A roof fall has occurred in the #6 entry on the northwest panel #5, 150 feet inby spad # 1788. It fell betrween the blocks between the last and nect to last break forming the bleeder across the panel. The fall measured 18 ft wide, 50 feet long adn 3 to 5 feet high. Persons, ventilation, equipment nor travel were impedded. This area wil not be rehabilitated.

A roof fall has occurred on the intake airway off of #1 section at spad # 4646. The fall measured 20 feet wide, 30 feet long, and 6 feet thick. Persons nor equipment were affected. Ventilation was affected. Employees were removed to the surface. A brattice line was moved over and another intake was established. This area has been dangered off.

Working the ratchet on a chain hoist when ratchet broke and the chain wrapped around his arm NOTE: EE worked did not file the inital incident report with the company until 8/12/2010. EE continued to work until November 16, 2010 at which time he went to see his doctor who restricted him from working.

Employee (Miner Operator) was cutting in #3 Entry on the 003 section. The continuous miner had dislodged a roof bolts while traming to the face. The result was that a piece of rock secured by the bolts fell and hit him.

EE was a passenger on a ride that collided with a mantrip on the tracks. EE sought medical attention at Pikeville Medical Center. He was given crutches and returned to work the following day on restricted duty.

EE was removing a canopy from a shuttle car. He was sitting under the canopy & when he removed a bolt, the canopy dislodged & struck him on the right shoulder. EE was taken to the hospital & was diagnosed with a compression fracture of the No. 12 vertebrae. He had a follow up visit with his family physician on 6/8/2010 and returned to work on 6/9/2010.

On 6/3/2010, after employee was suspended, he alledged that he was carrying his tool box and slipped on the track rail. He was seen by a doctor on 6/15/2010, but did not address when employee could work, but tests were scheduled. The company does not have any information as to whether this is a reportable event.

While installing a starter on a Cat 777C rock truck, the mechanic lost his grip and the starter slipped, pinching his finger between the starter and the engine. His middle finger on his right hand was pinched, requiring 2 stitches.

At approx. 6:05 am on 4/24/2010 I was told that our elevator went down with 1 employee stuck on it. He was able to get off at approx. 8:30am. The problem was a short in a door switch due to a bad trailing cable.

Employee was prying with a crowbar and the bar slipped causing pain in his right shoulder. He continued to work until 6/2/2010 at which time his doctor said he was unable to work. EE will have surgery for a tear in the rotator cuff. He is expected to be off work for approximately 20 weeks.
