Struck against stationary object

Employee noticed a rope hanging out of the feeder. EE tried to pull it out and as EE pulled, it broke free causing EE to strike their arm against a pull cord smith bracket.

EE was performing routine maintenance. EE was greasing the heavy media cyclone. EE picked up the pneumatic grease gun with right hand on the handle and left on the side of the pump. The pump cycled one last time catching EE's left hand index finger

Struck against a moving object

Operator was moving forward in the dozer and dropped the blade causing them to come to a sudden stop that resulted in hitting their head against the window latch.

Employee was installing a tail pulley guard, slipped and fell down onto the belt structure and hit their left side abdomen. Employee was diagnosed with a fractured rib. Employee returned back to work on the same day of injury.

Employee squatted down to examine load cells on truck scale and when ee stood up ee felt a pain in right knee. Employee started missing work on 1-26-2016 due to knee hurting.

Employee was shoveling mud out of the clean tunnel walkway and had a sharp pain in back. He continued to work and started missing work on 7/15/15 due to him not getting better.

Using the hand wrench to remove a tight bolt. The bolt was located in a confined area of the machine and when technician pulled the wrench to remove the bolt his left shoulder made a snapping noise and started hurting. The pain increased that evening in to the next day.

Employee was lifting and putting in a bottom roller and felt a pain in his shoulder. Had surgery on 4-29-2011. Started missing work 4-29-2011.

He was going up ladder, his foot slipped and he fell forward and hit his forehead. His safety glasses cut his forehead at the eyebrow. Sutures were required - no lost time.

ON FEBRUARY 21, 02 A DYE TEST CONFIRMED THAT A SMALL SEEPAGE FROM STONECOAL BRANCH SLURRY DAM WAS DISCHARGING INTO AN ABANDONED MINE. BRANCH SLURRY DAM IS STRUCTURALLY SOUND AND NO DAMAGE IS PRESENT.
