epicondylitis (right wrist sprain). His work restrictions made this event reportable.
Exiting shower, post shift, slipped and fell and hit L knee on bench, causing laceration.
Employee states he was getting into a uniloader he lost his footing and his hand slipped off the handle. He fell backwards landing on his left shoulder and hitting his head on top of structure skid.
Employee states he was in the borehole tank room and a pipe on the rock dust tank came loose, filling the air with rock dust. As he exited the room to get out of the rock dust, he walked into the vent air stream, which pushed him against the rib and bruised his left shoulder. Employee was placed on light duty.
Employee states he was walking in the mine, slipped, fell and landed on his right knee. Employee has not missed any time from work; however due to ongoing discomfort with his knee he was placed on light duty 11/02/2009.
Employee states he was walking down grade in the mine in a muddy area and as he stepped in mud on tire track, he twisted and sprained his left ankle.
Employee states he was walking around the side of the miner when he stepped on a slab of coal with his right foot which caused him to fall on his left foot. The fall resulted in a sprained left ankle and a spiral fracture to the left fibula.
Employee states as he was carrying some structure, he tripped on a magnum hanger bolted to the floor. As he started to fall, he reached out and grabbed a hold of the load bed to stop his fall and felt a pop in his left shoulder. He has shoulder surgery scheduled for July 30, 2009 which will be the first scheduled work shift he will miss.
Stocking bolt tray, standing on supply tray. Stepped across walk way to other side of tray, slipped and fell to walkway, hitting groin area.
WHILE EMPLOYEE WAS WALKING TOWARD ROOF BOLTER, HE SLIPPED AND FELL AT THE SAME TIME THE BOLTING MACHINE OPERATORS LOWERED THE BUMPER OF THE BOLTER IN ORDER TO LOAD MESH ON TO IT THE EMPLOYEES LEG HAD SLIPPED UNDERNEATH THE MACHINE. THE MACHINE CAME DOWN ON TOP OF HIS LEFT ANKLE BRUISING IT.
EE states while standing on belt structure lifting a cable and ring slipped causing the load to shift. EE was jerked backwards by the right arm holding the cable causing a pulling sensation on the right side of his abdomen. By the end of his shift he felt a knot developing.
Employee was working an overtime shift and was returning to the Tipple to eat lunch. He tripped while he was walking away from the truck. He heard his ankle pop and was taken to the Hospital. He has a broken right ankle.

Employee stepped in a hole on the belt line with his left heel causing it to get stuck in the mud. When he tried to step forward his knee locked up, hyper-extending and straining his left knee.
Ee was Firebossing when he slipped and fell striking his left knee in beltline.
Employee states as he lifted himself up to exit the cab of the 25X Scoop, his right foot caught between the seat and lever controls. He lost his grasp and his upper body started to fall from the cab. As he twisted to catch himself, his right knee was turned and sprained.
Employee states as he stepped back from some falling rib, he stepped on a chunk of coal, causing him to fall and land on his left hand.
Employee states as he was walking down some steps in the belt entry walkway, his boot slipped causing him to lose his balance and fall forward to the mine floor. When he landed on the ground, his right handed hit some loose debris cutting him on the palm of his right hand. The cut required several stitches.
Holding door open while vehicle passed through rather that using safety equipment provided to secure door and move to safer area to allow vehicle passage. Got caught between door and trailer attached to vehicle, knocking him to ground.
Employee states as she was walking through a fire curtain, she tripped on the curtain and fell, rolling her left ankle.
Employee states while surveying, he was dismounting a ladder, missed his footing and his foot slid to the floor. He felt a pop and pain in his left knee upon impact. Employee continued to work until he had knee surgery on 2/27/08.
Employee slipped and fell on icy pathway while walking to plant. Employee suffered back pain and sought medical attention on 2/13/08. See attached explanation.
