Employee noticed a rope hanging out of the feeder. EE tried to pull it out and as EE pulled, it broke free causing EE to strike their arm against a pull cord smith bracket.

EE was performing routine maintenance. EE was greasing the heavy media cyclone. EE picked up the pneumatic grease gun with right hand on the handle and left on the side of the pump. The pump cycled one last time catching EE's left hand index finger

Operator was moving forward in the dozer and dropped the blade causing them to come to a sudden stop that resulted in hitting their head against the window latch.

Employee was installing a tail pulley guard, slipped and fell down onto the belt structure and hit their left side abdomen. Employee was diagnosed with a fractured rib. Employee returned back to work on the same day of injury.

Miner failed to lock out/tag out conveyor while installing deicer spray bar brackets

Employee was cutting brush/tree saplings on an impoundment dam and slipped and fell. EE sustained a cut to left leg/shin that required sutures.

Employee was inspecting an impoundment area and slipped on some ice and fell down.

Employee squatted down to examine load cells on truck scale and when ee stood up ee felt a pain in right knee. Employee started missing work on 1-26-2016 due to knee hurting.

Employee was shoveling mud out of the clean tunnel walkway and had a sharp pain in back. He continued to work and started missing work on 7/15/15 due to him not getting better.

Using the hand wrench to remove a tight bolt. The bolt was located in a confined area of the machine and when technician pulled the wrench to remove the bolt his left shoulder made a snapping noise and started hurting. The pain increased that evening in to the next day.

Employee was lifting and putting in a bottom roller and felt a pain in his shoulder. Had surgery on 4-29-2011. Started missing work 4-29-2011.

He was going up ladder, his foot slipped and he fell forward and hit his forehead. His safety glasses cut his forehead at the eyebrow. Sutures were required - no lost time.

He was climbing down a feeder and fell and hit right shoulder.

Foreman/Operator had been checking JD 400 for mechanical problems, he was coming down the ladder and missed the last step and fell.

EE was taking belts off of an overflow pump and got finger caught between belt and sheave. 6 sutures required.

Employee cut his left 3rd, 4th and 5th digit fingers with an electric chop saw in the shop area of the plant. Started missing work on 2/23/08.

Fractured hand while working on chute and come-a-long slipped and let chute on hand. Started missing on 10/11/07.

EE was working on Vibrator Screen, reached down to pick up side rail and shoulder came out of place.

EE WAS HOLDING REBAR AND RT MIDDLE FINGER GOT HIT WITH SLEDGE HAMMER, HE DID RECEIVE STITCHES ON MIDDLE RT FINGER.

EMPLOYEE INJURED HIMSELF SHOVELLING ON 3/12/05 AND BEGAN MISSING WORK ON 5/05/05 BECAUSE OF BACK STRAIN

EMPLOYEE WAS LIFTING METAL PIECE AND FELT PAIN IN HIS LOWER BACK. ORIGINALLY INJURED ON 6/28/04 BUT BEGAN MISSING WORK ON 7/19/04.

EE STEPPED OFF A PLATFORM AND STARTED HAVING PAIN IN LOWER BACK. DR. HAS DIAGNOSED HIM AS HAVING DEGENERATIVE DISC DISEASE. ""SEE ATTACHED DR.'S NOTES.""

EE WAS TURNING A VALVE WITH A PIPE WRENCH & BUMPED HIS RIGHT LEG BELOW HIS KNEE. EE BEGAN MISSING SCHEDULED WORK ON 10-27-03.

EE WAS SITTING WORKING ON SUPER SCALPER AND WHEN HE RAISED UP TO CHANGE POSITION HE FELT PAIN INHIS LEFT HIP DOWN TO HIS KNEE.

EE STOOD UP FROM A KNEELING POSITION AND LEFT KNEE FLEW OUT OF PLACE. EE HAD SURGERY ON THIS KNEE LAST YEAR. IT WAS DETERMINED THAT A NEW INJURY OCCURRED ON JAN. 28TH 2003.

HE WAS OPERATING A DOZER WHEN JOLTING AND JARRING STARTED HIS BACK TO HURTING. MRI SHOWS DEGENERATIVE CONDITION IN HIS BACK. EE BEGAN MISSING WORK ON 1-2-03

EE WAS PUSHING A PIECE OF BELT DOWN INSIDE A SIEVE BIN WHEN BELT SLIPPED AND SEEM HIT SIDE OF BIN. RECIEVED MEDICAL INFORMATION THAT WOUND REQUIRED STITCHES ON 8-26-02.

EE PICKED UP A 72"" ROLLER AND FELT PAIN IN LEFT SIDE OF GROIN. HE WAS DIAGOSED WITH A HERNIA AND HAD SURGERY ON 8-13-02.

ON FEBRUARY 21, 02 A DYE TEST CONFIRMED THAT A SMALL SEEPAGE FROM STONECOAL BRANCH SLURRY DAM WAS DISCHARGING INTO AN ABANDONED MINE. BRANCH SLURRY DAM IS STRUCTURALLY SOUND AND NO DAMAGE IS PRESENT.

LIFTING FAN HOUSING STRUCTURE WHEN IT FELL ON HIS LEFT FOOT.

EE WAS ATTEMPTING TO ROUTE A 4"" WATER LINE TO A DIFFERENT POSITION. HE AND A FELLOW EE RAISED THE LINE ABOVE HEAD LEVEL. TENSION FROM A BOW IN THE LINE WHEN RELEASED CAUSED THE PERSON TO LOS B ALANCE AND HIS BACK STRUCK A SHARP METAL OBJECT RESULTING IN A FRACTURE TO THE SPINAL COLUMN.

EMPLOYEE TRIPPED AND FELL DOWN 10 STEPS AND INJURED BACK AND PAIN IN LEGS.

FINGER GOT CAUGHT IN FAN ON MOTOR. LACERATION REQUIRED SUTURES.

EMPLOYEE STATED THAT HE TWISTED HIS LEFT KNEE WHEN HIS FOOT SLIPPED OFF OF A FLIGHT.

EMPLOYEE MASHED HIS RIGHT THUMB WHEN A LID TO A DRIER CAUGHT HIS THUMB.
