Injured employee (Maintenance Oiler) was helping another maintenance employee lift & position the product pipe on top of Roller Mill #2 and twisted his left knee. Pre-existing condition is the primary contributing factor. He was evaluated by an orthopedic doctor on July 2 who placed him on restricted work duty (no lost time).

Raymond Mill Operator had just finished help spot a railcar hopper underneath the loading shoot. He was traveling on a metal walkway (catwalk). He alleges his steel-toed boot (left foot) tread (lugs) hung on the metal walkway's grating openings. He caught himself (handrails) but strained his left leg. Doctor diagnosed a possible torn tendon resulting in lost time.

Washing off spilled sand between #2 & #3 F-20 Storage Bins, slipped, & fell twisting right knee. Employee refused medical attention. On May 9, after working half the day, employee was examined by a doctor resulting in lost time (beginning May 10).

Filter Plant Helper was cleaning #2 Filter and the metal floor grating gave way. The employee partially fell causing injury to his right knee & left hand. On April 8, the employee did not seek medical attention. On April 17, the employee requested medical attention. The treating physician issued work restrictions resulting in lost time (no "seated" light duty available).

Dredge operator stood up in the john boat, lost his balance, fell, and hit his lower back on the john boat. Employee reported to management, appeared OK, & refused medical treatment. On Jan. 28 employee reported pain. On Jan. 29 employee was taken to a doctor. Doctor issued "no work" restrictions until Feb. 5. Although, employee is expected to return without restrictions.

Driver had pushed truck tarp arm back. It was raining heavily at the time. Driver was on scale catwalk, slipped and lost footing and fell. Driver's leg went down between catwalk and scale platform.

While moving a loaded railcar injured employee was using the wheel brake and collided with another railcar causing his head to strike the railcar he was standing on. Injured employee was not able to tighten the wheel brake fast enough. Injury is suspected a mild concussion and doctor put employee on lost time starting May 25.

Employee was moving a railcar and applying the wheel brake. The railcar did not stop completely when connecting to another railcar causing the employee to strike the railcar's railing. Employee bruised the right side of his ribcage (no fractures). Doctor issued 2-days of lost time. Employee returned to work full-duty on Monday, May 22.

Per the treating physician, metal shavings with some rust entered his left eye causing discomfort, blurred vision, & an infection the following week. It is unclear exactly when & how the injury took place. It is believed to be during a job on April 14 involving a cutting torch & grinder to remove nuts & bolts from a whizzer blade. Appropriate eye protection was being worn.

While attempting to open the discharge door to the No. 1 Dry Plant's hot tank with a 6-foot pry bar, the employee scrapped his left hand's knuckles on both the little & ring fingers. A deep laceration required staples and physician issued work restrictions.

While positioning No. 1 Roller Mill's 150-hp fan motor with forklift boom, chain, & cable, the motor became wedged. Three maintenance mechanics physically pulled & pushed and the motor came free, swung, and crushed one mechanic's left forearm (pinned between the gear box & drive coupling hub). The one injured mechanic required emergency medical treatment & surgery.

Interestingly, employee has a history of pre-existing back problem conditions (non-work-related minor league baseball and alleged work-related injury reported on July 6, 2004).

Rock breaker was sitting in the middle of a pile of big rock (150 - 300 lb); had to climb over them to fill rock breaker. On the way back down rock pile began to slide causing me to lose my balance and fall on rock. The edge was sticking up like a wedge and hit me in the spine. I lay there about 10 to 15 minutes with breath knocked out of me before returning to the truck.

While employee was descending the right side of the front-end loader he fell to the ground resulting in a left knee injury (inflammation). Doctor immobilized the left knee (knee brace) and issued work restrictions. Employee returned to work and was accomodated for his work restrictions. Employee will be re-examined on September 6 at 9:00 AM.

Treating physician did not immobilize the finger until August 3.

Employee was manually (by hand) turning the drive belts to a screw's drive motor to try to get the screw started. The belts pulled his right hand in between the belts and the pulley causing amputation (from tip to first joint) of his right hand ring finger. He did not follow a known & recognized safety practice (Lock-Out, Tag-Out, & Try-Out).

Injured employee was using a cast hammer mill hammer to try to open a slide gate. The employee slipped when swinging the hammer and struck his left hand with the hammer. The force and hammer's sharp edge amputated the tips of the 1st index and middle of the left hand. Rubber mallets are available for this and all workers had been informed to use only the rubber mallets.

Treating physician at the Hospital suspected a Hernia; however, Specialist confirmed as Groin Strain on April 26, 2005. Employee has work restrictions and has returned to work on Light Duty.

Employee was descending the Euclid E-25 Haul Truck's front access ladder and stepped off the ladder onto the ground and alleges he twisted his Left Knee causing a Strain. It was not clear until April 19, 2005 after the injured employee was seen by a Specialist whether the injury was work-related. Injured employee is having Left Knee Surgery April 27, 2005.

Injured employee was taking turns with another employee driving the hitch into a trailer's receiver with a 5-lb. hammer. While injured employee was standing behind and to the left of the other employee swinging the hammer, the hammer's wedge flew-out and struck the employee just below the chin causing a laceration that required sutures to close the wound.

Injured EE, #1 Loader Operator, was helping the Mechanic replace the #1 Loader's battery. Injured employee's right, little finger was accidentally caught between the new battery (weight about 50-lbs.) & the battery box on the loader. X-rays indicated fracture of employee's right, little finger and work restrictions were issued. Employee returned to work.

While removing plates in the 30" x 42" Austin Westin jaw crusher, one welded eye disconnected as the crane was lifting the plate. The welded eye struck the injured employee in the upper left chest area breaking #6 and #7 ribs.

Physicians have documented a "Fuller's Earth Pneumoconiosis," as of August 2, 2004. Employer attempted to obtain an "ILO Verification" of this diagnosis; however, the employee has recently obtained an attorney. In the interest of fulfilling statutory requirements, the company is reporting as a reportable illness until such time as the diagnosis can be properly confirmed.

HE WAS M0VING A LOADING HOSE FROM ONE HOLE TO ANOTHER. IT APPEARS THAT HE LIFTED AND TWISTED WITH HIS BACK AT THE SAME TIME. RESULT WAS PULLED MUSCLE.
