Employee was decending loader with Lunch Bucket when he slipped and fell approximately 3 feet to the ground.
EMPLOYEE WAS HELPING MOVE A CONVEYOR WHICH WAS SUSPENDED BY A CHAIN ON A HOIST IN THE SHOP. THE LIGHTEST END GOT OUT OF CONTROL AND SWUNG AROUND AND HIT EMPLOYEE IN THE LEFT KNEE. THERE WAS NO ROOM FOR HIM TO GET OUT OF THE WAY.
HE HAS NOT TOLD US WHAT HAPPENED. EE CALLED THE OFFICE ON 3/28/03 WANTING THE WORKERS COMP. # TO GIVE TO THE DR. HE TOLD ME THAT HE HURT HIMSELF ON 3/21/03.
