Found an unintentional fall on the #3 Belt line (#3 Entry) at break # 20 on the #5 PM Panel.
Employee was observing the continuous miner and a piece of draw rock fell and struck the back of his leg and ankle. Fractured ankle.

A portion of a coal rib was being removed with a jack hammer for the purpose of correcting an issued close clearance citation. The injured employee was removing the loose coal away from the rib when a rock brow fell onto him.

An unintentional Roof Fall occurred at break #6 of the # 33 Beltline. Fall dimensions are 35' long x 35' wide x 8.5' thick
EE was drilling a hole in the top and a loose piece of scale fell and hit his left hand. Fractured the index carpal bone. He started missing work on 1-9-2014.
While bolting the ribs, EE got right 4th finger tip in the slide/hinge on boom and cut and bruised finger.
Employee was operating shuttle car and as he was turning his coat sleeve caught the rib and it pulled his arm between car and the rib and bruised his arm in which medical personnel later put a cast on it to help protect the muscle and tendons from future damage.
Employee was using a utility knife to trim the edges of a belt splice and slipped and cut his leg. 3 sutures were required.
Employee was doing rehab bolting work outby and a piece of drawrock fell from the top and struck his hand and fractured his left index finger. Diagnosed with a small non-displaced fracture.
Using the hand wrench to remove a tight bolt. The bolt was located in a confined area of the machine and when technician pulled the wrench to remove the bolt his left shoulder made a snapping noise and started hurting. The pain increased that evening in to the next day.
