A PICI contract employee was working on the No. 29 Belt and slipped in the mud and twisted his left knee.

Employee was dragging an oxygen tank down the slope and he tripped and fell onto his right side/shoulder. He has been diagnosed with a fractured rib.

PICI Contract employee strained his lower back while helping hang the miner cable. He was lifting cable with his left shoulder and slipped on a rock and the cable fell on him.

Employee was pulling a dust hose through a man door & twisted his left foot. He was diagnosed with a stress fracture of his left 5th tarsal bone.

EE WAS THROWING A TRACK SWITCH WHEN THE BAR BROKE CAUSING EE TO FALL ON LEFT WRIST. EE SUFFERED A FRACTURED WRIST AND A CAST WAS APPLIED

Employee slipped on belt guide and fell between dolly and LoLo structure.

EE TRIPPED AND FELL THRU AN OFFICE DOOR. HE DIS-LOCATED A BONE IN HIS FOREARM AND A PLASTER CASTWAS APPLIED.

EMPLOYEE JUMPED FROM A MOVING BATTERY MOTOR AND INJURED HIS LOWER BACK. HE EXPERIENCED EXTREME PAIN AND WAS HOSPITALIZED FOR OBSERVATION.

EMPLOYEE WAS STANDING ON INBY SIDE OF TAKE-UP WHEN HE TRIPPED AND FELL, LANDING ON HIS BACK AND W-65 SELF-RESCURER.

EE WAS SPOT-BOLTING WHEN HE SLIPPED AND FELL WHILE TRAMMING ROOF BOLT MACHINE ON HIS LEFT FOOT.

EE TRIPPED AND FELL WITH HIS LEFT KNEE STRIKING A BLOCK OF COAL.

EE STEPPED ON SIDE OF LO LO STRUCTURE & SLIPPED,INJURING HIS LOWER BACK.

EMPLOYEE WAS INSERTING A PIN IN HOLE STRUCTURE WHEN A RAIL SLIPPED AND KNOCKED HIM TO THE GROUND. EMPLOYEE BEGAN MISSING WORK ON 7-3-01.

EE WAS CROSSING A MBC BRIDGE UNIT WHEN HIS FOOT SLIPPED AND HE FELL INJURING HIS RIGHT TESTICLE. HE HAD OUT-PATIENT SURGERY BECAUSE OF INFECTION 5-23-01.

EE WAS PULLING BELT WITH A SCOOP WHEN HE TOOK CHAIN OFF AND SLIPPED AND TWISTED HIS KNEE(LEFT). EE MISSED THE 14TH & 15TH BECAUSE OF INABILITY TO SCHEDULED DR. APPOINTMENT.

SLIPPED AND THREW RIGHT KNEE OUT OF PLACE.

EE TWISTED ANKLE & BROKE BONE IN FOOT. EE DID NOT GO TO DR UNTIL 2/9/01 & HE FOUND OUT THEN THATHIS FOOT WAS BROKEN

EMPLOYEE WAS SETTING TIMBERS, SLIPPED AND FELL, HURTING BACK.

GETTING OUT OF A RAILMOTOR AND TWISTED HIS KNEE.STARTED MISSING 9-20-2000.

EMPLOYEE SLIPPED AND FELL AND HURT BACK.

EMPLOYEE TWISTED HIS RIGHT KNEE AS HE STEPPED OFF TAILPIECE.

EE WAS ROCK DUSTING BELT ENTRY WHEN HE FELL AGAINST THE BELT STRUCTURE. EE MISSING WORK 4-25-00.

EMPLOYEE WAS STEPPING OFF THE BELT AND TWISTED HIS LEFT ANKLE. BEGAN MISSING ON 4/12/00

EE STATED THAT HE WAS WALKING AROUND SCOOP AND FELL, HURTING HIS BACK.
