The geologist was collecting drill hole samples on a quarry bench and did not see a rock and stepped on it causing ankle to twist. This employee filed a report at that time and thought it would be ok. After some time the discomfort returned and the employee went to visit the DR. and the subsequent modified duty was issued.

Showing all 3Employee was called in to work (8pm) for a repair on the 154 mill. While trying to clean up the dispersant on the 154 mill, employee went to unroll the hose under the 153 motor deck. When the employee turned to pull the water hose to the 154 dispersant pumps, employee then lost footing when stepping on the grinding media that was on the floor.
Employee stepped off B Track Indexer. The water hose he was using to wash up was laying on the ground and he stepped on the hose. This caused his foot to roll to the inside and twisted his ankle. (Sprained ankle)
